CREW REPORTED THE RIGHT ENGINE HAD AN OIL LEAK. MAINTENANCE INSPECTED THE ENGINE, BORESCOPED THE CYLINDERS AND FOUND THE #5 CYLINDER TO HAVE EXCESSIVE OIL POOLING AND PITTING ON THE EXHAUST VALVE. THE CYLINDER WAS REPLACED WITH NO FURTHER DEFECTS NOTED.
DURING AN ENGINE CHANGE, THE LEFT EXHAUST PIPE WAS FOUND TO BE CRACKED. THE PIPE WAS REPLACED WITH NO FURTHER DEFECTS NOTED.
PILOT REPORTED DURING CRUISE, THE LEFT ENGINE ONLY PRODUCING 59% POWER AT 2100 RPM. MAINTENANCE INSPECTED AND FOUND THE LEFT ENGINE #6 CYLINDER TO HAVE TWO BROKEN CYLINDER STUDS. THE ENGINE WAS REPLACED WITH NO FURTHER DEFECTS NOTED.
PILOT REPORTED LEFT ENGINE NOT PRODUCING FULL POWER AT 2500 FT, 28" MP AND 2515 RPM. MAINTENANCE INSPECTED AND FOUND THE LEFT REAR EXHAUST PIPE FLANGE BROKEN LEADING TO TRANSITION BLOCK. THE PART WAS REPLACED WITH NO FURTHER DEFECTS NOTED.
DURING SCHEDULED INSPECTION MAINTENANCE FOUND THE LEFT ENGINE EXHAUST TRANSITION PIPE CRACKED. THE TRANSITION WAS REPLACED WITH NO FURTHER DEFECTS NOTED.
THROUGH REVIEW OF ENGINE PERFORMANCE DATA IT WAS DISCOVERED THE RIGHT ENGINE WAS OVER SPED BY MORE THAN 10% DURING FLIGHT TRAINING DOING BACK-TO-BACK PRACTICES OF SIMULATED ENGINE OUT OPERATIONS. PER MANUFACTURE'S GUIDANCE THE ENGINE WILL BE REPLACED.
PILOT REPORTED LEFT OIL PRESSURE DROPPED IN FLIGHT WITH BLACK SMOKE. MAINTENANCE INSPECTED AND FOUND EXCESSIVE OIL IN THE LEFT ENGINE #4 CYLINDER. REPLACED CYLINDER WITH NO FURTHER DEFECTS NOTED.
PILOT REPORTED BURNING SMELL WHEN AVIONICS TURNED ON. MAINTENANCE INSPECTED AND FOUND PFD #1 COOLING FAN LT SIDE OF INSTRUMENT PANEL INOPERATIVE. THE FAN IS BEING REPLACED
PILOT REPORTED A SMOKE SMELL IN COCKPIT. MAINTENANCE FOUND PILOT LEG HEATER FAN TO BE INOPERATIVE. THE FAN WAS PLACED ON MEL AND WILL BE REPLACED.
THE PILOT REPORTED A BURNING SMELL AND SMOKE IN THE CABIN DURING CRUISE. MAINTENANCE INSPECTED AND FOUND THE NR 1 PFD FAN INOPERATIVE, AND REMOVED FOR FURTHER INSPECTION. THE FAN WAS FOUND TO HAVE A BURNED ELECTRICAL SMELL COMING FROM THE FAN ASSEMBLY AND THEN REPLACED WITH NO FURTHER DEFECTS NOTED.
THE NLG ATTACH STRUCTURE WAS FOUND TO BE DAMAGED DURING AN INSPECTION INITIATED DUE TO SIMILAR FINDINGS ON OTHER COMPANY AIRCRAFT. THE DAMAGE IS LOCATED AT FWD MOST FUSELAGE BULKHEAD, LWR CENTER SECTION, IN THE AREA CORRESPONDING TO THE NLG MOUNTS. THE STRUCTURE AFT OF NLG BULKHEAD WAS INSPECTED WITH THE FOLLOWING DEFECTS NOTED: 1) THE FWD SECTIONS OF FLOORBOARD SUPPORTS AS THEY ARE RIVETED TO FORWARD-MOST FUSELAGE BULKHEAD ARE CRACKED. TECNAM SB387-CS EDITION 1 REVISION 0 PRESCRIBES REPAIR AND MODIFICATION OF THESE AREAS AND WILL SERVE AS CORRECTIVE ACTION FOR THIS DAMAGE.
