FORWARD SHELL ASSEMBLY OUTER SKIN WAS FOUND TO BE CORRODED -LOCAL TO AN EXISTING INSERT LOCATION. THE REPAIR DOCUMENTATION WILL BE SUBMITTED FOR FAA APPROVAL. THE FORWARD SHELL ASSEMBLY WILL THEN BE REPAIRED IN ACCORDANCE WITH THE FAA-APPROVED REPAIR DATA.
ENGINE REMOVED 12-25-18 DUE TO METAL IN OIL. DETERMINED CAUSE TO BE TURBINE NR 5 BEARING SPALLING. INSTALLED NEW BEARING. PART WILL BE RETURNED FOR EVALUATION ON SHIPPING NUMBER 4904011120191.
ENGINE REMOVED 8-20-16 FOR NR 5 BEARING UPGRADE. NR 5 BEARING EXHIBITED SPALLING AT OUTER RACE SPLIT. REPLACED BEARING.
ENGINE REMOVED DUE TO A CRACKED EXHAUST COLLECTOR. NR 5 BEARING PROBABLE SPALLING ON OUTER RACE SPLIT. RETURNED TO MFG FOR FURTHER EVALUATION.
ENGINE REMOVED 11-25-14 DUE TO TURBINE OVERHAUL. NR 5 BEARING EXHIBITED SPALLING BALLS. PART RETURNED TO ROLLS ROYCE FOR FURTHER EVALUATION.
APPROXIMATELY 5 MINUTES AFTER TAKEOFF, THE ENGINE FAILED AND AUTOROTATION WAS COMPLETED TO THE WATER. ENGINE WAS REMOVED DUE TO ENGINE SHUTDOWN. SUSPECT 3RD STAGE WHEEL FAILURE, 3RD STAGE WHEEL FOUND WITH ALL BLADES FRACTURED AT ROOT. TURBINE WAS SHIPPED TO MFG FOR FAILURE ANALYSIS.
METAL, THE CHIP LIGHT WAS CAUSED BY SLIVER OF METAL CREATED WHEN MFG INSTALLED BEARING 406-040-037-103 INTO THE LOWER MAST HOUSING 407-040-054-101. REPLACED TRANSMISSION. REQUESTS FOR WARRANTY CREDIT FOR PARTS AND LABOR DUE TO METAL SLIVER BEING CREATED DURING ASSEMBLY AT MFG.
