DURING MAINTENANCE A CRACK WAS VISUALLY DISCOVERED ON THE INBOARD FACE OF THE RIGHT HAND INBOARD ENGINE BEAM IN THE AREA OF THE VERTICAL ATTACH CHANNELS FOR THE CROSSOVER BEAM. THIS CRACK WAS DISCOVERED AFTER CHANGING THE CROSSOVER BEAM VERTICAL ATTACH BRACKET P/N: 5654107-15 AND THEREFORE NOT VISIBLE UNTIL THE BRACKET WAS REMOVED. THE CRACK MEASURES APPROXIMATELY 1.210 INCHES LONG, AND IS CONSISTENT WITH OTHERS FOUND IN THIS AREA USING A DETAILED VISUAL INSPECTION. THIS BEAM HAS BEEN REPLACED USING NEW PARTS.
DURING MAINTENANCE AN AREA OF CORROSION WAS VISUALLY IDENTIFIED ON THE TOP EDGE OF THE VERTICAL LEG OF THE FORWARD LEFT HAND LOWER AFT WING FITTING. CORROSION MEASURES APPROXIMATELY 1.25 INCHES LONG, WITH A MAX DEPTH OF APPROXIMATELY .017 INCHES DEEP AND IS CONSISTENT WITH CORROSION DAMAGE FOUND ON THE SAME FITTING ON PAST AIRCRAFT. THIS FITTING WILL BE REMOVED AND REPLACED USING NEW PARTS.
THE PILOT REPORTED THE HEATER PUTTING OUT AN ODOR DURING FLIGHT. MAINTENANCE INSPECTED AND FOUND THE HEATER CABIN FAN TO BE BROKEN. THIS WAS REPLACED WITH NO FURTHER DEFECTS NOTED.
THE PILOT REPORTED A FUEL SMELL IN THE CABIN DURING FLIGHT WITH THE HEATER ON. MAINTENANCE INSPECTED AND FOUND THE HEATER FUEL PUMP LEAKING DURING OPERATION. THIS WAS REMOVED AND REPLACED WITH NO FURTHER DEFECTS NOTED.
DURING APPROACH THE NOSE LANDING GEAR WOULD NOT EXTEND. AIRCRAFT LANDED WITH NOSE GEAR RETRACTED. NO INJURIES. INSPECTION FOUND THE NOSE GEAR DOOR RETURN SPRING TO BE BROKEN, WHICH CAUSED THE NOSE GEAR DOOR RETRACT BELLCRANK TO BECOME OUT POSITION JAMMING THE NOSE GEAR DOORS CLOSED AND UNABLE TO EXTEND.
THE PILOT REPORTED A BURNING SMELL IN THE CABIN IN FLIGHT WITH THE HEATER TURNED ON. MAINTENANCE INSPECTED AND REPLACED THE HEATER ASSEMBLY WITH NO FURTHER DEFECTS NOTED.

AT SCHEDULED INSPECTION, NLG DRAG BRACE, OEM PN 5142002-5, WAS FOUND TO BE CRACKED IN THE AREA OF NLG ACTUATOR ATTACH POINT. METHOD OF INSPECTION WAS DYE PENETRANT. THIS CRACK WAS FOUND TO BE BEYOND LIMIT ALLOWABLE. THIS UNIT HAS BEEN SCRAPPED & A REPLACEMENT DRAG BRACE HAS BEEN INSTALLED IN ACFT.
AT SCHEDULED INSPECTION, NLG DRAG BRACE, PN 5142002-5 WAS FOUND TO BE CRACKED IN THE AREA OF NLG ACTUATOR ATTACH POINT. METHOD OF INSPECTION WAS DYE PENETRANT. THIS CRACK WAS FOUND TO BE BEYOND LIMIT ALLOWABLE OF MEB91-11R1. THIS UNIT HAS BEEN SCRAPPED & A REPLACEMENT DRAG BRACE HAS BEEN INSTALLED IN ACFT.
PILOT REPORTED THAT DURING CRUISE FLIGHT, BOTH ALTERNATORS DROPPED OFF LINE AND A SMELL OF SMOKE WAS NOTED IN THE COCKPIT. THE ACFT SUBSEQUENTLY DIVERTED AND LANDED SAFELY. UPON MX INSPECTION, INITIAL C/A OF SUSPECTED OVER VOLTAGE RELAY SHORTING AGAINST PANEL, ADJUSTED CLEARANCE FOR PANEL, AS WELL AS R & R LEFT AND RIGHT VOLTAGE REGULATORS. BOTH OF THESE C/A RESULTED IN SATISFACTORY GROUND RUNS, HOWEVER IN FLIGHT THE PROBLEM RECURRED AFTER SEVERAL MINUTES OF FLIGHT. UPON FURTHER INSPECTION OF ACFT INVOLVING REMOVAL OF FLOOR PANELS, WALL PANELS, ENGINE COWLINGS, AND STUB WING PANELS, A ALTERNATOR FIELD FUSE WAS FOUND LOOSE IN ITS HSG. AFTER CORRECTION, ACFT OPERATIONALLY CHECKED GOOD AND WAS RELEASED FOR A SATISFACTORY FUNCTIONAL CHECK FLIGHT.
WHILE IN CRUISE FLIGHT, THE PILOT REPORTED THE OVERHEAD LIGHT ASSY ON THE F/O SIDE SHORTING OUT. THE PILOT THEN DEACTIVATED THE SYSTEM BY DEACTIVATING THE CIRCUIT BREAKER. UPON ARRIVAL THE ACFT WAS INSPECTED BY MX WHERE THE FIRST OFFICER SIDE OVERHEAD LIGHT RESISTOR AND TRANSISTOR WERE REPLACED.
AT SCHEDULED INSPECTION, NLG DRAG BRACE OEM PN 5142002-5 WAS FOUND TO BE CRACKED IN THE AREA OF NLG ACTUATOR ATTACH POINT. METHOD OF INSPECTION WAS DYE PENETRANT. THIS CRACK WAS FOUND TO BE WITHIN ALLOWABLE LIMIT. THIS UNIT HAS BEEN REPAIRED IAW PROCEDURE OUTLINED IN MEB941-11R1 & RETURNED TO SERVICE.
ELECTRICAL ODOR AND SMOKE IN CABIN DURING APPROACH. SUBSEQUENT INSPECTION FOUND LOOSE SCREW SECURING WIRE TO CIRCUIT BREAKER TERMINAL AND EVIDENCE OF ARCING. (K)
