DURING NIGHT/IMC CONDITIONS, AND WHILE BEING VECTORED FOR THE ILS FINAL APPROACH, THE LEFT BOOST PUMP LIGHT ILLUMINATED. CONFIRMED BOTH BOOST PUMP SWITCHES WERE ON, ENGAGED BOTH EMERGENCY BOOST PUMP SWITCHES. APPROXIMATELY 10 SECONDS LATER, THE ENGINE BEGAN TO SURGE. SENSING EITHER PENDING ENGINE FAILURE OR SIMPLE FUEL STARVATION, SWITCHED BOTH TANKS TO THE INBOARD POSITION. THE LEFT ENGINE CONTINUED TO SURGE, IT SEEMED ANY ATTEMPT TO RESTORE FUEL FLOW WOULD BE UNSUCCESSFUL, FOLLOWED SINGLE ENGINE FAILURE PROCEDURES, SECURED THE ENGINE, DECLARED AN EMERGENCY AND CONTINUED THE FINAL APPROACH TO A SUCCESSFUL SINGLE-ENGINE LANDING. ON INSPECTION, DEBRIS WAS FOUND IN THE METERING ORIFICE. CLEANING AND SUBSEQUENT ENGINE PERFORMANCE RUNS FOUND THE ENGINE OPERATION NORMAL AND THE AIRCRAFT RETURNED TO SERVICE.
CLIMBING THROUGH 4,000 FEET OUT OF PDX, PILOT NOTED PROMINENT VIBRATION. PILOT TRIED TO DETERMINE WHICH ENGINE WAS SOURCE OF VIBRATION, BUT WITHIN 30 SECONDS, THE LEFT ENGINE STOPPED PRODUCING POWER (DID NOT SEIZE). PROPELLER WAS FEATHERED AND AIRCRAFT RETURNED TO PDX. ENGINE TEAR-DOWN REVEALED THAT NR 5 CONNECTING ROD HAD COME LOOSE. SUSPECT CAUSED BY THE BREAKING OF ONE OF THE CONNECTING ROD BOLTS.
AIRCRAFT STUCK ON RUNWAY DUE TO 'LOCKED BRAKES' ON TWO FLIGHTS. MAINTENANCE FOUND NO DEFECTS IN BRAKE SYSTEMS. FOUND PUSH TO TALK COIL CORD FROM PILOT'S YOKE WOULD WRAP AROUND PARKING BRAKE 'T' HANDLE AND ENGAGE PARKING BRAKE WHEN YOKE PULLED AFT.
BOOST PUMP INOPERATIVE. ELECTRIC MOTOR DOES NOT RUN.
TURBO SUPPORT BOLT FOUND BROKEN 3 INCHES FROM HEAD. BROKEN IN SHEAR. SUBMITTER STATED THIS IS RECURRING PROBLEM WITH UPPER LT MOUNT BOLT FOR TURBO SUPPORT ASSY. SUBMITTER SUGGESTS BOLT WITH MORE STRENGTH.
JANITROL HEATER FUEL PRESSURE REGULATOR WAS INSTALLED NEW 8-22-96. ON 9-19-96 WITH 20.1 HRS TIME IN SERVICE, UNIT FAILED CAUSING FLUCTUATION OF RT FUEL SYSTEM PRESSURE. WHEN REMOVED, FOUND FUEL LEAKING AROUND GASKET.
FUEL PRESSURE REGULATOR FOR COMBUSTION HEATER CAUSED FUEL PRESSURE GAUGE FLUCTUATIONS. EXAMINATION FOUND REGULATOR LEAKING EXTERNALLY. OPERATOR HAS RECORDS OF MULTIPLE FAILURES OF THESE UNITS.
A 6 INCH PIECE OF THE OUTBOARD WHEEL HALF WAS FOUND BROKEN OFF.
