DURING FLIGHT AFTER SHORTY AFTER LEVEL OFF, THE FLIGHT CREW EXPERIENCE A LOW HYDRAULIC PRESSURE MALFUNCTION AND AIR RETURNED BACK TO BASE. THE EMERGENCY LANDING GEAR HANDLE WAS MANUALLY ACTUATED DUE TO THE FAILURE AND THE AIRCRAFT LANDED WITHOUT FURTHER INCIDENT. MAINTENANCE INSPECTED THE HYDRAULIC SYSTEM AND FOUND THAT A HYDRAULIC LINE CHAFFED AGAINST A FRAME PIECE CAUSING A SMALL LEAK IN THE SYSTEM. THE LINE WAS REPLACED, THE SYSTEM WAS SERVICED, AND THE LANDING GEAR WAS CYCLED ON AIRCRAFT JACKS TO VERIFY PROPER OPERATION POST REPAIR. ALL SYSTEM FUNCTIONS WERE NORMAL, AND THE AIRCRAFT HAS BEEN RETURNED TO SERVICE.
JUST PRIOR TO LEVEL OFF AT 13000, STEADY WING OVERHEAT ANNUNCIATOR LIGHT CAME ON AND SMALL AMOUNT OF SMOKE CAME INTO THE CABIN. THE PILOT WENT THROUGH THE MEMORY ITEMS AND CHECKLIST AND THE LIGHT WENT OUT AND NO MORE SMOKE. RETURNED TO THE DEPARTURE AIRPORT. PHOENIX ATC ESCALATED TO AN EMERGENCY CATEGORY AND THE AIRCRAFT LANDED SAFELY WITHOUT INCIDENT. PHX MAINTENANCE INSPECTED CARGO BAY AND RIGHT WING FOR BLEED LEAKAGE, NO LEAK WAS FOUND. PERFORMED GROUND RUN, FOUND RT ACM COOLING TURBINE WAS SEIZED. RT ACM WAS BEING DEFERRED UNDER FLOW CONTROL VALVE SYSTEM, PER MEL 21-4. PART ON ORDER.
AMF FLT 1367 - ON DECENT INTO KLAS PILOT NOTICED A BURNING SMELL / ODOR COMING FROM THE RH ESSENTIAL CIRCUIT BREAKER PANEL. CREW CONTINUED WITH SAFEST COURSE OF ACTION AND LANDED IN KLAS WITHOUT ISSUES. INSPECTED RH ESSENTIAL BUS AND FOUND R DUCT ANTI-ICE CIRCUIT BREAKER HAD BURNT CONTACTORS. REMOVED AND REPLACED CIRCUIT BREAKER WITH IAW AMM 20-00-00. PERFORMED OPERATIONAL CHECK IAW AMM 30-60-00.
DISCREPANCY: THE PILOT DEPARTED KPHX HEADING TO KOLS. DURING THE CLIMB, THE PILOT REPORTED THAT THE RIGHT WING OVERHEAT ANNUNCIATOR ILLUMINATED. THE PILOT REQUESTED TO ATC TO RETURN TO KPHX. AFTER PERFORMING QRH, APPROXIMATELY TWO MINUTES LATER, THE CABIN STARTED FILLING UP WITH SMOKE. WHILE DECLARING AN EMERGENCY AND TALKING TO ATC, THE SMOKE CLEARED OUT. THE LANDING WAS WITHOUT INCIDENT. CORRECTIVE ACTION: MAINTENANCE INSPECTED THE AIRCRAFT AND FOUND THE AC CIRCUIT BREAKER POPPED FROM SUSPECTED PROLONGED USE OF THE MANUAL HEAT ON HIGH SETTING. MAINTENANCE PUSHED CIRCUIT BREAKER IN AND PERFORMED ENGINE RUN WITH HEAT ON. NO CABIN SMOKE DETECTED AND RIGHT-WING OVERHEAT ANNUNCIATOR LIGHT DID NOT COME ON. THE OPERATIONAL CHECK WAS NORMAL AND THE AIRCRAFT WAS RETURNED TO SERVICE.
