THE PILOT REPORTED A FUEL SMELL WHEN THE HEATER WAS ON IN FLIGHT. MAINTENACE INSPECTED AND FOUND THE FUEL LINE AT THE FUEL NOZZLE CRACKED. THE LINE WAS REPAIRED AND REINSTALLED WITH NO FURTHER DEFECTS NOTED.
THE PILOT REPORTED A BURNING SMELL IN THE CABIN IN FLIGHT WITH THE HEATER TURNED ON. MAINTENANCE INSPECTED AND REPLACED THE HEATER ASSEMBLY WITH NO FURTHER DEFECTS NOTED.
HSI HAD FAILED DURING CRUISE FLIGHT ON AN IFR FLIGHT PLAN. SMOKE EMITTED FROM THE INSTRUMENT WITH THE BREAKER TRIPPING. SMOKE WAS BOTH VISIBLE & WAS ABLE TO BE SMELLED. QRH WAS IN HAND IN CASE SMOKE WAS NOT SUBSIDING & AN EMER LANDING AIRPORT WAS FOUND BUT NOT NEEDED. ATC WAS INFORMED OF SITUATION NO EMER WAS DECLARED DUE TO SMOKE SUBSIDING. R & R HSI.
DURING A TRAINING FLIGHT THE PILOT REPORTED A FUEL SMELL IN THE COCKPIT WHEN THE HEATER WAS ON. HE SHUT OFF THE HEATER & REPORTED THE DISCREPANCY TO MX. SECURED THE B NUT AT THE HEATER FUEL NOZZLE. OPERATIONAL & LEAK CHECKS WERE PERFORMED WITH NO DISCREPANCIES.
FLIGHT 5581, IND TO IAH. DURING CRUISE THE FLIGHT CREW RECEIVED A HYD 2 SYS LOW PRESS MESSAGE. THE HYDRAULIC PAGE WAS BROUGHT UP IT WAS CONFIRMED THAT THE SYSTEM WAS INDICATING 0% QUANTITY AND VERY LOW PRESSURE. THE QRH PROCEDURES WERE FOLLOWED, AND THE FLIGHT DIVERTED TO KBNA WHERE IT LANDED WITHOUT INCIDENT. MAINTENANCE INSPECTED THE HYDRAULIC SYSTEM AND FOUND A PIN-HOLE IN THE SYSTEM 2 HYDRAULIC LINE GOING THROUGH THE AFT EQUIPMENT BAY. THE HYDRAULIC LINE WAS REPLACED ALONG WITH THE HYDRAULIC PUMP AND THE SYSTEM LEAK AND OPS CHECKED GOOD. (A)
FLIGHT 5756, IAH TO SDF - DURING CRUISE, THE CREW RECEIVED AN APU HIGH OIL TEMPERATURE MESSAGE ON EICAS. THE FLIGHT DIVERTED TO LIT AND LANDED WITHOUT INCIDENT. MAINTENANCE INSPECTED THE APU AND VERIFIED THE OIL WAS SERVICED TO THE CORRECT LEVEL. THE CONNECTOR TO THE HIGH OIL TEMPERATURE BULB WAS CLEANED AND RESECURED. THE APU WAS RUN WITHOUT FURTHER DEFECTS. ON THE 31ST,THE OIL COOLER WAS REPLACED DUE TO INTERNAL CLOGGING AND LEAKAGE.
FLIGHT 5601, IAH TO JAX - DURING CRUISE THE CREW RECEIVED AN APU OIL TEMP MESSAGE ON EICAS. THE PROPER QRH CHECKLISTS WERE FOLLOWED, AND THE FLIGHT DIVERTED TO PNS, WHERE IT LANDED WITHOUT INCIDENT. MAINTENANCE INSPECTED THE APU AND FOUND THAT IT WAS LOW ON OIL. AFTER SERVICING THE OIL TO THE CORRECT LEVEL THE APU THEN OPERATED NORMALLY AND THE AIRCRAFT WAS RELEASED FOR SERVICE. ON THE 31ST OF MARCH, THE APU WAS TROUBLESHOT DUE TO THE HIGH OIL TEMPERATURE PROBLEMS AND THE APU OIL COOLER WAS FOUND TO BE CLOGGED AND LEAKING OIL. THE OIL COOLER WAS REPLACED AND THE APU OPERATED NORMALLY.
IAH TO ATL - AFTER LEVELING OFF IN CRUISE FLIGHT, THE CREW RECEIVED AN APU HIGH TEMP MESSAGE ON EICAS. AFTER FOLLOWING THE APPROPRIATE QRH CHECKLISTS THE FLIGHT DIVERTED TO MGM WHERE IT LANDED WITHOUT INCIDENT. MAINTENANCE INSPECTED THE APU AND FOUND IT WAS LOW ON OIL. AFTER PROPERLY SERVICING THE APU WITH OIL, IT OPERATED NORMALLY AND THE AIRCRAFT WAS RELEASED TO SERVICE.
FLIGHT 5378- DURING CLIMB-OUT OF OMA RECEIVED A MOMENTARY MESSAGE INDICATING SMOKE IN LAVATORY, FLIGHT RETURNED TO OMA. REMOVED, CLEANED, INSPECTED AND REINSTALLED LEFT AND RIGHT COALESCER FILTER ELEMENTS.
FLIGHT 5384 - DURING LGA APPROACH RECEIVED A FLAP FAIL CAUTION MESSAGE AT ZERO DEGREES, CREW DECLARED AN EMERGENCY AND FLIGHT LANDED WITHOUT INCIDENT. COMPLIED WITH AD 98-20-01 FLAP CAUTION MESSAGE -FIRST EVENT- MAINTENANCE ACTION, PARAGRAPH (B) 1, 2, 3 (A, B, C AND E). FLAP SYSTEM WAS RESET WITH NO FURTHER FLAP CAUTION MESSAGES.
