DURING CRUISE FLIGHT, AT 45,000FT AGL, THE FLIGHT CREW ACTIVATED THE ENGINE COWL ANTI-ICE AND SAW SLIGHT SMOKE IN COCKPIT AND CABIN. IMMEDIATELY TURNED OFF THE SYSTEM AND SMOKE CLEARED FROM CABIN. CONTINUED FLIGHT WITH NO FURTHER ISSUES. PERFORMED AN OPS CHECK IAW AMM 36-00-00, SYSTEM OPS CHECKED GOOD, COULD NOT DUPLICATE THE SMOKE IN THE CABIN.
THE CREW REPORTED THAT THE GALLEY DOOR WAS VIBRATING IN FLIGHT OVER 150 KNOTS. FLIGHT 2470 THEN DIVERTED TO BWI. THE AIRCRAFT WAS THEN FERRIED TO CAK. MAINTENANCE INSPECTED AND FOUND A SPLICE IN THE STABILZER ROD OF THE GALLEY DOOR. MAINTENANCE REMOVED AND REPAIRED THE SPLICE AND WIRE IN ACCORDANCE WITH SMP. OPERATIONAL CHECKS WERE PERFORMED AND THE AIRCRAFT WAS RELEASED FOR SERVICE.
DURING FLIGHT, THE CREW RECEIVED A GROUND AIR DOOR WARNING DURING CLIMBOUT AND RETURNED TO PIT. MAINTENANCE INSPECTED, CLEANED AND ACTUATED THE DOOR SWITCH IN ACCORDANCE WITH DORNIER MAINTENANCE MANUAL 52-00-00. OPERATIONAL CHECKS WERE PERFORMED AND THE AIRCRAFT WAS THEN RELEASED FOR SERVICE.
THE CREW REPORTED A LAV SMOKE FAIL INDICATION. MAINTENANCE THEN DEFERRED THE LAV SMOKE DETECTOR IN ACCORDANCE WITH 26-3 CODE C.
THE CREW REPORTED THAT THE EMERGENCY LIGHTING WAS NOT ILLUMINATING. MAINTENANCE INSPECTED AND REPLACED THE 7LK EMERGENCY BATTERY IN ACCORDANCE WITH DORNIER MAINTENANCE MANUAL 33-00-00. OPERATIONAL CHECKS WERE PERFORMED AND THE AIRCRAFT WAS RELEASED FOR SERVICE.
DURING TAKEOFF, THE AIRCRAFT FAILED TO PRODUCE TAKEOFF TORQUE AND THE CREW ABORTED TAKEOFF. MAINTENANCE INSPECTED AND REPLACED THE HYDROMECHANICAL UNIT IN ACCORDANCE WITH DORNIER MAINTENANCE MANUAL 73-00-00. OPERATIONAL CHECKS WERE PERFORMED AND THE AIRCRAFT WAS RELEASED FOR SERVICE.
THE CREW REPORTED THAT THE EMERGENCY LIGHTING AT ROWS 9 AND 10 WERE NOT ILLUMINATING. MAINTENANCE INSPECTED AND REPLACED THE EMERGENCY BATTERY IN ACCORDANCE WITH DORNIER MAINTENANCE MANUAL 33-00-00. OPERATIONAL CHECKS WERE PERFORMED AND THE AIRCRAFT WAS RELEASED FOR SERVICE.
THE CREW REPORTED THAT THE EMERGENCY LIGHTING AT ROWS 4 TO 7 WERE INTERMITTENT. MAINTENANCE INSPECTED AND REPLACED THE 5LK EMERGENCY BATTERY IN ACCORDANCE WITH DORNIER MAINTENANCE MANUAL 33-00-00. OPERATIONAL CHECKS WERE PERFORMED AND THE AIRCRAFT WAS THEN RELEASED FOR SERVICE.

THE CREW REPORTED THAT THE EMERGENCY FLOOR LIGHTING WAS NOT WORKING AT SEAT 9A. MAINTENANCE INSPECTED AND REPLACED THE EMERGENCY FLOOR PATH LIGHT STRIP IN ACCORDANCE WITH DORNIER MAINTENANCE MANUAL 33-00-00. OPERATIONAL CHECKS WERE PERFORMED AND THE AIRCRAFT WAS RELEASED FOR SERVICE.
DURING TAKE OFF, THE CREW RECEIVED A RIGHT HAND ALTERNATOR FAIL CAS MESSAGE AND ABORTED TAKE OFF. THE AIRCRAFT WAS THEN FERRIED TO PIT WHERE MAINTENANCE REPLACED THE RIGHT HAND ALTERNATOR AND THE GARLOCK SEAL IN ACCORDANCE WITH DORNIER MAINTENANCE MANUAL 24-00-00. OPERATIONAL CHECKS WERE PERFORMED AND THE AIRCRAFT WAS RELEASED FOR SERVICE.
