Maintenance disclosures
AFTER TAKEOFF, DURING CLIMB AT APPROXIMATELY 12,000 FEET THE LT ENGINE SN CD0072 SHUTDOWN WITH A VERY LOUD NOISE OCCURRING AND AT THE SAME TIME A VERY DESTINCT VIBRATION UNTIL THE ENGINE ROTAION STOPPED. IMMEDIATELY THE ACFT WAS RETURNED TO FIELD WITH NO INCIDENT. A PRELIMANARY BORESCOPE INSPECTION REVEALED DAMAGE TO THE FIRST, SECOND, THIRD, FOURTH AND FIFTH STAGE HIGH PRESSURE TURRBINE SECTIONS. IT HAS NOT BEEN DETERMINED AT THIS TIME THE SPECIFIC CAUSE OF THIS EVENT.
THE HONEYWELL DU-870 DISPLAY UNIT REFERENCED HEREIN FAILED JUST PRIOR TO TAKEOFF. THE SUSPECTED SPECIFIC COMPONENT FAILURE IS THE HIGH VOLTAGE POWER SUPPLY P/N:7018704-902 INSTALLED INSIDE THE CRT DISPLAY UNIT. FURTHER INSPECTION WILL BE REQUIRED TO VERIFY THE EXACT CAUSE OF DU FAILURE.

VIBRATION IN NOSE LANDING GEAR DURING CLIMB. RTF. MX PERFORMED DETAILED INSPECTION OG NLG, NOSE GEAR BAY, NOSE GEAR DOORS, RADOME. NO DEFECTS NOTED, FOUND NLG TIRES OUT OF ROUND. REMOVED AND REPLACED LT & RT TIRES IAW DORNIER 328 JIC 32-41-01, LANDING GEAR INSPECTION JIC 32-11-00.
ABORTED TAKEOFF DUE TO BLEED SOV FAILURE (HPSOV DID NOT OPEN AND LPSOV DID NOT OPEN ABOVE 80 PERCENT). MX INSPECTED BOTH ENGINES AND PERFORMED FUNCTIONAL CHECK FOR BOTH BSOV IAW JIC 71-03-03. NO FURTHER DISCREPANCIES AT THAT TIME.
CENTER FLOOR EMERGENCY LIGHTS INOP. RECYCLED POWER. OPS CHECK GOOD.
DURING INSPECTION THE EMERGENCY LIGHTS WERE FOUND INOPERATIVE. MAINTENANCE PERSONNEL FOUND THE FLIGHT ATTENDANTS EMERGENCY LIGHT SWITCH IN THE ON POSITION AND RECHARGED THE EMERGENCY BATTERIES VIA AIRCRAFT DC POWER. A SATISFACTORY OPERATIONAL CHECK WAS PERFORMED AND AIRCRAFT WAS RELEASED FOR CONTINUED SERVICE.
DURING MAINTENANCE INSPECTION THE THIRD SECTION OF EMERGENCY TRACK LIGHTING WAS FOUND INOPERATIVE. MAINTENANCE PERSONNEL REMOVED AND REPLACED THE EMERGENCY POWER SUPPLY. A SATISFACTORY OPERATIONAL CHECK WAS PERFORMED AND AIRCRAFT WAS RELEASED FOR CONTINUED SERVICE.
DURING MAINTENANCE INSPECTION THE NR 7 EMERGENCY BATTERY WAS DISCOVERED INOPERATIVE. MAINTENANCE PERSONNEL REMOVED AND REPLACED THE EMERGENCY POWER SUPPLY. A SATISFACTORY OPERATIONAL CHECK WAS PERFORMED AND AIRCRAFT WAS RELEASED FOR CONTINUED SERVICE.
DURING PREFLIGHT INSPECTION, THE CREW REPORTED THAT THE EMERGENCY EXIT LIGHTS AT SEAT ROWS 9 AND 10 WERE INOPERATIVE. MAINTENANCE PERSONNEL RESET THE CIRCUIT BREAKER FOR THE EMERGENCY EXIT LIGHTS. AN OPERATIONAL CHECK WAS SATISFACTORY AND THE AIRCRAFT WAS RETURNED TO SERVICE.
DURING PREFLIGHT INSPECTION, THE CREW REPORTED THAT THE EMERGENCY FLOOR TRACK LIGHTS WERE INOPERATIVE FROM SEAT ROW 4 THROUGH 6. MAINTENANCE PERSONNEL SECURED A LOOSE CONNECTOR FOR THE EMERGENCY EXIT LIGHTS. AN OPERATIONAL CHECK WAS SATISFACTORY AND THE AIRCRAFT WAS RETURNED TO SERVICE.
DURING CLIMB, THE CREW REPORTED HEARING A LOUD BANG FOLLOWED BY AIRFRAME VIBRATION. THE FLIGHT RETURNED TO BOS AND LANDED WITHOUT FURTHER INCIDENT. MAINTENANCE PERSONNEL FOUND THAT THE OXYGEN SERVICE FAIRING PANEL HAD DEPARTED THE AIRCRAFT. THE PANEL WAS REPLACED AND THE AIRCRAFT WAS RETURNED TO SERVICE.
