MAIN LANDING GEAR LEAF SPRING INNER ATTACH BOLT P/N NAS1106-24 ON LH SIDE OF AIRCRAFT, FAILED MID SHANK ABOVE THE THREADS, 50HR AFTER INSTALLED DURING REPETITIVE INSPECTION BROUGHT ON BY MANDATORY SERVICE BULLETIN-99. BOLT FAILURE CAUSED DEFORMATION OF THE FUSELAGE TO LEAF SPRING CLEVIS AND DEPARTURE OF LH MLG FROM AIRCRAFT WHILE TAXIING AFTER LANDING.
LEFT MAIN TIRE WENT FLAT UPON TAXIING IN FROM LANDING. TIRE AND INNER TUBE REPLACED WITH NEW. TIRE WAS A 7.00 X 6, 8PR. INNER TUBE TR20, S02602. TIRE AND TUBE TIME IN SERVICE: 219.5, LANDINGS APPROX.:400. INNER TUBE FAILURE WAS A SLIT ON SIDEWALL DEVELOPING RADIALLY ALONG A MFG BONDING LINE APPROXIMATELY 1" LONG, RT MAIN TIRE AND TUBE CHANGED 16 HOURS LATER AND SAME MAKE AND MODEL INNER TUBE FOUND WITH SIMILAR SPLIT DEVELOPING ALONG MFG BONDING LINE. INNER TUBE TT IN SERVICE: 94.3HRS, APPROX. 180 LANDINGS.
A CRACK WAS DISCOVERED IN THE CRANKCASE AT THE LOWER FORWARD CYLINDER BASE STUD OF THE NR 2 CYLINDER OF APPROXIMATELY 1.5 INCHES. STARTS FROM THE BOTTOM OF STUD AREA AND TRAVELS DOWN AND AFT. A SLIGHT AMOUNT OF OIL LEAKING FROM AROUND THE BASE OF CYLINDER AT THIS AREA.
DURING 100 HR INSPECTION FOUND FATIGUE CRACKS ON FRAME MEMBERS OF FUSELAGE SECTION NR 8 AT MAIN LANDING GEAR FORWARD TRUSS MOUNTING BRACKET ON BOTH LEFT AND RIGHT SIDE. THESE CRACKS EMINATED FROM BOTTOM BOLT HOLES AND AT OUTSIDE FLANGE RADIUS OF FRAME. THEY BECAME VISIBLE AFTER TRAVELING PAST THE MOUNTING BRACKET (APPROX .5 INCH).