THE PILOT REPORTED AFTER TAKEOFF, THE LEFT ENGINE BEGAN BY-PASSING, FUEL PRESSURE AND ENGINE RPM REDUCED. THE PILOT FOLLOWED PROCEDURES AND PERFORMED AND INFLIGHT SHUT DOWN OF THE LEFT ENGINE. THE PILOT DECLARING AN EMERGENCY LANDED WITHOUT INCIDENT BACK AT DEPARTURE. MAINTENANCE INSPECTED THE LEFT ENGINE AND DETERMINED THE FUEL SHUT OFF SOLENOID VALVE TO BE FAULTY. MAINTENANCE REMOVED AND REPLACED. MAINTENANCE PERFORMED GROUND RUNS AND OPERATION OF THE LEFT ENGINE WAS NORMAL. THE AIRCRAFT WAS RETURNED TO SERVICE.
THE PILOT WAS EN-ROUTE TO KPHX WHEN HE NOTICED SMOKE DEVELOPING IN THE COCKPIT AND SHORTLY AFTER THAT THE RIGHT WING OVERHEAT ANNUNCIATOR ILLUMINATED STEADY. THE PILOT FOLLOWING PROCEDURE COMPLETED THE CHECK LIST BY EXTENDING THE LANDING GEAR AND THE LIGHT EXTINGUISHED, THEN LANDED IN KPHX WITHOUT INCIDENT. MAINTENANCE INSPECTED THE AIRCRAFTâS RIGHT ENVIRONMENTAL SYSTEM AND FOUND THAT THE WATER SEPARATOR BAG WAS EXTREMELY CONTAMINATED WITH DIRT AND REPLACED IT WITH A SERVICEABLE UNIT. THEN MAINTENANCE VERIFIED FREEDOM OF MOVEMENT OF THE RIGHT AIR-CYCLE MACHINE AND PERFORMED GROUND RUNS, OPERATION OF THE RIGHT ENVIRONMENTAL SYSTEM WAS NORMAL AND THE AIRCRAFT WAS RETURNED TO SERVICE.
THE PILOT WAS ON CLIMB-OUT FROM KPHX WHEN THE ENVIRONMENTAL SYSTEM BEGAN TO BLOW VERY HOT AIR FROM THE VENTS AND THE RIGHT WING OVERHEAT ANNUNCIATOR ILLUMINATED. THE PILOT FOLLOWED PROCEDURES TO REDUCE THE WING OVER HEAT CONDITION AND THE LIGHT EXTINGUISHED. THEN DECLARED AN EMERGENCY AND LANDED WITHOUT INCIDENT BACK IN KPHX. MAINTENANCE INSPECTED THE AIRCRAFT AND FOUND THAT THE CIRCUIT BREAKER TO THE RIGHT HAND AIR-CONDITION CONTROL HAD TRIPPED. THIS CAUSED THE MIXING VALVE OF THE RIGHT ENVIRONMENTAL SYSTEM TO BE STUCK IN THE FULL HOT CONDITION THUS CREATING THE EXTREMELY HOT AIR TO ENTER THE CABIN. ONCE THE CIRCUIT BREAKER WAS RESET THE SYSTEM WAS OPERATIONALLY GROUND TESTED IN ALL PHASES OF ENVIRONMENTAL CONTROL AND OPERATION WAS NORMAL. THE AIRCRAFT WAS RETURNED TO SERVICE AND NO SUBSEQUENT ISSUES HAVE BEEN REPORTED.
