DURING ROUTINE MAINTENANCE IN ANCHORAGE, AK (ANC), MAINTENANCE NOTED THAT (2EA.) FASTENERS COMMON TO THE HORIZONTAL STABILIZER LOWER SKIN & SPAR CHORD HAVE CRACKED OUT. THE CRACKS MEASURE .204” AND .242” LONG, RESPECTIVELY, AND THEY EMANATE IN THE OUTBOARD DIRECTION FROM THE FASTENER HEADS.

DURING ROUTINE OPERATIONS IN ANCHORAGE, AK (ANC), MAINTENANCE DISCOVERED DAMAGE TO THE ENDS OF LOWER FRAME 13 & 16 INNER FRAME T-CHORD FLANGES AT LBL19 & RBL19, NEAR THE SEAT TRACK SUPPORTS. THE T-CHORD FLANGE CORNERS WERE LOCALLY BENT DOWN WITH GOUGING ON THE TOP SIDE CAUSED BY CONTACT WITH A LONG FLOORBOARD SCREW DURING INSTALLATION. CRACKING WAS ALSO NOTED ALONG THE FILLET RADIUS OF THE FRAME T-CHORD ADJACENT TO THE DEFORMED AREAS. THE DAMAGE IS OUT OF LIMITS PER SRM APPENDIX B.

MULTIPLE CIRCUIT BREAKERS ON OVERHEAD PANEL POPPED IN FLIGHT. FLIGHT CREW CONTINUED TO DESTINATION AND LANDED WITHOUT INCIDENT. BREAKERS THAT TRIPPED INSTRUMENT LIGHT, ANTI SKID AND TEST, RADIO POWER AND NR 2 INVERTER OUTPUT 26 VAC AND 115 VAC. DETERMINED THE WIRING CHAFED AT ANTI-SKID CONTROL SWITCH AREA. FOR CORRECTION ACTION PERMANENT REPAIRS AND RE-ROUTING OF WIRING BUNDLE WERE ACCOMPLISHED.

RUDDER SERVO FAILED 4.4 HOURS AFTER INSTALL. RUDDER WENT HARD OVER TO THE LT AND JAMMED IN PLACE. ONCE POWER WAS REMOVED THE PILOTS COULD AT LEAST MOVE THE RUDDER EVEN THOUGHT THE CLUTCH WAS STILL ENGAGED. UPON TEARDOWN FOUND CLUTCH HUB SCREWS HAD BACKED OUT CAUSING CLUTCH ENGAGEMENT WITH NO POWER APPLIED. APPARENT MECHANICAL FAILURE DUE TO HUB RETAINING SCREWS BACKING OUT; MOST LIKELY FROM VIBRATION DURING CLUTCH OPERATION. REPAIRED UNIT AND INCORPORATED SB 7002260-22-A0013.
