DURING CLIMBOUT, THE CREW RECEIVED AN ELEVATOR ASYMMETRY CAS MESSAGE THEN FLIGHT 2540 RETURNED TO PIT. MAINTENANCE INSPECTED AND FOUND THE RIGHT HAND TRIM CIRCUIT BREAKER POPPED. MAINTENANCE THEN RESET THE CIRCUIT BREAKER IN ACCORDANCE WITH WIRING MANUAL 20-10-00. OPERATIONAL CHECKS WERE PERFORMED AND THE AIRCRAFT WAS RELEASED FOR SERVICE.
DURING FLIGHT, THE CREW RECEIVED A TAKEOFF CONFIGURATION WARNING INDICATION AND RETURNED TO PIT. MAINTENANCE INSPECTED AND REPLACED THE NR 1 IAC IN ACCORDANCE WITH DORNIER MAINTENANCE MANUAL 31-00-00. OPERATIONAL CHECKS WERE PERFORMED AND THE AIRCRAFT WAS RELEASED FOR SERVICE.
THE CREW REPORTED THAT THE EMERGENCY LIGHTING WAS NOT WORKING. MAINTENANCE INSPECTED AND REPLACED THE EMERGENCY BATTERIES IN ACCORDANCE WITH DORNIER MAINTENANCE MANUAL 33-00-00. OPERATIONAL CHECKS WERE PERFORMED AND THE AIRCRAFT WAS RELEASED FOR SERVICE.

THE CREW RECEIVED AN INDICATION THAT THE LEFT HAND PROP WAS ONLY PRODUCING 36 NP. THE FLIGHT THEN RETURNED TO EVV. OUTSTATION MAINTENANCE INSPECTED AND CLEANED THE FEATHER SOLENOID AND ENGINE CONTROL UNIT CONNECTOR AND CYCLED THE I-2 CIRCUIT BREAKER IN ACCORDANCE WITH DORNIER MAINTENANCE MANUAL 73-00-00. OPERATIONAL CHECKS WERE PERFORMED AND THE AIRCRAFT WAS RELEASED FOR SERVICE.
DURING TAKEOFF, THE CREW RECEIVED A NOSE WHEEL STEERING FAIL CAS MESSAGE AND ABORTED TAKEOFF. THE AIRCRAFT WAS THEN FERRIED TO CAK. MAINTENANCE INSPECTED AND REPLACED THE HYDRAULIC STEERING CONTROL IN ACCORDANCE WITH DORNIER MAINTENANCE MANUAL 32-00-00. OPERATIONAL CHECKS WERE PERFORMED AND THE AIRCRAFT WAS RELEASED FOR SERVICE.
DURING THE CLIMBOUT, THE CREW HEARD A BANGING NOISE ON THE FUSELAGE AND RETURNED TO DAY. MAINTENANCE INSPECTED AND FOUND A PIECE OF WEATHER STRIPPING LOOSE AT THE FORWARD FUSELAGE. MAINTENANCE THEN TRIMMED THE STRIPPING IN ACCORDANCE WITH DORNIER MAINTENANCE MANUAL 53-00-00 AND THEN PLACED IT ON THE DMI LIST.
DURING CLIMBOUT, THE CREW RECEIVED AN ELEVATOR TRIM FAIL INDICATION. THE CREW FOLLOWED THE QUICK REFERENCE HANDBOOK AND WERE INSTRUCTED TO DIVERT TO DAY AND LANDED WITHOUT ANY FURTHER INCIDENTS. MAINTENANCE INSPECTED AND REPLACED THE 49CC AND 59CC REGULATORS IN ACCORDANCE WITH DORNIER MAINTENANCE MANUAL 27-00-00. OPERATIONAL CHECKS WERE PERFORMED AND THE AIRCRAFT WAS RELEASED FOR SERVICE.
