Maintenance disclosures
THE FLIGHT CREW REPORTED RECEIVING A "RIGHT FADEC MINOR" MESSAGE. THE MAINTENANCE TEAM DETERMINED THE RIGHT NO2 ENGINE IGNITOR BOX WAS DEFECTIVE. THE UNIT WAS REMOVED AND REPLACED WITH AN OVERHAULED IGNITOR. OPS CHECKS WERE SATISFACTORY AND THE AIRCRAFT WAS RETURNED TO SERVICE.
THE CREW REPORTED THAT THEY RECEIVED RT THRUST REVERSER FAIL CAS MSG ILLUMINATED DURING LANDING. THE THRUST REVERSER WOULD NOT FULLY DEPLOY. TROUBLESHOOTING FOUND THE RT SECONDARY LATCH ACTUATOR PN 288D0487-511 DEFECTIVE. THE LATCH ACTUATOR WAS REPLACED WITH REPAIRED UNIT. FOUR EACH THRUST REVERSER GUIDE RAIL RODS WERE BENT BECAUSE OF THE FAILURE OF THE SECONDARY LATCH & WERE REPLACED.
THE CREW REPORTED THAT DURING PREFLIGHT CHECKS IT WAS DISCOVERED THAT THE LT ENG FIRE SHUTOFF VLV NOT INDICATING CLOSED. THE DEFECTIVE VLV SN 001273 WAS REMOVED FROM SERVICE & AN OVERHAULED UNIT WAS INSTALLED. LEAK & FUNCTIONAL CHECKS WERE SATISFACTORY.
DURING POST FLIGHT CHECKS, BLUE WATER WAS NOTED DRIPPING FROM ACFT BELLY PANEL. INVESTIGATION FOUND THE LAVATORY WASTE DRAIN DUCT CRACKED INBD OF THE LAVATORY SERVICE PANEL. THE LAVATORY DRAIN DUCT WAS REMOVED & IT WAS NOTED THAT THERE WERE SEVERAL ATTEMPTS TO REPAIR CRACKS IN THE WALL OF DUCT. THE DUCT WAS REPLACED WITH NEW.
FLIGHT CREW REPORTED THAT THE MAIN ENTRY DOOR LIFTING SYSTEM WAS INOPERATIVE. TROUBLESHOOTING IDENTIFIED THE DOOR LIFT CABLE BROKE. THE SYSTEM WAS DISABLED WHILE MFG SEARCHED THE WORLD FOR A REPLACMENT CABLE. POOR PARTS SUPPORT, 6 WEEKS FOR REPLACEMENT CABLE TO BE DELIVERED.
EICAS DISPLAYED DE-ICE SYS AND DE-ICE SYS ALT, WHEN PNEUMATIC BOOT DEICING SYS WAS SELECTED ON NORM AND ALTERNATE. MX TROUBLESHOOTING IDENTIFIED THE PRESSURE REGULATOR VALVE AS DEFECTIVE. VALVE WAS REMOVED AND REPLACED WITH NEW VALVE. FUNCTIONAL CHECKED SATISFACTORY. ACFT RETURNED TO SERVICE.
CREW REPORTED APU HAD AN UNCOMMANDED SHUTDOWN. TROUBLESHOOTING IDENTIFIED THE APU OIL TEMP SENSOR AS DEFECTIVE. A NEW SENSOR WAS INSTALLED, FUNCTIONAL AND LEAK CHECKS WERE SATISFACTORY.
DURING CRUISE, CREW REPORTED THE NR 1 FMS SHUTDOWN, AUTOPILOT DISENGAGED AND VISIBLE SMOKE APPEARED FROM THE LT SIDE OF THE INSTRUMENT PANEL. AFTER EVALUATION, DIVERTED. AN EMERGENCY WAS DECLARED, & LANDED. THE NR 1 FMS WAS FOUND TO BE DEFECTIVE AND WAS REPLACED WITH AN INSPECTED UNIT. OPS CHECKED SATISFACTORY.
