FLIGHT 5652, IAH TO ATL - IN CRUISE FLIGHT THE FLIGHT CREW RECEIVED A FLAPS FAIL EICAS MESSAGE. THE FLIGHT CONTROL SYNOPTIC PAGE INDICATED THE FLAPS WERE OUTLINED IN YELLOW SHOWING ZERO DEGREES OF EXTENSION. THE AIRCRAFT LANDED BACK AT IAH WITHOUT INCIDENT. MAINTENANCE PULLED FECU CODES FOR THE RIGHT BRAKE POSITION SENSING UNIT AND THE FECU. THE RIGHT BRAKE POSITION SENSING UNIT WAS REPLACED AND THE FLAP SYSTEM OPERATED NORMALLY.
FLIGHT 5637, IAH TO ELP - DURING FINAL SEGMENT OF CLIMB THE FLIGHT CREW RECEIVED A FLAP FAIL MESSAGE ON EICAS. THE FLAPS WERE AT ZERO AND WERE INDICATING ZERO AT THE TIME. THE QRH WAS FOLLOWED AND THE AIRCRAFT LANDED WITHOUT INCIDENT. MAINTENANCE CHECKED THE FECU FOR FAULT CODES AND FOUND CODES FOR RT BPSU, FECU, AND WIRING FOR THE RT BPSU. THE SYSTEM WAS RESET AND THE FLAPS OPERATED NORMALLY.
IAH TO SRQ - DURING CLIMB, THE CREW RECEIVED A WARNING FOR E1 OIL PRESSURE. E1 OIL PRESSURE WAS NOTED TO FLUCTUATE FROM BELOW 25PSI TO 40PSI. CREW FOLLOWED THE QRH AND REDUCED THRUST ON E1, AFTER WHICH IT WAS NOTED THAT THE OIL PRESSURE HAD STABILIZED AT THE LOWER POWER SETTING. THE FLIGHT RETURNED TO FIELD AT IAH WITHOUT INCIDENT. MAINTENANCE INSPECTED E1 AND FOUND THE SEALS FOR THE ENGINE DRIVEN HYDRAULIC PUMP, INTEGRATED DRIVE GENERATOR, AND THE STARTER WERE LEAKING. THE AIRCRAFT WAS THEN FERRIED TO SDF WHERE THE EDP, IDG, AND STARTER SEALS WERE REPLACED. AFTER REPLACING THE SEALS AND SERVICING WITH OIL, THERE WERE NO FURTHER DEFECTS NOTED.
FLIGHT 5261 - AFTER TAKEOFF FROM RIC THE STAB AND MACH TRIM DROPPED OFF, FLIGHT RETURNED TO THE FIELD. CLEANED THE STAB TRIM SWITCHES ON YOKES AND PERFORMED AN OPERATIONAL AND FUNCTIONAL TEST OF STAB TRIM CHANNEL 1 AND 2. DISCREPANCY COULD NOT BE DUPLICATED BY MAINTENANCE.
FLIGHT 5786 - DURING TAKEOFF ROLL AT HPN RECEIVED AN APU OVER TEMPERATURE WARNING FOLLOWED AUTO-SHUTDOWN OF APU, FLIGHT WAS ABORTED AND RETURNED TO THE GATE. DEFERRED APU PER MEL 49-10-01-01.
FLIGHT 5352 - FLAPS FAIL CAUTION. REPLACED THE RIGHT FLAP SKEW DETECTOR.
FLIGHT 5462 - STRONG OILY ODOR IN COCKPIT WHEN BLEED AIR SWITCHED FROM APU TO ENGINES. REPLACED COALESOR SOCKS.
FLIGHT 5328 RECEIVED A FLAP FAIL CAUTON MESSAGE DURING PREFLIGHT WHEN SELECTING 20 DEGREE POSITION. COMPLIED WITH AD 98-20-01 FLAP CAUTION MESSAGE -FIRST EVENT- MAINTENANCE ACTION, PARAGRAPH (B) 1, 2, 3 (A, B, C AND E). FLAP SYSTEM WAS RESET WITH NO FURTHER FLAP CAUTION MESSAGES.
FLIGHT 5393 CARGO SMOKE MESSAGE. BOTTLES DISCHARGED. COULD NOT DUPLICATE. NO EVIDENCE OF SMOKE OR FIRE, REPLACED FIRE BOTTLES AND SMOKE DETECTORS.
FLIGHT 5917 CABIN ALTITUDE WARNING AND DISPLAY COOL CAUTION. COULD NOT DUPLICATE. INSPECTED AIRCRAFT, FOUND NO DEFECTS. SUSPECT CARGO DOOR NOT CLOSED PROPERLY.
PAX DOOR PRESSURE PANEL CABLE (ICE BREAK) IS BROKEN AT THE END OF THE CLEVIS DUE TO RUBBING ON THE FLAP (P/N 601R318122-3). PAX DOOR PRESSURE TO PANEL CABLE WAS REPLACED WITH A SERVICEABLE CABLE (P/N 601R3188127_3). THE FLAP DECOMPRESSION PANEL NEEDS TO HAVE A RECESSION CUT INTO THE INTERIOR LIP TO ALLOW THE CABLE MORE ROOM AND TO PREVENT IT FROM BEING BROKEN FROM NORMAL USE.
CARGO BAY, LEFT AND RIGHT, FLOOR ANGLE SUPPORT CRACKED AT FS 621. REMOVED DAMAGED FLOOR ANGLE ON LEFT HAND AND RIGHT HAND SIDE OF 621 FLOOR ANGLE IN ACCORDANCE WITH BOMBARDIER REO 601R-53-61-004. REPAIRED 621 FLOOR ANGLE, LEFT HAND AND RIGHT HAND SIDE, IN ACCORDANCE WITH BOMBARDIER REO 601R-53-61-004.
