Maintenance disclosures
DEPARTED YIP TO EYW. DURING TAKE-OFF AFTER V1 THE CREW EXPERIENCED A FAILURE OF THE LEFT CONVERTER (AC&DC). THE TAKE-OFF WAS CONTINUED, ATC WAS NOTIFIED OF THE STATUS AND VECTORED THE CREW BACK TO YIP. THE PIC FLEW THE PLANE AND HANDLED THE RADIO. THE SIC CONDUCTED THE SINGLE CONVERTER FAIL AC-DC CHECKLIST. AFTER THE CHECKLIST WAS COMPLETE THE CREW ASKED FOR AND RECEIVED VECTORS TO THE ILS 5R AND THEN CONDUCTED A NORMAL AND UNEVENTFUL APPROACH AND LANDING. THE TOWER DECLARED AN EMERGENCY FOR THE CREW BUT NO ASSISTANCE WAS REQUIRED AND AFTER THE LANDING THEY TAXIED NORMALLY BACK TO THE FBO.
UPON DEPARTURE FROM ROOFTOP PAD, NR 1 ENGINE TORQUE EXCEEDED 86.4 PERCENT AND NR 2 ENGINE TORQUE WAS NEAR ZERO WITH DECREASING RPM. ENDED UP THE ENG RIGGING NEEDED TO BE ADJUSTED.
PANEL ASSY IS CRACKED. APPROX. 10 INCH CRACK STARTING @ R/H AFT CORNER GOING FOWARD AND TOWARDS THE LEFT. THIS CRACK HAPPENED DUE TO STA. 233 FRAME ASSY. P/N: 222-031-503-101 CRACKING. PERFORMED BHT FIELD REPAIR PROCEDURE 96R1-037, D.O.Y. S.O.C. SERIAL # A4266, DATED 11-27-96.
PART IS CRACKED. ROOF WAS CRACKED DUE TO FRAME ASSY P/N: 222-031-503-101 (STA 233) CRACKING. INSTALLED STRAP DOUBLER P/N: 222-031-517-179 I/A/W BHT TB 222U-91-56, DATED 1-15-91.
FRAME ASSY IS CRACKED. CRACKED @ R/H CORNER THRU FASTNER HOLES. PERFORMED REPAIR I/A/W BHT FIELD REPAIR PROCEDURE 96R1-036 REV. A D.O.T. S.O.C. SERIAL # A4753, DATED 3-19-97.

MAST TORQUE METER FAILED AFTER 604.9 HOURS IN SERVICE. SUBMITTER STATES EXPERIENCING FREQUENT SENSOR FAILURES.
ISOLATION MOUNT IS BROKEN. THREADED BUSHING THAT ROD END ASSY THREADS INTO IS SEPARATED FROM ELASTOMERIC MATERIAL. DATE OF MANUFACTURE OF THIS PART WAS 5-29-93.