DURING TAKE OFF, THE AIRCRAFT STARTED PULLING TO THE LEFT AT 70 KNOTS AND THE CREW ABORTED TAKE OFF. MAINTENANCE INSPECTED AND REPLACED THE TORQUE LINKS, PSEU, AND ADJUSTED THE FEED BACK POTS IN ACCORDANCE WITH DORNIER MAINTENANCE MANUAL 32-00-00. OPERATIONAL CHECKS WERE PERFORMED AND THE AIRCRAFT WAS RELEASED FOR SERVICE.
WHEN THE CONDITION LEVERS WERE ADVANCED FOR TAKE OFF, THE AIRCRAFT JUMPED TO THE LEFT, WHEN BROUGHT BACK, JUMPED BACK TO THE RIGHT. THE CREW THEN ABORTED TAKE OFF. MAINTENANCE INSPECTED AND ADJUSTED THE NOSE WHEEL STEERING FEED BACK POTS IN ACCORDANCE WITH DORNIER MAINTENANCE MANUAL 32-00-00. OPERATIONAL CHECKS WERE PERFORMED AND THE AIRCRAFT WAS RELEASED FOR SERVICE.
DURING CLIMB, THE CREW ADVISED DISPATCH THAT THE NORMAL ELEVATOR TRIM WOULD NOT WORK IN THE NOSE DOWN DIRECTION. THE CREW FOLLOWED THE QUICK REFERENCE HANDBOOK AND INDICATED THAT THE STANDBY TRIM WAS WORKING NORMALLY. FLIGHT 4100 WAS THEN INSTRUCTED TO DIVERT TO PIT DUE TO GUSTY WINDS IN DCA. MAINTENANCE INSPECTED AND REPLACED THE 49CC AND 59CC RELAYS IN ACCORDANCE WITH DORNIER MAINTENANCE MANUAL 27-00-00. OPERATIONAL CHECKS WERE PERFORMED AND THE AIRCRAFT WAS RELEASED FOR SERVICE.

OPERATIONAL CHECKS WERE PERFORMED AND THE AIRCRAFT WAS RELEASED FOR SERVICE.
DURING TAKE OFF, THE CREW RECEIVED A LEFT HAND DC GENERATOR FAIL CAS MESSAGE AND ABORTED TAKE OFF. MAINTENANCE INSPECTED AND CLEANED THE GENERATOR CANNON PLUG AND SWAPPED GENERATOR CONTROL UNITS FROM SIDE TO SIDE IN ACCORDANCE WITH DORNIER MAINTENANCE MANUAL 24-00-00. OPERATIONAL CHECKS WERE PERFORMED AND THE AIRCRAFT WAS RELEASED FOR SERVICE.
THE CREW REPORTED THAT THE 9A EMERGENCY FLOOR LIGHT STRIP WAS LOOSE AND INOPERATIVE. MAINTENANCE INSPECTED AND REPAIRED A WIRE ON THE LIGHT STRIP AND RESECURED THE LIGHTING STRIP IN ACCORDANCE WITH DORNIER MAINTENANCE MANUAL 33-00-00. OPERATIONAL CHECKS WERE PERFORMED AND THE AIRCRAFT WAS RELEASED FOR SERVICE.

DURING TAKE OFF THE CREW RECEIVED A LEFT HAND INBOARD BRAKE OVERHEAT INDICATION AND ABORTED TAKE OFF. THE CREW CALLED MAINTENANCE CONTROL AND DEFFERRED THE BRAKE INDICATION. FLIGHT 4015 THEN DEPARTED WITH NO FURTHER INCIDENTS.
DURING TAKEOFF, THE CREW RECEIVED A LEFT HAND ELECTRONIC CONTROL UNIT FAIL MESSAGE AND ABORTED TAKEOFF. THE CREW FOLLOWED THE QUICK REFERENCE HANDBOOK AND RESET THE SYSTEM. FLIGHT 4021 WAS THEN CLEARED FOR ANOTHER TAKEOFF ATTEMPT AND DEPARTED WITH NO FURTHER INCIDENTS.