WHILE ENROUTE, THE PILOT REPORTED THAT THE LEFT AND RIGHT HYDRAULIC LOW PRESSURE ANNUNCIATORS ILLUMINATED, FOLLOWED BY A COMPLETE LOSS OF HYDRAULIC PRESSURE. THE PILOT FOLLOWING PROCEDURES PERFORMED A MANUAL GEAR EXTENSION, THEN A ZERO FLAPS LANDING AT DESTINATION WITHOUT INCIDENT. MAINTENANCE INSPECTED THE AIRCRAFT AND FOUND THAT THE RIGHT HYDRAULIC PUMP INLET LINE HAD FAILED. REMOVED AND REPLACED THE RIGID LINE. DUE TO BOTH HYDRAULIC PUMPS RUNNING LONGER THAN BOOK LIMITS, THEY WERE REPLACED, THE SYSTEM FLUSHED AND THE FILTER CHANGED. DURING GROUND RUN NO LEAKS WERE NOTED.
WHILE TURNING TO ESTABLISH LOC RECEIVED A RT WING OVERHEAT INDICATION. SHORTLY AFTER THE ANNUNCIATOR ILLUM THERE WAS A SMALL AMOUNT OF SMOKE IN THE COCKPIT. FOLLOWING EMERGENCY CHECKLIST, REDUCED SPEED IN ORDER TO EXTEND THE LG. CONTACTED APPROACH AND DECLARED AN EMERGENCY. BETWEEN DECLARING AN EMERGENCY AND COMPLETING THE EMERGENCY CHECKLIST THE WING OVERHEAT LIGHT HAD EXTINGUISHED AND THE SMOKE HAD DISSIPATED BY DEACTIVATION OF SYSTEMS. DID AN AIR TURNBACK TO KBUR AS THE RISK OF AN ACTUAL FIRE HAD DIMINISHED. LANDED WITHOUT INCIDENT. INSPECTED RT WING, NO EVIDENCE OF A FIRE. DISCOVERED THE RT HAND COOLING TURBINE SEAL HAD FAILED. R&R THE RT COOLING TURBINE WITH A SERVICEABLE UNIT. R&R RT LG WW WING OH SENSOR.
WHILE ENROUTE, THE PILOT NOTICED THE RT WING OVER HEAT ANNUNCIATOR LIGHT ILLUMINATED. THIS WAS FOLLOWED BY VISIBLE SMOKE OBSERVED IN THE COCKPIT. THE PILOT FOLLOWED THE EMERGENCY PROCEDURES, TURNED OFF THE RT BLEED AIR, LOWERED THE LANDING GEAR AND DONNED HIS OXYGEN MASK. ONCE THE LANDING GEAR WAS LOWERED AND THE RT BLEED AIR WAS TURNED OFF, THE SMOKE STARTED TO DISSIPATE AND THE WING OVERHEAT LIGHT EXTINGUISHED. THE PILOT LANDED WITHOUT INCIDENT. MAINTENANCE TROUBLESHOOTING REVEALED THE RT COOLING TURBINE FAILED CAUSING THE SMOKE IN THE COCKPIT AND THE WING OVER HEAT LIGHT TO ILLUMINATE. R & R THE RT COOLING TURBINE, PN: 204755-4-6, SN: 110-991. GROUND RUNS COMPLETED THE RT COOLING TURBINE FUNCTIONAL CHECKED NORMAL; NO DEFECTS WERE NOTED AT THIS TIME.
AFTER TAKEOFF, PILOT NOTICED THE RT WING OVERHEAT LIGHT ILLUMINATED & OBSERVED SMOKE IN THE COCKPIT. PILOT DECLARED AN EMER & RETURNED, LANDED WITHOUT INCIDENT. PERFORMED HIGH POWER ENG RUNS WITH THE RT BLEED AIR SYS ON & OFF, BUT COULD NOT DUPLICATE THE DISCREPANCY. INSPECTED THE BLEED AIR DUCTS, BLEED AIR SYS & WING OVERHEAT SYS WIRING, NO DISCREPANCIES NOTED. NO DISCREPANCIES HAVE BEEN REPORTED ON SUBSEQUENT FLIGHTS.