DURING TAKE OFF, THE CREW RECEIVED A RIGHT HAND ALTERNATOR FAIL CAS MESSAGE AND ABORTED TAKE OFF. AFTER THE POWER LEVERS WERE PULLED BACK THE ALTERNATOR RESET PRIOR TO EXITING RUNWAY. MAINTENANCE CONTROL HAD THE CREW PERFORM AN AIRCRAFT RUN UP AND THERE WERE NO DISCREPENCIES. FLIGHT 4115 THEN RETURNED TO THE GATE WHERE OUTSTATION MAINTENANCE INSPECTED AND FOUND NO DEFECTS. THE AIRCRAFT WAS THEN CLEARED FOR ANOTHER TAKE OFF ATTEMPT. FLIGHT 4115 THEN DEPARTED WITH NO FURTHER INCIDENTS.
THE CREW RECEIVED A FIRST OFFICER WINDSHIELD HEAT FAIL CAS MESSAGE. THE CREW THEN FOLLOWED THE QUICK REFERENCE HANDBOOK AND CALLED MAINTENANCE CONTROL. FLIGHT 4056 WAS THEN INSTRUCTED TO RETURN TO CAK. MAINTENANCE INSPECTED AND REPLACED THE FIRST OFFICER'S WINDSHIELD HEAT CONTROLLER IN ACCORDANCE WITH DORNIER MAINTENANCE MANUAL 30-00-00. OPERATIONAL CHECKS WERE PERFORMED AND THE AIRCRAFT WAS RELEASED FOR SERVICE.
DURING TAKE OFF, THE CREW RECEIVED A TAKE OFF CONFIGURATION WARNING ON THE RED WARNING PANEL AND ABORTED TAKE OFF. MAINTENANCE INSPECTED AND THE HIGH PRESSURE PUMP WAS ALLOWING FLUCTUATIONS OF THE PROP BLADE ANGLES. MAINTENANCE THEN REPLACED THE HIGH PRESSURE PUMP IN ACCORDANCE WITH DORNIER MAINTENANCE MANUAL 61-00-00. OPERATIONAL CHECKS WERE PERFORMED AND THE AIRCRAFT WAS RELEASED FOR SERVICE.
DURING CLIMB, THE CREW NOTICED A VIBRATION SUSPECTED FROM THE LEFT HAND PROPELLER AND RETURNED TO PIT. MAINTENANCE INSPECTED AND THEN BALANCED THE LEFT HAND PROPELLER IN ACCORDANCE WITH DORNIER MAINTENANCE MANUAL 61-00-00. OPERATIONAL CHECKS WERE PERFORMED AND THE AIRCRAFT WAS RELEASED FOR SERVICE.
THE CREW REPORTED THAT THE CABIN TEMPERATURE WOULD NOT GO BELOW 35 DEGREES CELSIUS AND RETURNED TO DCA. THE AIRCRAFT WAS FERRIED TO DAY WHERE MAINTENANCE INSPECTED AND REPLACED THE ENVIRONMENTAL CONTROL UNIT IN ACCORDANCE WITH DORNIER MAINTENANCE MANUAL 21-00-00. OPERATIONAL CHECKS WERE PERFORMED AND THE AIRCRAFT WAS RELEASED FOR SERVICE.
DURING TAKE OFF, THE CREW RECEIVED A RIGHT HAND DC GENERATOR FAILURE CAS MESSAGE. AFTER CLEARING THE RUNWAY, THE CREW RESET THE RIGHT HAND GENERATOR AND THE MESSAGE EXTINGUISHED. FLIGHT 4015 WAS THEN CLEARED FOR ANOTHER TAKE OFF ATTEMPT. FLIGHT 4015 THEN DEPARTED WITH NO FURTHER INCIDENTS.
EN ROUTE, THE CREW RECEIVED A DOORS RED WARNING PANEL LIGHT. ALL DOORS WERE SECURE AND THE AIRCRAFT DID NOT LOSE PRESSURIZATION. THE CREW FOLLOWED THE QUICK REFERENCE HANDBOOK AND DIVERTED TO PIT. MAINTENANCE INSPECTED AND ADJUSTED THE 9A EMERGENCY DOOR PROXIMITY SENSOR BRACKET IN ACCORDANCE WITH DORNIER MAINTENANCE MANUAL 52-00-00. OPERATIONAL CHECKS WERE PERFORMED AND THE AIRCRAFT WAS RELEASED FOR SERVICE.