THE CREW REPORTED THAT THE CARGO SMOKE AND FIRE DETECT LIGHTS WERE INTERMITTENTLY COMING ON. MAINTENANCE INSPECTED AND REPLACED THE NUMBER 2 DATA ACQUISITION UNIT IN ACCORDANCE WITH 31-00-00. OPERATION CHECKS WERE PERFORMED AND THE AIRCRAFT WAS RELEASED FOR SERVICE.
THE CARGO FAIL CAS MESSAGE AND THE RED CARGO FIRE BUTTON CAME ON INTERMITTENTLY. MAINTENANCE INSPECTED AND REPLACED THE DIMMER BOX IN ACCORDANCE WITH DORNIER MAINTENANCE MANUAL 33-00-00. OPERATIONAL CHECKS WERE PERFORMED AND THE AIRCRAFT WAS RELEASED FOR SERVICE.
FLIGHT 4075 WAS CLIMBING THROUGH 15,000 FEET AND THE WEATHER RADAR BEGAN TO MALFUNCTION. THE CREW CONTACTED DISPATCH AND WAS ADVISED TO RETURN TO PIT. MAINTENANCE INSPECTED AND REPLACED THE WEATHER RADAR IN ACCORDANCE WITH DORNIER MAINTENANCE MANUAL 30-00-00. OPERATIONAL TESTS WERE PERFORMED AND THE AIRCRAFT WAS RELEASED FOR SERVICE.
ON CLIMB OUT OF PIT THE RIGHT LOW PRESSURE BLEED VALVE FAILED. AIRCRAFT RETURNED TO PIT AND MAINTENANCE REPLACED THE VALVE AND OPERATIONAL CHECK GOOD
WHILE TAXIING FROM THE RUNWAY AFTER LANDING, THE CREW NOTICED A VIBRATION WHICH WAS GETTING STRONGER. THE CREW STOPPED THE AIRCRAFT AND THE CAPTAIN GOT OUT AND INSPECTED THE AIRCRAFT AND FOUND THAT BOTH THE LEFT SIDE MAIN LANDING GEAR TIRES WERE FLAT. THE PASSENGERS WERE BUSSED TO THE TERMINAL AND MAINTENANCE REPLACED THE LEFT HAND MAIN LANDING GEAR WHEEL ASSEMBLIES IN ACCORDANCE WITH DORNIER MAINTENANCE MANUAL 32-00-00. OPERATIONAL CHECKS WERE PERFORMED AND THE AIRCRAFT WAS RELEASED FOR SERVICE.
DURING LANDING, AT THE OUTER MARKER THE CREW LOWERED THE GEAR BUT DID NOT RECEIVE THREE GREEN GEAR INDICATIONS. THE CREW EXECUTED A GO-AROUND, REFERENCED THE QUICK REFERENCE HANDBOOK, AND A FLY BY SO ATC COULD CONFIRM GEAR WAS DOWN. ANOTHER APPROACH WAS INITIATED AND THE FLIGHT LANDED WITHOUT FURTHER INCIDENT. MAINTENANCE INSPECTED AND REPLACED THE INOPERATIVE GEAR BULB IN ACCORDANCE WITH DORNIER MAINTENANCE MANUAL 33-00-00. OPERATIONAL CHECKS WERE PERFORMED AND THE AIRCRAFT WAS RELEASED FOR SERVICE.
DURING THE TAKEOFF ROLL, THE CREW RECEIVED A FLIGHT DATA RECORDER FAIL CAS MESSAGE AND ABORTED TAKEOFF. THE CREW CLEARED THE RUNWAY AND RESET THE FLIGHT DATA RECORDER. FLIGHT 4011 WAS CLEARED FOR ANOTHER TAKEOFF ATTEMPT AND DEPARTED WITHOUT ANY FURTHER INCIDENTS.
