Maintenance disclosures
AFTER TAKEOFF WHEN RETRACTING THE GEAR, THE LEFT MAIN UNLOCK WAS ILLUMINATED ON THE MAIN PANEL AS WELL AS THE OVERHEAD PANEL. REMOVED AND REPLACED LH UPLOCK BOX, OPS CHECK GOOD, SERVICED GREEN AND BLUE ACCUMULATORS WITH NITROGEN.
AFTER TAKEOFF LANDING GEAR HANDLE STAYS LOCKED IN POSITION ON THIRD ATTEMPT IT MOVED TO THE UP POSITION. CORRECTIVE ACTION REMOVED AND REPLACED LANDING GEAR CONTROL LEVER OPS CHECK GOOD.
CREW DISCOVERED LEFT MAIN TIRE BLOWN AND INDICATION OF MAIN BERING FAILURE. AFTER REMOVAL OF WHEEL FOUND THAT THE AXLE IS DAMAGED AND REQUIRES REPLACEMENT.
FLIGHT 51 TLH/TPA FLAP STUCK AT 15 DEGREES DECLARED EMERGENCY. CORRECTIVE ACTION: PERFORMED FUNCTIONAL TEST OF FLAP CONTROL SEVERAL TIMES FOUND SYSTEM WORKING PROPERLY AT ALL DEGREES.
FLIGHT: 62, LANDING GEAR HANDLE WOULD NOT RETRACT ON TAKEOFF, AFTER 10-12 ATTEMPTS IT FINALLY MOVED UPWARD. REMOVED AND REPLACED LANDING GEAR HANDLE OPERATIONAL CHECK OK.