APPROXIMATELY SIX MILES FROM THE BAL VOR, THE CREW EXPERIENCED A FLASH AND LOUD BOOM. THE CREW ASSUMED THAT THE AIRCRAFT HAD BEEN STRUCK BY LIGHTNING. THE CREW DECLARED AN EMERGENCY AND LANDED WITH NO FURTHER INCIDENTS. THE AIRCRAFT WAS FERRIED TO DAY WHERE MAINTENANCE INSPECTED AND REPLACED THE LEFT HAND PROPELLER IN ACCORDANCE WITH DORNIER MAINTENANCE MANUAL 61-00-00. OPERATIONAL CHECKS WERE PERFORMED AND THE AIRCRAFT WAS RELEASED FOR SERVICE.
THE CREW REPORTED THAT THE EMERGENCY LIGHTING WAS NOT ILLUMINATING AT ROWS 6 THROUGH 9 ON ENGINE START UP. MAINTENANCE INSPECTED AND REPLACED THE 6LK EMERGENCY BATTERY IN ACCORDANCE WITH DORNIER MAINTENANCE MANUAL 33-00-00. OPERATIONAL CHECKS WERE PERFORMED AND THE AIRCRAFT WAS RELEASED FOR SERVICE.
AFTER TAKE OFF, THE CREW WAS GIVEN A HEADING AND DISCOVERED THAT THE FLIGHT DIRECTOR WAS GIVING IMPROPER INDICATIONS. THE CREW DECIDED TO RETURN TO YOW AND CALL MAINTENANCE. MAINTENANCE HAD THE CREW RECYCLE AIRCRAFT POWER. OPERATIONAL CHECKS WERE PERFORMED AND THE AIRCRAFT WAS RELEASED FOR SERVICE. FLIGHT 4190 DEPARTED WITH NO FURTHER INCIDENTS.
DURING CLIMB, THE CABIN PRESSURE WAS CLIMBING WITH THE AIRCRAFT. AFTER CLIMBING TO 8000 FT MSL THE CABIN PRESSURE WAS INDICATING 7500 FT MSL. FLIGHT 4030 RETURNED TO PIT WHERE MAINTENANCE INSPECTED AND FOUND THE AVIONICS MAINTENANCE DOOR IMPROPERLY INSTALLED. MAINTENANCE INSTALLED THE AVIONICS MAINTENANCE DOOR IN ACCORDANCE WITH DORNIER MAINTENANCE MANUAL 26-00-00. OPERATIONAL CHECKS WERE PERFORMED AND THE AIRCRAFT WAS RELEASED FOR SERVICE.
THE CREW REPORTED THAT SOME OF THE EMERGENCY LIGHTING WAS NOT ILLUMINATING. MAINTENANCE INSPECTED AND REPLACED THE EMERGENCY BATTERY AT THE 4LK POSITION IN ACCORDANCE WITH DORNIER MAINTENANCE MANUAL 33-00-00. OPERATIONAL CHECKS WERE PERFORMED AND THE AIRCRAFT WAS RELEASED FOR SERVICE.
AFTER STARTING THE TAKE OFF ROLL, THE CREW RECEIVED A RIGHT HAND ELECTRONIC CONTROL UNIT FAIL CAS MESSAGE AND ABORTED TAKE OFF. FLIGHT 4082 TAXIIED BACK TO THE GATE WHERE MAINTENANCE INSPECTED AND REPLACED THE INTERCOMP BLEED VALVE IN ACCORDANCE WITH DORNIER MAINTENANCE MANUAL 73-00-00. OPERATIONAL CHECKS WERE PERFORMED AND THE AIRCRAFT WAS RELEASED FOR SERVICE.
WHILE TAXIING, THE CREW HEARD WHAT SOUNDED LIKE A TIRE BLOWING. THE CAPTAIN CONTACTED GROUND AND RECEIVED PERMISSION TO SHUT DOWN THE ENGINES AND DO A VISUAL CHECK. THE CAPTAIN FOUND THE LEFT MAIN OUTBOARD TIRE TO BE BLOWN. THE PASSENGERS WERE BUSSED BACK TO THE TERMINAL. THE AIRCRAFT WAS TAXIED TO THE HANGAR WHERE OUTSTATION MAINTENANCE INSPECTED AND REPLACED THE TIRE IN ACCORDANCE WITH DORNIER MAINTENANCE MANUAL 32-00-00.