DURING THE TAKEOFF ROLL, THE CREW RECEIVED A CAS MESSAGE FOR LEFT HAND ELECTRONIC CONTROL UNIT FAIL. FLIGHT 4135 ABORTED TAKEOFF. AFTER THE AIRCRAFT CLEARED THE RUNWAY, THE CREW RESET THE ELECTRONIC CONTROL UNIT AND WAS CLEARED FOR ANOTHER TAKEOFF ATTEMPT. FLIGHT 4135 DEPARTED WITH NO FURTHER INCIDENTS.
FLIGHT 4020, WHILE EN ROUTE FROM EVV TO PIT, DIVERTED TO CVG DUE TO SMOKE IN COCKPIT AND CABIN. MAINTENANCE TROUBLESHOT AND DISCOVERED THE AVIONICS FAN WAS FAILING. MAINTENANCE DEFERRED THE AVIONICS FAN AND GROUND RAN THE AIRCRAFT WHICH CHECKED GOOD. FLIGHT 4020 CONTINUED TO PIT WITH NO FURTHER INCIDENTS.
THE CREW RECEIVED A LEFT HAND PUSHER AND SHAKER FAIL CAS MESSAGE. MAINTENANCE CONTROL HAD THE CREW CYCLE A CIRCUIT BREAKER BUT THAT DID NOT FIX THE PROBLEM. MAINTENANCE CONTROL HAD FLIGHT 4171 RETURN TO THE FIELD. MAINTENANCE INSPECTED AND REPLACED THE ANGLE OF ATTACK SENSOR IN ACCORDANCE WITH DORNIER MAINTENANCE MANUAL 27-00-00. OPERATIONAL CHECKS WERE PERFORMED AND THE AIRCRAFT WAS RELEASED FOR SERVICE.
AFTER THE AIRCRAFT TOOK THE RUNWAY, THE FIRST OFFICE NOTICED HIS RUDDER PEDALS WOULD NOT RESPOND. THE CREW DECIDED TO ABORT THE TAKEOFF AND RETURN TO THE GATE. MAINTENANCE FOUND THE RUDDER PEDAL ADJUSTMENT KNOB STUCK. MAINTENANCE CLEANED AND ADJUSTED THE RUDDER ADJUSTMENT KNOB IN ACCORDANCE WITH DORNIER MAINTENANCE MANUAL 27-00-00. OPERATIONAL CHECKS WERE PERFORMED AND THE AIRCRAFT WAS RELEASED FOR SERVICE.
THE CREW REPORTED THAT THE EMERGENCY AFT TRACK LIGHTING WOULD NOT ILLUMINATE. MAINTENANCE INSPECTED AND REPLACED THE EMERGENCY BATTERY AT THE 7LK POSITION. OPERATIONAL CHECKS WERE PERFORMED AND THE AIRCRAFT WAS RELEASED FOR SERVICE.
WHILE EN ROUTE, THE CREW EXPERIENCED AN AIRFOIL DE-ICE FAIL ON THE TAIL IN FAST AND SLOW MODES. MAINTENANCE ADVISED THE CREW TO RETURN TO PIT DUE TO ICING CONDITIONS REPORTED EN ROUTE TO EVANSVILLE. MAINTENANCE INSPECTED AND REPLACED THE EJECTOR FLOW CONTROL IN ACCORDANCE WITH DORNIER MAINTENANCE MANUAL 30-13-06. OPERTIONAL CHECKS WERE PERFORMED AND THE AIRCRAFT WAS RELEASED FOR SERVICE.
THE CREW REPORTED THAT THE EMERGENCY LIGHT STRIP AT ROW 9A WOULD NOT ILLUMINATE. MAINTENANCE INSPECTED AND REPLACED THE EMERGENCY BATTERY AT THE 6LK POSITION IN ACCORDANCE WITH DORNIER MAINTENANCE MANUAL 33-51-01. OPERATIONAL CHECKS WERE PERFORMED AND THE AIRCRAFT WAS RELEASED FOR SERVICE.
