THE PILOT REPORTED AFTER TAKEOFF THAT THE COCKPIT FILLED WITH SMOKE. AFTER TURNING THE ACM SELECTOR SWITCH TO THE OFF AND COLD POSITION, THE HEAT POSITION STAYED ON. THE PILOT RETURNED TO DEPARTURE AND LANDED WITHOUT INCIDENT. MAINTENANCE INSPECTED AND FOUND A DEFECTIVE LEFT ACM MIXING VALVE. THE LEFT AMC MIXING VALVE WAS REPLACED WITH A SERVICEABLE ONE. THE OPERATIONAL CHECK IN AUTO AND MANUAL MODE WERE NORMAL. THE OPERATIONAL FLIGHT CHECK WAS SUCCESSFUL. THE AIRCRAFT WAS RELEASED AND RETURNED TO SERVICE.
PILOT REPORTED WHILE ENROUTE FROM KBUF TO CYUL, THE CENTER HAD CONTACTED HIM REGARDING A PIECE OF RUBBER FOUND ON THE DEPARTING RUNWAY IN KBUF. REQUESTED A LOW FLY BY TO SEE IF ANY OBVIOUS SIGNS OF DAMAGE COULD BE SEEN, THE GEAR AND WHEELS APPEARED DOWN AND LOCKEDâ REQUESTED A SECOND LOW PASS FOR FURTHER REASSURANCES. ATC THEN REPLIED ALL LOOKED GOOD. THE PILOT THEN DECLARED AN EMERGENCY AND LANDED WITHOUT INCIDENT IN CYUL. UPON POST FLIGHT, CONFIRMED THE NUMBER 3 TIRE HAD LOST ITS TREAD. MX INSPECTED THE AIRCRAFT AND R&R THE NR 3 WHEEL TIRE ASSEMBLY WITH AN OVERHAULED UNIT. MX ALSO R&R THE NUMBER 4 WHEEL TIRE ASSEMBLY WITH AN OVERHAULED UNIT. THE AIRCRAFT WAS RETURNED TO SERVICE AND NO SUBSEQUENT ISSUES HAVE BEEN REPORTED.
FOLLOWING TAKEOFF, BOTH HYDR PRESSURE LIGHTS CAME ON RIGHT AFTER GEAR RETRACTION. PILOT DECLARED AN EMERGENCY, CONDUCTED AN EMERGENCY GEAR EXTENSION PROCEDURE, MADE A LANDING WITHOUT FURTHER INCIDENT. UPON EXAMINATION, IT WAS DISCOVERED THAT HYDR ACCUMULATOR END CAP ON HYDR SIDE HAD EXPERIENCED A CATASTROPHIC METALLURGICAL FAILURE, RESULTING IN SIGNIFICANT LOSS OF HYDR FLUID AND THEREFORE PRESSURE. THIS ACCUMULATOR IS AN "ON CONDITION" PART, WITH NO O/H REQUIREMENTS. THIS COMPANY HAS EXPERIENCED (2) SIMILAR FAILURES IN 2005 AND 2003 ON THIS MAKE AND MODEL OF AC, AND AN ADDITIONAL FAILURE OF THIS TYPE ON ANOTHER AC WHICH HAS AN ACCUMULATOR PN 22300-3. IT WOULD SEEM PRUDENT TO INSTITUTE SOME SORT OF NDI SCHEDULE FOR THESE PARTS AT A SPECIFIC INTERVAL, RATHER THAN LEAVE IT AS JUST ON CONDITION. (K)

FLT 3099 - ON FINAL APPROACH INTO MSP, NO NOSE GEAR GREEN DOWN LIGHT SLOW TO ILLUMINATE. AIRCRAFT LANDED WITHOUT INCIDENT. MAINTENANCE CLEANED AND LUBED SWITCHES, COMPLETED SEVERAL GEAR SWINGS. INDICATION SYSTEMS OPS CHECK GOOD. (M)