THE FLIGHT CREW REPORTED THAT THE EMERGENCY FLOOR PATHWAY LIGHTING STRIP WOULD NOT ILLUMINATE ADJACENT TO SEAT 1D/F. MAINTENANCE INSPECTED AND RESECURED THE LIGHT STRIP IN ACCORDANCE WITH DORNIER MAINTENANCE MANUAL 33-51-12. OPERATIONAL CHECKS WERE COMPLETED AND THE AIRCRAFT RETURNED TO SERVICE.
THE CREW REPORTED THAT THE EMERGENCY LIGHT AND BOARDING LIGHT BY ROW 9A WAS NOT WORKING. MAINTENANCE INSPECT AND REPAIRED A BROKEN WIRE IN ACCORDANCE WITH STANDARD MAINTENANCE PRACTICES. OPERATIONAL CHECKS WERE PERFORMED AND THE AIRCRAFT WAS RELEASED FOR SERVICE.
THE CREW REPORTED THAT THE EMERGENCY LIGHTING AT ROW 9A WAS NOT WORKING. MAINTENANCE INSPECTED AND REPLACED THE EMERGENCY BATTERY IN ACCORDANCE WITH DORNIER MAINTENANCE MANUAL 33-51-01. OPERATIONAL CHECKS WERE PERFORMED AND THE AIRCRAFT WAS RELEASED FOR SERVICE.
DURING TAKEOFF, THE CREW RECEIVED A GEAR WARNING HORN CAS MESSAGE. THE CREW ABORTED TAKEOFF AND RETURNED TO THE GATE. THE AIRCRAFT WAS THEN FERRIED TO CAK. MAINTENANCE INSPECTED AND REPLACED THE PSEU IN ACCORDANCE WITH DORNIER MAINTENANCE MANUAL 32-00-00. OPERATIONAL CHECKS WERE PERFORMED AND THE AIRCRAFT WAS RELEASED FOR SERVICE.
THE CREW ADVANCED POWER FOR TAKEOFF, THE LEFT HAND LEVER WAS STICKING. THE CREW ABORTED TAKEOFF AND RETURNED TO THE GATE. MAINTENANCE INSPECT AND LUBRICATED THE LEVER IN ACCORDANCE WITH DORNIER MAINTENANCE MANUAL 73-00-00. OPERATIONAL CHECKS WERE PERFORMED AND THE AIRCRAFT WAS RELEASED FOR SERVICE. FLIGHT 4163 DEPARTED WITHOUT FURTHER INCIDENT.
FLIGHT 4060 WAS EN ROUTE AT FL240 AND THE RIGHT HAND ENGINE OIL PRESSURE BEGAN TO FLUCTUATE. THE RIGHT HAND ENGINE COMPLETELY LOST POWER. THE CREW REFERENCED THE QUICK REFERENCE HANDBOOK PROCEDURES AND ATTEMPTED TO CONTACT MAINTENANCE CONTROL BUT WERE UNSUCCESSFUL. THE FIRST OFFICER WAS FLYING AND STARTED A COORDINATED DIVERSION TO ROANOKE WITH CENTER. THE FLIGHT ATTENDANT PREPARED THE CABIN FOR A EMERGENCY LANDING AND POSSIBLE EVACUATION. FLIGHT 4060 LANDED AT ROANOKE WITHOUT FURTHER INCIDENT. MAINTENANCE INSPECTED AND REPLACED THE RIGHT HAND ENGINE IN ACCORDANCE WITH DORNIER MAINTENANCE MANUAL 72-00-00. OPERATIONAL CHECKS WERE PERFORMED AND THE AIRCRAFT WAS RELEASED FOR SERVICE.
ON THE TAKEOFF ROLL, THE CREW RECEIVED A NOSE WHEEL STEERING POWER UP FAIL MESSAGE. ONCE THE TAKEOFF WAS ABORTED, THE CREW RECEIVED THREE GEAR UNSAFE INDICATIONS ALONG WITH THREE GEAR DOWN AND LOCKED INDICATIONS. MAINTENANCE INSPECTED THE AIRCRAFT AND REPLACED THE LIGHT DIMMER CONTROL UNIT IN ACCORDANCE WITH DORNIER MAINTENANCE MANUAL 33-13-11. OPERATIONAL CHECKS WERE COMPLETED AND THE AIRCRAFT RETURNED TO SERVICE.
