CREW ENTRY DOOR ARMED LEVER IS EXTREMELY DIFFICULT TO LOCK. OPERATIONAL CHECK OF L1 DOOR IAW AMM 52-11-00-862-004, NOTED NO FAULTS.
CREW ENTRY DOOR LOCK LATCH STICKS TO STOW/LOCK. SOMETIMES CANNOT LOCK DOOR. ACCOMPLISHED INTERNAL AND EXTERNAL MANUAL OPERATION OF THE L1 ENTRY/SERVICE DOOR IN ACCORDANCE WITH B767 AMM 52-11-00. L1 DOOR OPENS, CLOSES, LOCKS AND UNLOCKS AS INTENDED.
CREW REPORTED ON TAXI OUT BOTH EICAS SCREENS BLANK FOLLOWED BY COMPACT MODE. UPPER EICAS BLANK THEN HOT ELECTRICAL SMOKE NOTED. CREW ELECTED A BLOCK RETURN FOR FAULT ISOLATION AND CORRECTIVE ACTIONS. AIRCRAFT GROUNDED FOR MAINTENANCE. GENERAL VISUAL INSPECTION VERIFIED UPPER DISPLAY UNIT HAD DISTORTED COLOR AND INDICATION. INTEGRITY AND SECURITY OF EQUIPMENT COOLING SOURCE TO DISPLAY UNIT GOOD. EHSI & EICAS INDICATOR ASSEMBLY REMOVED AND REPLACED. LEFT AND RIGHT EICAS COMPUTERS OPERATIONAL CHECKS GOOD PER BOEING 767-300 AIRCRAFT MAINTENANCE MANUAL 31-41-01 AND 31-41-02. NO ADDITIONAL FAULTS NOTED, AIRCRAFT RELEASED TO SERVICE.

PRE-DEPARTURE, CREW REPORTED FO WINDOW NOT FULLY CLOSING. STOPS AT "CLOSED" HANDLE 1 SHY OF FULLY CLOSED. SUBSEQUENT TO GENERAL VISUAL INSPECTION, LINE MAINTENANCE IDENTIFIED #2 WINDOW OUT OF ADJUSTMENT. WINDOW READJUSTED PER B767 AMM 56-11-02. OPERATIONAL CHECK GOOD. NO ADDITIONAL FAULTS NOTED.

POST FLIGHT CREW REPORTED, AFTER ARRIVING AT GATE STRONG RUBBER ODOR WAS NOTED IN THE FLIGHT STATION. INITIATED DEFERRAL MAINTENANCE ITEM 44639 OF THE LEFT AIR CONDITIONING PACK IAW B767 200/300 MINIMUM EQUIPMENT LIST 21-51-01-01. COMPLIED WITH CONDITIONAL INSPECTION FOR SMOKE/FUMES IN THE CABIN IAW B767 AMM 05-51-56 AND B767 AMM 21-00-21. NO FAULTS FOUND. AIRCRAFT RELEASED TO SERVICE. DOWNLINE MAINTENANCE REACTIVATED LEFT PACK SYSTEM AND OPERATED WITH APU LEFT AND RIGHT ENGINES INDEPENDANTLY. NO BURNT SMELL AND OPERATIONAL CHECK GOOD IAW B767 AMM 21-51-00. DEFERRAL MAINTENANCE ITEM 44639 CLEARED.
ON DESCENT PASSING THROUGH 11000 UNTIL 9000, CREW REPORTED A HARSH RUBBER SMELL IN THE FLIGHT STATION. LANDING WAS WITHOUT INCIDENT AND NO PRECAUTIONARY ACTIONS. ON ARRIVAL, ODOR DISSIPATED. AIRCRAFT WAS GROUNDED FOR UNSCHEDULED MAINTENANCE. COMPLIED WITH A DETAILED VISUAL INSPECTION OF LEFT AND RIGHT ENGINES FOR FLUID LEAKS AND INSPECTED PNEUMATIC DUCTING IAW B767 AMM 75-00-00. NO DEFECTS NOTED. ADDITIONALLY, COMPLIED WITH CONDITIONAL INSPECTION SMOKE/FUMES IN THE CABIN IAW B767 AMM 05-51-56 AND REMOVAL OF OIL CONTAMINATION FROM AIR CONDITIONING AND PNEUMATIC SYSTEMS IAW B767 AMM 21-00-21. NO FAULTS FOUND.
ENGINE WAS REMOVED DUE TO END OF LEASE AND TRANSPORTED TO ENGINE SERVICE FACILITY FOR OVERHAUL. ACCORDING TO THE ENGINE TEARDOWN REPORT A LINE INSPECTOR FOUND A BOLT BETWEEN THE RADIAL DRIVE SHAFT (RDS) AND THE FAN FRAME. THE BOLT HAS CAUSED MAJOR DAMAGE TO THE RDS DURING ENGINE OPERATION. PLUG D742 RUBBED THROUGH IDG CABLE SLEEVE. CREATING A SHORT TO GROUND THROUGH THROTTLE CABLE TRB-3, SEVERING CABLE RESULTING IN RT THROTTLE STAYING AT 93.3, N1. R & R CONNECTOR D742, RT ENG ANTI-ICE PRESS SWITCH, THROTTLE PUSH-PULL CABLE & IDGR CABLE. IDG & DISCONNECT GOOD, TAI VALVE GOOD, IDLE TO TAKEOFF PWR, OPS NORM IAW AMM 71-00-00.