DURING SCHEDULED INSPECTION IN MSP, MAINTENANCE FOUND CHAFE DAMAGE AT FS 241 AND ON BOTH SIDES OF BL 0. CHAFE DAMAGE IS APPROX .025 DEEP AND .125 INCH IN DIAMETER. FABRICATED AND INSTALLED AN EXTERNAL REPAIR DOUBLER (.050 INCH AND 4 INCH BY 5 INCH) IAW FAC SRM 51-90-04 PG 7-8 REV JUN 1/93. (M)
DURING A SCHEDULED INSPECTION IN MSP, AFTER REMOVAL OF AFT LOWER CARGO DOOR FRAME FILLET, P/N 27-24082-9003 AT FS 491, MAINTENANCE FOUND THE FUSELAGE SKIN RIVET HOLES TO BE OVERSIZED (CARGO DOOR SURROUND SKIN). REMOVED AND REPLACED THE AFT LOWER SKIN SECTION (7 INCH BY 7 INCH) AND INSTALLED A REPAIR DOUBLER OVER SKIN SPLICE (2.5 INCH BY 3.5 INCH). ALL WORK COMPLETED IAW FAC SRM 51-90-04 PGS 7-8 REV JUN 1/93 AND AEROTECH DESIGNS EA01-96090 REV A, DATED SEPT 18/96. (M)
DURING A SCHEDULED INSPECTION IN MSP, MAINTENANCE FOUND THE AFT UPPER CARGO DOOR SKIN JUST FORWARD OF FS 491 CHAFED APPROX .50 INCH. CHAFED DAMAGE CAUSED FROM CARGO DOOR HINGE PIN END, HINGE PIN LENGTH DETERMINED TO BE TOO LONG. CARGO SKIN AREA REPAIR IAW FAC ARP 52-30-9130 DATED JUNE 20, 1991. (M)
DURING BLOCK INSPECTION IN MSP, MAINTENANCE COMPLETED REPAIR TO FUSELAGE FORWARD CARGO DOOR FRAME P/N 27-22085 AT FS 438 DUE TO .250 INCH CRACK NOTED IN AREA OF THE DOOR GAS STRUT SUPPORT LOWER MOUNT HOLE. REMOVED CRACKED AREA AND INSTALLED REPAIR DOUBLERS IAW FAC ARP 53-20-9503. (M)
DTW - FLT 3386 - ON APPROACH TO DTW THE NOSE GEAR WAS SLOW TO SHOW DOWN AND LOCKED AFTER GEAR EXTENSION. MAINTENANCE WAS UNABLE TO DUPLICATE THE DISCREPANCY. AS A PRECAUTION, THE NOSE GEAR AND POSITION SWITCHES WERE CLEANED AND LUBED. (X)
DURING A SCHEDULED INSPECTION IN MSP, MAINTENANCE PERSONNEL NOTED A CRACK IN THE PASSENGER DOOR AFT FRAME. THEY REPAIRED THE CRACK BY REMOVING 7 INCHES OF THE FRAME AND INSTALLING A FILLER AND DOUBLER IAW FAC SRM 52-90-10. (M)
FLT NR 3345 - EN ROUTE FROM DTW TO CVG, THE LEFT INTAKE HEAT WOULD NOT FUNCTION WHEN SELECTED ON. THE AIRCRAFT RETURNED TO DTW WITHOUT INCIDENT. MAINTENANCE PERSONNEL REPLACED THE LEFT INTAKE HEAT VALVE AND REPAIRED BROKEN WIRES AT THE INTAKE HEAT VALVE CANNON PLUG. ALL SYSTEMS OPERATED SATISFACTORILY. (M)