FLIGHT 4087- WAS ON APPROACH TO PIT WHEN A NOSE WHEEL STEERING POWER-UP CAS MESSAGE WAS DISPLAYED. THE FLIGHT CREW REVIEWED THE QRH AND PERFORMED A SECOND APPROACH AND LANDED WITHOUT DIFFICULTIES. MAINTENANCE INSPECTED AND FOUND THE RT CONDITION LEVER MICROSWITCH TO BE STUCK. MAINTENANCE EXERCISED THE MICROSWITCH AND PERFORMED OPERATIONAL CHECKS IN ACCORDANCE WITH DORNIER MAINTENANCE MANUALS 76-11-05 AND 32-51-00 AND ALL WAS FOUND TO OPERATE NORMALLY. THE AIRCRAFT WAS RETURNED TO SERVICE.
WHILE IN CRUISE FLIGHT, THE LEFT ENGINE OIL PRESSURE INDICATION BEGAN TO FLUCTUATE AND THEN TURNED AMBER. THE FLIGHT CREW FOLLOWED THE QRH AND FEATHERED THE LEFT PROPELLER. THE LEFT ENGINE OIL PRESSURE CONTINUED TO FLUCTUATE AND THEN TURNED INTO A RED INDICATION WHICH RESULTED IN THE FLIGHT CREW SHUTTING DOWN THE LEFT ENGINE. THE AIRCRAFT DIVERTED TO CRW AND LANDED WITHOUT INCIDENT. MAINTENANCE INSPECTED THE AIRCRAFT AND FOUND THE LEFT ENGINE ALTERNATOR GARLOCK SEAL TO BE LEAKING. THE SEAL WAS REPLACED IN ACCORDANCE WITH DORNIER MAINTENANCE MANUAL 72-10-04. OPERATIONAL AND LEAK CHECKS WERE COMPLETED AND THE AIRCRAFT WAS RETURNED TO SERVICE.
THE LEFT HAND BRAKES WERE REPORTED FROM THE CREW AS SPONGEY WHEN SELECTED, DURING THE TAXI FOR TAKEOFF. THE AIRCRAFT RETURNED TO THE GATE. MAINTENANCE INSPECTED AND BLED THE BRAKE SYSTEM, IN ACCORDANCE WITH DORNIER MAINTENANCE MANUAL 32-42-00. OPERATIONAL CHECKS WERE COMPLETED, AND THE AIRCRAFT WAS RETURNED TO SERVICE.
AT 12,000 FT, RECEIVED EMERGENCY PARKING BRAKE ACCUMULATOR LOW WARNING LIGHT. COMPLIED WITH QUICK REFERENCE HANDBOOK AND LIGHT EXTINGUISHED, 3 MINUTES LATER LIGHT ILLUMINATED AGAIN. RETURNED TO FIELD AND LANDED WITHOUT INCIDENT. FOUND HYDRAULIC CHECK VALVE DEFECTIVE. REPLACED VALVE IN ACCORDANCE WITH DORNIER MAINTENANCE MANUAL 29-21-09, OPERATIONAL CHECK GOOD.
AFTER TAKEOFF, THE LEFT ENGINE INLET DE-ICE FAILED. RESET SWITCH AND THE SYSTEM FAILED AGAIN. RETURNED TO THE FIELD WITHOUT INCIDENT. MAINTENANCE REMOVED AND REPLACED THE ENGINE INLET, OPERATIONAL CHECKED GOOD.
CREW REPORTED HEARING A LOUD BANG, NO APPARENT CHANGE IN ANY OF THE AIRCRAFT SYSTEMS. AIRCRAFT CONTINUED ON WITH THE FLIGHT AND LANDED IN PIT WITHOUT FURTHER INCIDENT. PIT MX CONDUCTED A LIGHTNING STRIKE INSPECTION AND NOTICED THAT THE RUDDER CAP SHOWED SIGNS OF LIGHTNING STRIKE DAMAGE, BURN MARKS. REPAIRS WERE ACCOMPLISHED IAW MANUFACTURERS RECOMMENDATIONS.
CLT - FLT 4081 - EN ROUTE FROM CLT TO CAK, CREW REPORTED THAT OUTER PANE OF CAPTAIN'S WINDSHIELD FAILED. CREW THEN FOLLOWED ESTABLISHED ABNORMAL PROCEDURES BY DESCENDING FL 100 REDUCING CABIN PRESSURE DIFFERENTAL. FLIGHT CONTINUED TO ITS ORIGINAL DESTINATION, AIRCRAFT LANDED WITHOUT FURTHER INCIDENT. CAK MX REPLACED LT WINDSHIELD IAW MM 56-10-00. AIRCRAFT RETURNED TO SERVICE.
FLT 4079 - AFTER TAKEOFF, CREW REPORTED A 'RED' INTRANSIT LIGHT LT MAIN GEAR, AFTER GEAR WAS SELECTED 'UP'. AIRCRAFT RETURNED TO THE FIELD, LANDED WITHOUT FURTHER INCIDENT.
