DURING ROUTINE INSPECTION, A CRACK WAS FOUND ON THE 10L FRAME AT THE 10 OCLOCK CUTOUT FOR THE OMEGA DOUBLER ON THE UPPER PORTION, EXTENDING UP .5000 INCHES THEN GOING ACROSS .6250 INCHES TO THE TOOLING HOLE, AND STARTING UP AGAIN AT THE OTHER END OF THE TOOLING HOLE AND EXTENDING 1 INCH AND THEN STOPPING AT A RIVET HOLE.
SPAS-COMPUTER, DC VOLTAGE IN, NO VOLTAGE OUT. REMOVED AND REPLACED.
DIRECTIONAL GYRO, 20 DEGREES OFF ON READING, BETWEEN POLES. REMOVED AND REPLACED.
COMMUNICATION SYSTEM, RADIO WOULD NOT CHANGE FROM RT1 TO RT2 AND SHOWED FAULT. REMOVED AND REPLACED.
N2 ON NR 2 ENGINE SIDE DRAGS DOWN. REMOVED AND REPLACED. (X)
DISASSEMBLY OF ENGINE REVEALED METAL IN BOTTOM OF GEARBOX ASSY. INSPECTION REVEALED IDLER GEAR TO HAVE ONE EACH NUT MISSING FROM ASSY BOLTS AND 3 EACH NUTS EITHER DAMAGED OR PARTIALLY MISSING PIECES. TORQUE ON SEVERAL REMAINING BOLTS WERE LOOSE. SUBMITTER RECOMMENDED RE-TORQUE OF1,200 HOUR INTERVAL CHECKS. REPLACED ANY LOOSE NUTS AND BOLTS. NUT P/N: MS21043-5. BOLT P/N: 4-081-119-04. ENGINE TT: 2,943.8 HOURS.
TRIPLE TACH NR 2 ENGINE RPM INTERMITTENT. REMOVED AND REPLACED.
STORMSCOPE UNIT NOT RECEIVING INPUT. REMOVED AND REPLACED.
SUBMITTER STATES EXCESSIVE FUEL MANIFOLD LEAKS OVER THE LAST 18 MONTHS. ALLIED SIGNAL IS IN THE PROCESS OF RELEASING AN ASB LIMITING THE LIFE OF THE FUEL MANIFOLD TUBES. OTHER OPERATORS SUBMITTER CONTACTED ARE HAVING THE SAME PROBLEMS. OMNIFLIGHT HAS GONE TO A POST-FLIGHT CHECK AND A SHIFT-CHANGE CHECK OF THIS MANIFOLD.
REMOVED NR 2 STARTER/GENERATOR. FAN CHAFED HOLE IN SCREEN. REMOVED AND REPLACED.
DIRECTIONAL GYRO DRIFTS OFF HEADING. FAILED AFTER ONLY 4.5 HOURS OPERATION.

TAIL ROTOR GEARBOX OUTPUT SHAFT SILVER PLATING MISSING ON THREE SPOTS ON INSIDE DIAMETER OF SHAFT. NOT ACCEPTABLE IAW CURRENT REM 305 MANUAL. PART IS NO GOOD AS IS.
COMPUTER CAUSING SPAS NOT TO TEST. UNIT ONLY HAS 13.90 HOURS, SINCE REPAIR.
KNOB HARD TO TURN, FREQUENCY DOES NOT ADJUST PROPERLY, SOMETIMES UP, SOMETIMES DOWN. REMOVED AND REPLACED.
COMPASS SYSTEM GIVING ERRONEOUS HEADING INDICATION. ALSO, NR 2 RMI COMPASS WAS INOPERATIVE TO SLAVING MODE.
PRECESSES - WILL NOT HOLD ATTITUDE. OVERHAUL DONE ON 8-23-94, INV NR 62281 AND TESTED ON P/O 88573, ORDER NR 75988.
LARGE HOLE ON OUTSIDE OF FACE OF END PLATE. WAS REPAIRED 1/5/95 FOR CRACKS IN SAME AREA.
VERT FIN ENDPLATE CRACKED AT KNOCKOUT PLUG.