THE CREW REPORTED THAT THE EMERGENCY EXIT SIGN AT ROW 9A WAS NOT WORKING. THE AIRCRAFT WAS FERRIED TO PIT WHERE MAINTENANCE INSPECTED AND REPLACED THE EMERGENCY BATTERY AT THE 6LK POSITION IN ACCORDANCE WITH DORNIER MAINTENANCE MANUAL 33-51-00. OPERATIONAL CHECKS WERE PERFORMED AND THE AIRCRAFT WAS RELEASED FOR SERVICE.
DURING TAKEOFF, FLT 9469 EXPERIENCED SUDDEN DECELERATION DUE TO HITTING HARD SLUSH, FOLLOWED BY A DOOR AND AHARS CAS MESSAGES. THE CREW THEN ABORTED TAKEOFF. MAINTENANCE HAS INSPECTED THE AIRCRAFT AND IS WAITING FOR REPAIR INSTRUCTIONS FROM THE MANUFACTURER. REPAIRS WERE COMPLETED BY DUNCAN AVIATION ON WORK ORDER WT6AA. THESE RECORDS ARE ON FILE AT PSA AIRLINES.
WHILE EN ROUTE, THE FLIGHT CREW RECEIVED A RED WARNING PANEL DOORS ALERT AND A MAIN CABIN DOOR CAS MESSAGE. MAINTENANCE CONTROL INSTRUCTED THE CREW TO RETURN TO THE FIELD WHERE THE AIRCRAFT LANDED WITHOUT FURTHER INCIDENT. MAINTENANCE INSPECTED AND CLEANED THE SHOOT BOLT IN ACCORDANCE WITH DORNIER MAINTENANCE MANUAL 52-00-0O THERE WERE NO FURTHER DEFECTS NOTED AND THE AIRCRAFT WAS RETURNED TO SERVICE.
THE CREW REPORTED THE AIRCRAFT BRAKES WERE PULSATING AND WOULD NOT STOP THE AIRCRAFT ADEQUATELY. MAINTENANCE INSPECTED AND REPLACED THE BRAKE METERING UNIT, IN ACCORDANCE WITH DORNIER MAINTENANCE MANUAL 32-42-04. OPERATIONAL CHECKS WERE COMPLETED AND THE AIRCRAFT WAS RETUNED TO SERVICE.
DURING THE INSPECTION FOR HARTZELL ALERT SERVICE BULLETIN HD-ASB-61-021, A SUSPECTED CRACK WAS NOTED ON THE RIGHT HAND PROPELLER HUB. MAINTENANCE REPLACED THE PROPELLER, IN ACCORDANCE WITH DORNIER MAINTENANCE MANUAL 61-10-00. OPERATIONAL CHECKS WERE COMPLETED, AND THE AIRCRAFT WAS RETURNED TO SERVICE.
THE AIRCRAFT RETURNED TO THE FIELD AFTER RECEIVING A FLIGHT DIRECTOR, FLIGHT GUIDANCE AND A LEFT ATTITUDE FAIL WARNING MESSAGES. MAINTENANCE INSPECTED AND REPLACED THE NR 1 ATTITUDE HEADING REFERANCE (AHRS) COMPUTER, IN ACCORDANCE WITH DORNIER MAINTENANCE MANUAL 34-21-20. OPERATIONAL CHECKS WERE SATISFACTORY, AND THE AIRCRAFT WAS RETURNED TO SERVICE.
THE LEFT HAND POWER LEVER FAILED TO ADVANCE ABOVE FLIGHT IDLE WHEN SELECTED DURING THE TAKEOFF ROLL. THE TAKEOFF WAS ABORTED AND THE AIRCRAFT RETURNED TO THE GATE. THE AIRCRAFT RETURNED TO THE GATE FOLLOWING A CARGO DOOR WARNING MESSAGE. MAINTENANCE INSPECTED AND CLEANED THE DOOR SENSORS, IN ACCORDANCE WITH DORNIER MAINTENANCE MANUAL 52-41-00. OPERATIONAL CHECKS WERE COMPLETED, AND THE AIRCRAFT WAS RETURNED TO SERVICE.
