DURING ROUTINE INSPECTION, FOUND FORWARD FLAP HANDLE MOUNTING BRACKET CRACKED, BRACKET IS LOCATED IN THE TUNNEL AND IS THE MOUNTING POINT FOR THE 2 FORWARD BOLTS FOR THE FLAP HANDLE ASSEMBLY.

PILOTS NOTICED FUEL POURING FROM THE ENGINE COWLING OF THE AIRCRAFT; IT ONLY STOPPED AFTER THE FUEL SELECTOR WAS TURNED TO OFF. INSPECTION REVEALED THE CARBURETOR FLOAT WAS STUCK DOWN OR HAD SUNKEN, CAUSING FUEL TO OVERFLOW FROM THE CARB BOWL. HAD THIS CONDITION PRESENTED IN FLIGHT, IT WOULD HAVE LIKELY CAUSED A LOSS OF ENGINE POWER DUE. THIS CARB WAS INSTALLED ONLY 6 DAYS PRIOR.

DURING A ROUTINE INSPECTION, A LARGE CRACK WAS DISCOVERED IN THE LT MLG BETWEEN THE TORQUE LINK MOUNTING EARS. IF ALLOWED TO CONTINUE, IT IS LIKELY THE EAR WOULD HAVE SEPARATED, ALONG WITH THE LOWER END OF THE TORQUE LINKS. ALTHOUGH THE CRACK HAD PROGRESSED TO THE POINT IT WAS READILY IDENTIFIABLE, IT WOULD BE POSSIBLE FOR THE CRACK TO BE OBSCURED BY GREASE AND BE HARD TO DETECT.

Part: BRACKET — CRACKED (ZONE 100) · airframe 17,100 hrs

DURING A PROGRESSIVE INSPECTION EVENT, A BRACKET WAS FOUND CRACKED BELOW WHERE THE FLAP HANDLE IS MOUNTED. THE BRACKET IN QUESTION IS LOCATED IN THE TUNNEL IN THE CENTER OF THE CABIN FLOOR AND IS USED TO MOUNT THE FLAP HANDLE ASSEMBLY. IT CONTAINS NUT PLATES TO ALLOW THE HANDLE TO BE BOLTED THROUGH THE TUNNEL. THE CRACK IS IN THE UPPER RT CORNER OF THE BRACKET, BETWEEN THE NUT PLATE AND THE EDGE OF THE METAL. IT IS SPECULATED THAT THE CRACK IS CAUSED BY THE MOVEMENT OF THE FLAP HANDLE CAUSING STRESS TO THE BRACKET.

DURING A ROUTINE INSPECTION, A MECHANIC NOTED THE AFT STIFFING CHANNEL, PN 62086-002, CRACKED AT THE MIDDLE LIGHTENING HOLE. ADDITIONAL INSPECTION REVEALED NO FURTHER DAMAGE. THE CRACKS APPEAR NEW. THIS CRACK IS NOT TYPICAL OF THE CRACKS FOUND PREVIOUSLY. THIS IS A MFG INSTALLED PART.

Part: SCREW — BACKED OUT (CONTROL WHEEL)

THE SCREW THAT HOLDS THE CONTROL WHEEL TO THE YOKE WAS FOUND TO BE BACKED OUT ON THE PILOTS WHEEL BY A .2500 TURN. WHEN THE SCREW BACKS OUT ENOUGH, IT FALLS OUT ALLOWING THE WHEEL TO ROTATE FREELY AROUND THE YOKE. THIS RESULTS IN NO FLIGHT CONTROL INPUT. PROBABLE CAUSE OF THE SCREW BACKING OUT IS THAT THE SCREW IS TOO SHORT. THE MS24964S59 SCREW IS 1.0937 INCH LONG WITH ONLY HALF A THREAD SHOWING THROUGH THE NUTPLATE.
