THE RT MLG FREEFELL TO THE DOWN AND LOCKED POSITION WHILE THE GEAR WAS BEING EXTENDED. INSP REVEALED FAILURE OF THE NUT INSIDE ASSY NR 50-8103672. REPLACED BOTH RT AND LT MAIN GEAR ACTUATORS.
DURING A RECURRENT INSP REQUIRED BY AD 2004-23-02C, A CRACK WAS FOUND AT THE LOCATION ILLUSTRATED. WERE THE WELD MEETS THE SURFACE, AFTER USING DYE PENETRANT THE CRACK BECAME VISIBLE.
ON STARTUP, ONCE THE BENDIX GEAR ENGAGED, A LOUD NOISE WAS HEARD, WHICH SOUNDED LIKE GEARS GRINDING. UPON CLOSER INSP, NO TEETH REMAINED ATTACHED TO THE BENDIX. THE HAD ALL SHEARED OFF THE CASING AND THE CASING WAS LOOSE. THE SNAP RING WHICH NORMALLY HELD IT DOWN AT THE BACK, WAS OFF ALSO. (K)
PART REMOVED FOR OVERHAUL AFTER INTERMITTENT OPERATION. (K)
(2) OF THE HINGE LINKS HAD CRACKED OFF ON ONE END. UPON DISASSEMBLY IT WAS DETERMINED THAT A CRACK HAD FORMED LENGTHWISE BETWEEN THE FASTENERS AND THE HINGE LINKS. POSSIBLE CAUSE MAY HAVE BEEN FATIGUE. (K)
A CRACK WAS FOUND ON THE RT BELLCRANK SUPPORT ASSY. DURING A MANDITORY 5 YEAR INSPECTION IAW MSB 2231 REV 1. (K)
SMALL PARTICLES OF METAL FOUND IN OIL. IDENTIFIED NR 5 AND NR 6 CAM AND ASSOCIATED LIFTERS WORN. REMOVED FOR REPAIR. RECOMMEND THE USE OF 16702 ENGINE OIL ADDITIVE ON A REGULAR BASIS. (K)

PUMP FAILED UNDER NORMAL OPERATING CONDITIONS (VFR DAY).
PRIMARY COIN HOLD DOWN RETAINERS LOOSE WITHIN MAGNETO CASE. THE LOOSE RETAINERS, IN TURN, GOT CAUGHT IN THE TIMING WHEEL/GEAR AND CHEWED UP SEVERAL TEETH. PROBABLE CAUSE- THE RETAINERS WERE NOT INSTALLED PROPERLY. FORTUNATELY THE DAMAGE WAS AT THE R20 TIMING MARK, WHERE WE WERE ABLE TO SEE THE DAMAGE DURING A 10 HOUR (ENGINE BREAK IN) VISUAL INSPECTION.
