Maintenance disclosures
DURING CRUISE FLIGHT, SMOKE WAS COMING OUT OF AIR CONDITIONING VENT. THE PILOT SHUT THE AIR CONDITIONING OFF. LANDED IN FIELD. NOTED SMOKE CLEARED ONCE A/C WAS SHUT OFF. AMT INSPECTED THE A/C SYSTEM. REMOVED AND REPLACED THE AFT AIR CONDITIONER BLOWER MOTOR. THE AIR CONDITIONING OPS CHECKED WITH NO DEFECTS NOTED.
THE PILOT WAS ON FINAL APPROACH, WHEN AIRCRAFT EXPERIENCED COMPLETE AVIONICS FAILURE. ALL SCREENS WENT BLACK, SMOKE IN COCKPIT COMING FROM THE OVERHEAD CIRCUIT BREAKER PANEL. PILOT WAS ABLE TO MAINTAIN A NORMAL LANDING PROCEDURE DURING THIS EVENT. ON LANDING, AN INSPECTION OF THE OVERHEAD CIRCUIT BREAKER PANEL AREA YIELDED A CHAFED WIRE AGAINST COMM 1 RADIO CONNECTOR. THIS RESULTED IN A SHORT IN THE AREA CONTAINED BETWEEN THE OVERHEAD CIRCUIT BREAKER PANEL AND THE UPPER ROOF SKIN. THE WIRE IN QUESTION RUNS FROM AVIONICS MASTER RELAY TO THE AVIONICS BUSS.
PANEL 422BT HAS CRACK SEAL RETAINER. R & R SEAL RETAINER ON PANEL 422BT IAW SRM 51-42-06.
SMALL HOLE FOUND ON BOTTEM OF TAIL BOOM AFT OF 2ND FRAME IN TAIL BOOM. 556MM AFT OF TAIL BOOM FLANGE. REPAIRED IAW MFG REPAIR SCHEME MG110220053 NO REVISION, DATED 11/2/05 AND FAA FORM 8110-03 FILE NR DERY-710242-SW/05/331 DATED 11/2/2005. FAA FORM 337 SUBMITTED 11/4/2005.
AIRCRAFT WAS ON APPROACH AND THE LOWER PORTION OF THE WINDSCREEN CRACKED THROUGH, THE LOWER PORTION OF THE WINDSCREEN FROM THE JOGGLE POINTS IN LOWERED INTO THE CABIN. DID NOT COME LOOSE FROM THE BONDING MATERIEL. REPAIRED I/A/W EUROCOPTER STANDARD PRACTICE MANUAL SECTION 20.03.05.401. REPLACED WINDSCREEN WITH NEW. WEATHER COLD UNDER 20 DG F.
AIRCRAFT WAS ON APPROACH AND THE LOWER PORTION OF THE WINDSCREEN CRACKED THROUGH, THE LOWER POTION OF THE WINDSCREEN FROM THE JOGGLE POINTS IN LOWERED INTO THE CABIN. DID NOT COME LOOSE FROM THE BONDING MATERIAL. REPAIRED IAW MM, REPLACED WINDSCREEN WITH NEW. WEATHER COLD UNDER 20 DEGREES F. THIS IS SECOND WINDSCREEN TO CRACK IN FLIGHT IN COLD WEATHER. (K)
DURING 100 HOUR INSPECTION, A HAIR LINE CRACK WAS NOTED ON THE LEFT HAND SIDE OF THE NR 2 TRDS BEARING SUPPORT (FROM FWD). ON REMOVAL THE CRACK WENT THROUGH THE SUPPORT ARM. CRACK LOCATED 32MM ABOVE THE CENTERLINE OF ATTACHMENT BOLT HOLE. THE SUPPORT IS STAINLESS AND THE CRACK IS DIFFICULT TO SEE. REMOVED SUPPORT AND REPLACED WITH NEW.