DURING A SCHEDULED INSPECTION IN MSP, MAINTENANCE NOTED THAT THE RIGHT LOWER WING HAD A CRACK AROUND THE RIVET HOLE AT WS 113. REPAIRS WERE COMPLETED BY INSTALLING A SURFACE DOUBLER IAW FAC LAR 95-MALA-280 AND INSTALLING STRINGER REPAIR DOUBLERS IAW FAC SRM 51-90-05 PAGES 1-3. (M)
DURING A SCHEDULED INSPECTION IN MSP, THE AFT CARGO FUSELAGE FRAME AT FS 491 WAS FOUND TO BE CRACKED AT THE GAS STRUT MOUNT POST HOLE. REPAIRS WERE COMPLETED BY FABRICATING AND INSTALLING A REPAIR ANGLE IAW FAIRCHILD ARP 53-20-9503. (M)
MSP - DURING PRE-FLIGHT INSPECTION, THE CREW NOTED A BIRD STRIKE DENT ON THE LEFT WING EXTENSION. MAINTENANCE PERSONNEL REPAIRED THE WING DENT BY REMOVING A 11.75 INCH SECTION OF LEADING EDGE SKIN AND REPLACING IT WITH A FLUSH PATCH AND REPAIR SPLICE IAW FAIRCHILD SRM 57-90-20. (X)
DURING A SCHEDULED INSPECTION IN MSP, THE PASSENGER DOOR LOWER FRAME WAS FOUND TO BE DENTED. REPAIRS WERE COMPLETED BY REPLACING THE CORNER FRAME, P/N 27-24044-009, STRAIGHT FRAME, P/N 27-24043-003, AND STRAIGHT FRAME, P/N 27-24043-035 IAW FAC SRM 51-00-00 PAGE 4 PART FOR PART REPLACEMENT, 51-40-51, 51-40-02 STANDARD PRACTICES. FFM - FLT 3074 - AFTER TAKEOFF, NOSE LANDING GEAR WOULD CYCLE PARTIALLY OUT+OF +HEEL WELL. NO GEAR INDICATIONS WOULD ILLUMINATE. THE FLIGHT CONTINUED TO MSP WITHOUT INCIDENT. MAINTENANCE REPLACED THE RT MAIN GEAR UPLOCK. THE SYSTEM OPERATED SATISFACTORILY. (W)
MSP - FLT 3218 - ON CLIMB-OUT FROM MSP, THE GEAR WARNING HORN SOUNDED. THE WARNING HORN CONTINUED TO SOUND UNTIL THE GEAR WAS SELECTED DOWN. THE AIRCRAFT RETURNED TO MSP. MAINTENANCE REPLACED A MICROSWITCH AT THE RIGHT POWER LEVER. SYSTEM OPERATED SATISFACTORILY. (X)
MSP - FLT 3352 - ON CLIMB-OUT FROM MSP, THE GEAR WARNING HORN BEGAN TO SOUND. THE AIRCRAFT RETURNED TO MSP. MAINTENANCE WAS UNABLE TO DUPLICATE THE DISCREPANCY. AS A PRECAUTION, MAINTENANCE REPLACED THE SAS COMPUTER. SYSTEM OPERATED SATISFACTORILY DURING A TEST FLIGHT. (X)
DURING A SCHEDULED INSPECTION IN MSP, MAINTENANCE FOUND THE RIGHT FLAP INBOARD END RIB TO HAVE CHAFING DAMAGE FROM CONTACT WITH THE SWING LINK BRACKET. REPAIRS WERE COMPLETED BY REPLACING THE RIGHT FLAP INBOARD END RIB. (Z)
FLT NR 3496 - ON APPROACH TO MSP, THE NOSE GEAR INTRANSIT LIGHT AND DOWN AND LOCKED LIGHT WAS SLOW TO ILLUMINATE. THE AIRCRAFT CONTINUED TO MSP. MAINTENANCE REPLACED THE NOSE GEARS LEFT UPLOCK SWITCH. SYSTEM OPERATED SATISFACTORILY. (Z)
