BLEED AIR HEAT INOP, NO AIRFLOW AND HOT SMELL WHEN BLEED AIR HEAT IS TURNED ON. MECHANICS FOUND SCAT TUBE AT CABIN AIR VALVE HAD COME LOOSE. RE-SECURED IAW 208MM 21-41-00, OPS CHECK GOOD.
HOT ELECTRICAL SMELL. SMELL DISAPEARED ON TAXI BACK TO RAMP. SUSPECTED THAT DEICE FLUID WAS SPRAYED ON EXHAUST, MECHANICS INSPECTEDT ENGINE BAY, JUNCTION BOX, CIRCUIT BREAKER PANELS AND ALL WIRES BEHIND INSTRUMEENT PANEL, NO DEFECTS DETECTED.. PERFORMED RUNUPS, NO SMELL DETECTED.
FIRE ALARM ACTIVATED WITH AUDIBLE PORTION ONLY , (NO LIGHT ILLUMINATED ON INSTRUMENT PANEL). FIRE DETECT WARNING HORN DEFERRED. NO FURTHER INFORMATION AVAILABLE AT THIS TIME.
LEFT BRAKE ASSEMBLY BROKEN AND PARTIALLY DRAGGING. MAINTENANCE INSPECTED BRAKE ASSEMBLY AND DETERMINED THAT THE BOLT(PIN) HAD BROKEN AND CAUSED THE BRAKE ASSEMBLY TO CONTACT TIRE ASSEMBLY AND CAUSED IT TO START DRAGGING. DUE TO THIS EVENT THE FOLLOWING ITEMS WERE REPLACED: TIRE ASSEMBLY, BRAKE ASSEMBLY AND TORQUE PLATE ASSEMBLY.
LEFT TIRE ASSEMBLY STOPPED ROTATING UPON LANDING. INSPECTED & DETERMINED THAT ONE OF THE TWO ANCHOR BOLTS, PIN, ON THE BRAKE ASSEMBLY HAD BROKEN & CAUSED THE BRAKE ASSEMBLY TO JAM AGAINST THE TIRE ASSEMBLY THUS PREVENTING THE TIRE ASSEMBLY FROM ROTATING. REPLACED DEFECTIVE BRAKE ASSEMBLY. ALSO REPLACED TIRE ASSEMBLY DUE TO DAMAGE THAT OCCURRED DURING THIS EVENT. THE BRAKE ASSEMBLY HAD ACCUMULATED 1516.4 SINCE IT WAS INSPECTED & SERVICED ON THE BENCH. THE DATA IN 5G & 6G OF THIS REPORT IS NOT ACCURATE, THOSE TIME ARE NOT KNOWN. SUBMITTER ONLY INSERTED THOSE NUMBERS SO THAT THE REPORT COULD BE FILED OTHERWISE THE COMPUTER WOULD NOT ACCEPT REPORT. AFT CANTED BULKHEAD AT BS 474.4 CRACKED AT ELEVATOR PUSH PULL TUBE OPENING. DURING REMOVAL AFT VERTICAL SPAR WEB (2631021-12) WAS FOUND TO HAVE EXCESSIVE FRETTING, VERTICAL SPAR WEB ALSO REPLACED.
DURING ENGINE MAINTENANCE, 2 CRACKS APPROX .5 INCH LONG WERE DISCOVERED ON THE UPPER ENGINE MOUNT BRACKET ASSY-COWL ATTACH EMANATING FROM THE BOLT HOLE. A NEW BRACKET AND BOLT WERE INSTALLED AND AIRCRAFT WAS RETURNED TO SERVICE.
