Maintenance disclosures
DURING SID 57-10-16 INSPECTION, 3 AREAS OF CORROSION WERE IDENTIFIED ON THE RIGHT SPAR STRAP OF N402VN. THESE AREAS ARE LOCATED ON THE OUTBOARD END OF THE STRAP BETWEEN APPROXIMATE WING STATIONS, 145.87-155.62, AND ARE BEYOND LIMITS FOR REPAIR AS OUTLINED IN SID 57-10-16. THIS STRAP HAS BEEN REPLACED USING NEW PARTS.
LEFT AILERON SKIN DE-LAMINATED IN FLIGHT. THE FLIGHT LANDED SAFELY WITH NO EMERGENCY ACTION. THE AILERON AND TRIM TAB WERE REPLACED WITH NO FURTHER DEFECTS NOTED.
PILOT REPORTED ON START-UP THE LEFT ENGINE STARTER STUCK. MAINTENANCE INSPECTED AND FOUND THE STARTER RELAY STUCK IN THE CLOSED POSITION. THE STARTER RELAY WAS REPLACED WITH NO FURTHER DEFECTS NOTED.
PILOT REPORTED RIGHT ENGINE SURGES, SHUTTERS AND VIBRATES ON ALL POWER SETTINGS. MAINTENANCE INSPECTED AND FOUND CYLINDER NR 4 WEAR IN EXHAUST VALVE KEEPER AREA, AS WELL AS INTAKE AND EXHAUST LIFTERS NOT WORKING PROPERLY. REMOVED AND REPLACED CYLINDER #4 AS WELL AS THE INTAKE AND EXHAUST LIFTERS WITH NO FURTHER DEFECTS NOTED.
PILOT REPORTED THE LEFT MAIN GEAR WOULD NOT RETRACT. MAINTENANCE INSPECTED AND FOUND THE LEFT MAIN LANDING GEAR UPLOCK HOOK BROKEN. THE PART WAS REPLACED WITH NO FURTHER DEFECTS NOTED.
PILOT REPORTED, HEATER GIVES A MELTING PLASTIC SMELL IN FLIGHT. MAINTENANCE INSPECTED AND FOUND THE HEATER COMBUSTION BLOWER HAD SEIZED, THE BLOWER WAS REPLACED WITH NO FURTHER DEFECTS NOTED.
WHILE PERFORMING OTHER MAINTENANCE, A CRACK WAS DISCOVERED IN THE RT CANTED BEAM ASSEMBLY (CROSSOVER BEAM) ON THE AFT RIGHT HAND SIDE, AROUND ONE OF THE TURBO BRACKET MOUNTING HOLES. IT APPEARS THE CRACK WAS CAUSED BY PRE-LOADING OF THE TURBO BRACKET. BEAM ASSEMBLY WAS REPLACED WITH FACTORY NEW PART. SEE ATTACHMENT FOR PICTURES AND CRACK LENGTH.
DURING A ROUTINE INSPECTION, SIX CRACKS WERE FOUND IN THE LT CLOSE OUT RIB (P/N 5122155-55.) THESE CRACKS WERE LOCATED AT EACH OF THE SIX VERTICAL ATTACHING HOLES OF THE FORWARD ROW OF THE AFT TRUNNION SUPPORT. EACH CRACK IS TWO INCHES IN LENGTH. UPON REMOVAL, IT WAS NOTED THAT HOLES IN THE RIB WERE NO LONGER IN TOLERANCE FOR THE ATTACHING HARDWARE.
DURING A ROUTINE INSPECTION, A CRACK (.250 IN LENGTH) WAS FOUND IN THE RT CLOSE OUT RIB (P/N 5122155-56.) THIS CRACK WAS FOUND IN THE FORWARD LOWER MOST HOLE OF THE FORWARD TRUNNION SUPPORT. UPON REMOVAL, ALL FASTENERS HOLES WERE FOUND TO BE WITHIN TOLERANCE FOR THEIR ATTACHING HARDWARE.

AT SCHEDULED INSPECTION, NLG DRAG BRACE, PN 5142002-5 WAS FOUND TO BE CRACKED IN THE AREA OF NLG ACTUATOR ATTACH POINT. METHOD OF INSPECTION WAS DYE PENETRANT. THIS CRACK WAS FOUND TO BE BEYOND LIMIT ALLOWABLE OF MEB91-11R1. THIS UNIT HAS BEEN SCRAPPED & A REPLACEMENT DRAG BRACE HAS BEEN INSTALLED IN ACFT.
AT SCHEDULED INSPECTION, NLG DRAG BRACE, PN 5142002-5, WAS FOUND TO BE CRACKED IN THE AREA OF NLG ACTUATOR ATTACH POINT. METHOD OF INSPECTION WAS DYE PENETRANT. THIS CRACK WAS FOUND TO BE WITHIN ALLOWABLE LIMIT OF MEB91-11R1. THIS UNIT HAS BEEN REPAIRED IAW PROCEDURE OUTLINED IN MEB941-11R1 & RETURNED TO SERVICE.
THE PILOT REPORTED THAT ON APPROACH, THEY TURNED ON THE INSTRUMENT PANEL LIGHTS. SHORTLY AFTER TURNING THE PANEL LIGHTS ON, THEY SMELLED AN ELECTRICAL BURNING SMELL AND NOTICED A SMALL AMOUNT OF SMOKE COMING OUT OF THE RT RHEOSTAT. THEY IMMEDIATELY TURNED THE LIGHTS OFF, PULLED ALL LIGHT BREAKERS ASSOCIATED WITH INTERIOR LIGHTING AND THE SMOKE STOPPED. LANDED WITHOUT INCIDENT. MX INSPECTED THE SWITCH WITH NO DAMAGE NOTED. A SMALL AMOUNT OF LINT WAS CLEANED FROM THE SWITCH. THE UNIT THEN WAS OPERATIONALLY TESTED FOR SEVERAL MINUTES AND WAS FOUND TO BE SATISFACTORY.

THIS CRACK WAS FOUND TO BE BEYOND LIMIT ALLOWABLE OF MEB91-11R1. THIS UNIT HAS BEEN SCRAPPED & A REPLACEMENT DRAG BRACE HAS BEEN INSTALLED IN ACFT.
CREW REPORTED, RT MAIN GEAR SHEARED OFF ON LANDING ROLL. MX FOUND RT MAIN GEAR UPPER BARREL AND TRUNNION ASSY SHEARED JUST BELOW MLG ACTUATOR ATTACH POINT. MX IS IN THE PROCESS OF REPLACING RT MLG ASSY, RT ENGINE, RT PROPELLER, RT SIDE INBD AND OTBD FLAP PANEL ASSEMBLIES.
DURING FLIGHT, CREW NOTICED FUEL ESCAPING FROM RT UPPER ACCESS PANEL. ENGINES RUNNING NORMAL, EMERGENCY WAS DECLARED, LANDED WITHOUT INCIDENT. MECHANIC REMOVED AND REPLACED PANEL SEALS AND RELEASED AIRCRAFT BACK TO SERVICE.
AT REGULAR SCHEDULED INSPECTION, MECHANIC FOUND NOSE GEAR, DRAG BRACE CRACKED AT ACTUATOR ATTACH HOUSING. REMOVED AND REPLACED DRAG BRACE AND RETURNED AIRCRAFT TO SERVICE.