THE AIRCRAFT RETURNED TO THE GATE AFTER A LEFT HAND BLEED FAIL WARNING MESSAGE. MAINTENANCE INSPECTED AND REPLACED THE LEFT HAND LOW PRESSURE BLEED VALVE, IN ACCORDANCE WITH DORNIER MAINTENANCE MANUAL 36-11-07. OPERATIONAL CHECKS WERE COMPLETED, AND THE AIRCRAFT WAS RETURNED TO SERVICE.
THE RIGHT HAND PROPELLER FAILED TO COME OUT OF THE FEATHER POSITION, WHEN SELECTED. DURING THE ATTEMPT TO GET THE PROPELLER OUT OF FEATHER, THE RIGHT ENGINE WAS OVER TORQUED. THE AIRCRAFT RETURNED TO THE GATE. MAINTENANCE INSPECTED AND COMPLIED WITH A OVER TORQUE INSPECTION OF THE RIGHT HAND ENGINE, IN ACCORDANCE WITH DORNIER MANTENANCE MANUAL 05-57-01. MAINTENANCE INSPECTED AND REPLACED THE RIGHT HAND PROPELLER CONTROL UNIT, IN ACCORDANCE WITH DORNIER MAINTENANCE MANUAL 61-21-06. OPERATIONAL CHECKS WERE COMPLETED, AND THE AIRCRAFT WAS RETURNED TO SERVICE.
A LEFT HAND AIR CONDITIONING PACK OVERHEAT WARNING MESSAGE ILLUMINATED, DURING THE TAXI OUT FOR TAKEOFF. THE AIRCRAFT RETURNED TO THE GATE. OUTSTATION MAINTENANCE INSPECTED, AND APPROVED THE AIRCRAFT FOR A FERRY FLIGHT TO PIT. THE FERRY FLIGHT TOOK PLACE WITHOUT ANY PROBLEMS. PIT MAINTENANCE INSPECTED AND REPLACED THE LEFT HAND NACA DUCT CHECK VALVE, IN ACCORDANCE WITH DORNIER MAINTENANCE MANUAL 21-51-24. OPERATIONAL CHECKS WERE COMPLETED, AND THE AIRCRAFT WAS RETURNED TO SERVICE.
DURING TAXI, THE LEFT PROPELLER WHEN SELECTED, WOULD NOT COME OUT OF FEATHER. THE CREW ATTEMPTED A SECOND TIME TO UNFEATHER THE PROPELLER. THE PROPELLER UNFEATHERED, BUT WITH AN RPM AND TORQUE SURGE. THE TORQUE SURGE WAS LARGE ENOUGH TO OVER TORQUE THE PROPELLER AND ENGINE. MAINTENANCE INSPECTED AND REPLACED THE LEFT PROPELLER CONTROL UNIT, IN ACCORDANCE WITH DORNIER MAINTENANCE MANUAL 61-21-06. MAINTENANCE ALSO COMPLETED AN OVERTORQUE INSPECTION ON THE PROPELLER AND ENGINE, IN ACCORDANCE WITH DORNIER MANUAL 05-57-01. OPERATIONAL CHECKS WERE COMPLETED, AND THE AIRCRAFT WAS RETURNED TO SERVICE.
A RIGHT HAND AIR CONDITIONING PACK FAIL WARNING MESSAGE ILLUMINATED, DURING THE TAXI OUT FOR TAKEOFF. THE AIRCRAFT RETURNED TO THE GATE. MAINTENANCE INSPECTED AND REPLACED THE RIGHT HAND NACA DUCT CHECK VALVE, IN ACCORDANCE WITH DORNIER MAINTENANCE MANUAL 21-51-24. OPERATIONAL CHECKS WERE COMPLETED AND THE AIRCRAFT WAS RETURNED TO SERVICE.
