HEATER STUCK ON IN THE ON POSITION. WHEN MORE COLLECTIVE RAISED, FELT MORE HEAT, ALSO VERY FAINT SMELL OF SOMETHING BURNING. VALVE HANDLE LOOSE. DEBRIS IN HEATER DUCT EMITTED ODOR WHEN HOT.
EXPERIENCED A LOUD SQUEAL FOLLOWED BY A HYDRAULIC CAUTION LIGHT FOLLOWED BY FEEDBACK IN FLIGHT CONTROLS. TAIL ROTOR SERVO PRESSURE LINE RUPTURE. REPLACED HYDRAULIC PUMP DUE TO RUN DRY.
ENGINE REMOVED DUE TO METAL IN OIL. PTO BALL BEARING EXHIBITED SEVERE SPALLING OF INNER RACE. REPLACED PTO BALL BEARING.
ENGINE REMOVED 10-9-16 FOR NR 5 BEARING REPLACEMENT. NR 5 BEARING EXHIBITED SPALLED OUTER RACE AT SPLIT. INSTALLED UPGRADED NR 5 BEARING.
ENGINE DISASSEMBLED 9-26-15 DUE TO METAL IN OIL. NR 5 BEARING REMOVED DUE TO SPALLING ON BALLS, CRACKED AND ERODED SEPERATOR. REPLACED NR 5 BEARING. PART WILL BE RETURNED TO MFG FOR FURTHER EVALUATION.
ENGINE REMOVED 11-6-14 DUE TO METAL IN OIL. SPALLED OUTER RACE, SEVERAL BALLS AND WORN SEPERATOR POCKET.
IMPELLER BEARING NR 5 EXHIBITED SPALLING OUTER RACE. REPLACED WITH SERVICEABLE BEARING.
