Maintenance disclosures
CRANKSHAFT HAD BEEN ULTRASONIC INSPECTED IAW APPLICABLE CSB 99-6A DURING OVERHAUL. RETURNED TO SERVICE AND AFTER 141.1 HOURS, FAILED IN SAME GENERAL OF REQUIRE INSPECTIONS. BROKEN AT NR 3 MAIN JOURNAL TO NR 5 ROD THROW.
PILOT EXPERIENCED EGT FLUXUATIONS, MADE PRECAUTIONARY LANDING. INSPECTION REVEALED CYLINDER NR 4 INJECTOR LINE WAS CRACKED AND LEAKING FUEL AT THE BASE OF THE BRAZED ON SLEEVE BETWEEN THE SLEEVE AND THE BALL END.
ON TAKE OFF THE RT ENGINE FEATHERED. INSPECTION REVEALED PROPELLER SYNCROPHASER COIL HAD BECOME SEPARATED FROM THE GOVERNOR BODY, CAUSING THE PROPELLER TO FEATHER. THE ATTACHING STUDS ON THE COIL HAD SHEARED AND BECAME BOUND UP WITH THE FLYWEIGHT ASSY. PULLING THE METERING ROD NTO THE FEATHER POSITION. RECOMMEND A CLOSER INSPECTION OF THE (PN C20722) COIL OR POSSIBLY REPLACEMENT AT OVERHAUL. (K)
PILOT REPORTED EXCESSIVE OIL LEAK ON RIGHT ENGINE CAWLING PULLED AND 3 INCH - 4 INCH CRACK NOTED AT NR 5 CYLINDR BASE. IDENTIDAL PROBLEM OCCURRED.