WHILE AWAITING CLEARANCE FOR TAKEOFF, A FLAP CONTROL FAIL WARNING MESSAGE ILLUMINATED. THE AIRCRAFT RETURNED TO THE GATE. OUTSTATION MAINTENANCE PERFORMED A RE-RACK OF THE FLAP CONTROL UNIT AND COMPLIED WITH OPERATIONAL CHECKS, IN ACCORDANCE WITH DORNIER MAINTENANCE MANUAL 27-52-00. THE OPERATIONAL CHECKS WERE SATISFACTORY, AND THE AIRCRAFT WAS RETURNED TO SERVICE.
DURING THE TAXI PRIOR TO TAKEOFF, A ANTI-SKID FAIL WARNING MESSAGE ILLUMINATED. THE AIRCRAFT RETURNED TO THE GATE. MAINTENANCE INSPECTED AND REPLACED THE LEFT HAND INBOARD WHEEL SPEED TRANSDUCER, IN ACCORDANCE WITH DORNIER MAINTENANCE MANUAL 32-42-11. OPERATIONAL CHECKS WERE COMPLETED, AND THE AIRCRAFT WAS RETURNED TO SERVICE.
A STICK PUSHER FAIL WARNING OCCURED DURING THE TAXI OUT FOR TAKEOFF. THE AIRCRAFT RETURNED TO THE GATE. OUTSTATION MAINTENANCE INSPECTED AND APPROVED THE AIRCRAFT FOR A FERRY FLIGHT TO DCA, TO ACCOMPLISH REPAIR. THE FERRY FLIGHT TOOK PLACE WITHOUT INCIDENT. MAINTENANCE WAS DISPATCHED WITH PARTS TO DCA FROM PIT, TO MEET THE AIRCRAFT. MECHANIC INSPECTED AND REPLACED THE LEFT HAND ANGLE OF ATTACK SENSOR, IN ACCORDANCE WITH DORNIER MAINTENANCE MANUAL 27-33-01. OPERATIONAL CHECKS WERE COMPLETED, AND THE AIRCRAFT WAS RETURNED TO SERVICE.
DURING THE TAKEOFF, THE CREW NOTED SOME VIBRATIONS FROM THE NOSE LANDING GEAR. WHEN THE FLIGHT WAS IN RANGE OF BTV, THE CREW ADVISED TOWER THAT THEY MAY HAVE A FLAT, OR OUT OF BALANCE NOSE TIRE. THE TOWER DECLARED AN EMERGENCY, AND DISPATCHED EQUIPMENT. THE FLIGHT LANDED WITHOUT INCIDENT IN BTV, AND TAXIED TO THE GATE. MAINTENANCE INSPECTED AND REPLACED THE NOSEWHEELS IN ACCORDANCE WITH DORNIER MAINTENANCE MANUAL 32-41-01. THE AIRCRAFT WAS RETURNED TO SERVICE. PART SERIAL NUMBER B-0266 TOTAL TIME 3958.8, TOTAL LANDINGS 3617, TIME SINCE OVERHAUL 15.1, SERIAL NUMBER B-0394 STATS GIVEN BELOW.
DURING TAXI OUT, THE CREW NOTICED A FUEL CONNECT VALVE OPEN WARNING. THE CREW RETURNED TO THE GATE AND MAINTENANCE WAS CALLED OUT. MAINTENANCE INSPECTED AND CYCLED SWITCHES ON THE FUEL PANEL, OPERATION CHECKS WERE GOOD PER DORNIER MAINTENANCE MANUAL 73-00-00.
DURING TAKEOFF ROLL, A BLEED WARNING MESSAGE ILLUMINATED. THE CREW ABORTED TAKEOFF AND RETURNED TO THE GATE WITHOUT FURTHER INCIDENT. MAINTENANCE INSPECTED AND REPLACED THE LEFT HIGH PRESSURE BLEED SHUTOFF VALVE IN ACCORDANCE WITH DORNIER MANTENANCE MANUAL 36-11-07. FUNCTION CHECKS WERE GOOD AND THE AIRCRAFT WAS RETURNED TO SERVICE.