AFTER ROTATION PILOT SELECTED LG HANDLE IN THE RETRACT POSITION. LG REMAINED EXTENDED & LOCKED WITH THREE GREEN INDICATOR LIGHTS ILLUMINATED. PILOT SWITCHED THE GEAR CONTROL & INDICATOR TO RT BUS & VERIFIED THE EMER GEAR RELEASE SYS WAS PROPERLY STOWED, GEAR DID NOT RETRACT. PILOT DECIDED TO RETURN & LANDED WITHOUT FURTHER INCIDENT. TROUBLESHOOTING REVEALED AIR TRAPPED IN LG STRUT PREVENTED PISTON IN STRUT FROM FULLY EXTENDING. BLEED STRUT & ENSURED PISTON WAS FULLY BOTTOMED OUT. SERVICED STRUT IAW MM & CYCLED LG SEVERAL TIMES TO ENSURE PROPER OPERATION. ALL CHECKS WERE NORMAL.
DURING CRUISE FLIGHT, BOTH HYDRAULICS FAILED ANNUNCIATORS ILLUMINATED. HYD PRESSURE READ 200PSI. PERFORMED AN EMERGENCY GEAR EXTENSION AND A FLAPS UP LANDING WITHOUT INCIDENT. MX INSPECTED THE ACFT AND FOUND THE LT FLAP ACTUATOR "UP" FLEXIBLE HYDRAULIC LINE LEAKING FLUID. HYD LINE WAS CAPPED OFF AND THE ACFT WAS ISSUED A SPECIAL FLIGHT PERMIT FOR THE LINE REPLACEMENT.
FLT 3226 - ON APPROACH TO YQT, THE NOSE GEAR DOOR LIGHT FAILED TO ILLUMINATE FTER GEAR EXTENSION. THE CREW CYCLED THE GEAR UNTIL ALL INDICATIONS WERE NORMAL. MAINTENANCE CLEANED AND LUBED THE SWITCHES AND PERFORMED GEAR SWINGS. ALL CHECKS NORMAL. (M)
FLT 3342 - ON APPROACH TO DTW, THE NOSE GEAR DOWN AND LOCKED LIGHT FAILED TO ILLUMINATE AFTER GEAR EXTENSION. THE CREW CYCLED THE GEAR UNTIL THE LIGHT ILLUMINATED. MAINTENANCE CLEANED AND LUBRICATED THE DOWNLOCK SWITCHES. ALL CHECKS SATISFACTORY. (M)

FLT 3213 - ON APPROACH TO BJI, THE CREW DID NOT RECEIVE A GREEN DOWN AND LOCKED INDICATION FROM THE RIGHT GEAR. THE CREW PUSHED ON THE INDICATOR HOUSING AND THE LIGHT ILLUMINATED. THE CREW PERFORMED A FLY BY AND THE GEAR WAS DOWN AND LOCKED. THE AIRCRAFT LANDED WITHOUT INCIDENT IN BJI. MAINTENANCE REPLACED THE LANDING GEAR POSITION INDICATOR. SYSTEM OPERATED SATISFACTORILY DURING A TEST FLIGHT. (W)
FLT 3306 - IN CRUISE FLIGHT TO MSP, THE NOSE GEAR BEGAN TO CYCLE IN THE WHEEL WELL. THE CREW LOWERED THE GEAR AND CONTINUED TO MSP. MAINTENANCE REPLACED THE LEFT AND RIGHT NOSE LANDING GEAR ACTUATORS. SYSTEM OPERATED SATISFACTORILY DURING A TEST FLIGHT. (W)
FLT 3235 - ON CLIMB-OUT FROM YQT, THE NOSE GEAR CONTINUED TO CYCLE AFTER GEAR RETRACTION. THE CREW RECYCLED THE GEAR AND THE GEAR RETRACTED NORMALLY. MAINTENANCE ADJUSTED THE RIGGING OF THE NOSE LANDING GEAR UP LIMIT SWITCH AND RERIGGED THE NOSE LANDING GEAR DOORS AND THE RIGHT NOSE LANDING GEAR STRIKER ROD. SYSTEM OPERATED SATISFACTORILY DURING A TEST FLIGHT. (W)