ON TAKEOFF ROLL, RIGHT PROP WAS STRUCK BY A BIRD. ABORTED TAKEOFF AND RETURNED TO THE GATE WITHOUT FURTHER INCIDENT. MAINTENANCE INSPECTED AIRCRAFT AND NO DAMAGE WAS FOUND. AIRCRAFT WAS APPROVED FOR RETURN TO SERVICE.
BEGAN TO ADVANCE POWER FOR TAKEOFF AND RECEIVED A LT BLEED SOV FAIL CAS MESSAGE. REJECTED TAKEOFF AND RETURNED TO THE GATE. MAINTENANCE REMOVED AND REPLACED THE LT HP BLEED VALVE, OPERATIONAL CHECKS GOOD.
RADAR FAILED AT 22,000 FT, RETURNED TO FIELD WITHOUT INCIDENT. MAINTENANCE REMOVED AND REPLACED THE RADAR UNIT, OPERATIONAL CHECKED GOOD.
CREW REPORTED STRIKING A LARGE BIRD DURING THE TAKEOFF ROLL. TAKEOFF WAS ABORTED AND AIRCRAFT RETURNED TO THE GATE. OUT STATION MX INSPECTED THE AIRCRAFT FOR DAMAGE I.A.W. M.M. 05-10-00 , NO DEFECTS NOTED, THE AIRCRAFT WAS RETURNED TO SERVICE.
CREW REPORTED, WHEN ADVANCING THE POWER LEVERS, THE LEFT POWER LEVER JAMMED JUST ABOVE THE FLIGHT IDLE GATE. AIRCRAFT RETURNED TO THE GATE. MX LUBRICATED THE LEFT POWER LEVER PRIMARY FLIGHT IDLE LOCK OUT MECHANISM IAW MM 76-10-00 AND STD MX PRACTICES, GROUND CHECKED GOOD. AIRCRAFT RETURNED TO SERVICE.
DAY - FLT 4008 - CREW UNABLE TO RETRACT THE LANDING GEAR AFTER TAKEOFF. CYCLED THE SELECTOR HANDLE SEVERAL TIMES WITHOUT SUCCESS. AIRCRAFT RETURNED TO THE FIELD, LANDED WITHOUT INCIDENT. DAY MX REPLACED THE LANDING POSITION SELECTOR HANDLE I.A.W. M.M.32-30-00, GEAR SWING OPS CHECKED GOOD. AIRCRAFT RETURNED TO SERVICE. PART TOTAL CYCLES, 16.
LANDING GEAR WOULD NOT RETRACT AFTER TAKEOFF, CREW CYCLED THE GEAR POSITION SELECTOR HANDLE SEVERAL TIMES WITHOUT SUCCESS. CREW EXTENDED THE LANDING MANUALY AND RETURNED TO THE FIELD. AIRCRAFT LANDED WITHOUT FURTHER INCIDENT. MX REPLACED THE LANDING SELECTOR HANDLE IAW MM 32-30-00. GEAR SWING OPS, CHECKED GOOD.
CLIMBING THROUGH FL 160, RT GENERATOR FAILED. CREW WAS THEN ADVISED TO RETURN TO FIELD. AIRCRAFT LANDED WITHOUT FURTHER INCIDENT. REPLACED THE RT STARTER/GENERATOR IAW MM 24-30-00, GROUND CHECKED GOOD.
FLT 4028 - PROPELLER HEAT FAILED SHORTLY AFTER TAKEOFF. AIRCRAFT RETURNED TO FIELD, LANDED WITHOUT FURTHER INCIDENT. PIT MX CLEANED THE PROPELLER DEICE BRUSH BLOCK ASSEMBLIES (BOTH SIDES) IAW MM 61-00-01. GROUND CHECKED GOOD, AIRCRAFT RETURNED TO SERVICE. (X)
