Maintenance disclosures
THE FLIGHT CREW REPORTED THE VAPOR CYCLE AIR CONDITIONING INOPERATIVE. THE MAINTENANCE PROVIDER DETERMINED THAT THE SYSTEM CIRCUIT BREAKER WAS DEFECTIVE. THE MAINTENANCE PROVIDER REMOVED AND REPLACED THE CIRCUIT BREAKER. OPERATIONAL CHECKS WERE SATISFACTORY AND THE AIRCRAFT WAS RETURNED TO SERVICE.
THE FLIGHT CREW REPORTED THE PARKING BRAKE WAS INOP. (AIRCRAFT BRAKING OK) THE MAINTENANCE TEAM DISCOVERED THAT THE PARK BRAKE CABLE WAS BROKEN AT THE PULL HANDLE. THE CAUSE OF THE DAMAGE IS UNKNOWN. IT IS SUSPECTED THAT USE OVER TIME MAY HAVE CAUSED THE CABLE TO SEPARATE FORM THE HANDLE. THE PARKING BRAKE CABLE WAS REMOVED AND REPLACED WITH A NEW CABLE. OPS CHECKS WERE SATISFACTORY. THE AIRCRAFT WAS RETURNED TO SERVICE.
THE FLIGHT CREW REPORTED THAT THE EMERGENCY BATTERY WOULD NOT DISCONNECT WHEN SELECTED OFF. THE MAINTENANCE VENDOR TROUBLESHOT AND SUSPECTED THE RIGHT GENERATOR MASTER SWITCH P/N MS24524-23 TO BE DEFECTIVE. THE DEFECTIVE MASTER SWITCH WAS REMOVED AND INSTALLED A NEW SWITCH.
IT WAS REPORTED FROM AIR TRAFFIC CONTROL (ATC) THAT THE AIRCRAFTS NR 1 TRANSPONDER SYSTEM WAS NOT TRANSMITTING MODE C ON MULTIPLE LEGS. THE MAINTENANCE VENDOR FUNCTIONAL CHECKED THE SYSTEM AND WAS NOT ABLE TO VERIFY THE DISCREPANCY ON THE GROUND. ALL TESTING PASSED. THE AIRCRAFT WAS RETURNED TO SERVICE.
DURING TAXI AFTER LANDING, THE RT MAIN LANDING GEAR OUTBOARD WHEEL HALF SEPARATED FROM THE INNER WHEEL HALF, THE RT MAIN LANDING GEAR TIRE SEPARATED FROM THE INBOARD WHEEL HALF AND AIRCRAFT. THE CAUSE OF THE WHEEL TO DISASSEMBLE IS THOUGHT TO BE THE RESULT OF WHEEL BOLTS INSTALLED ON THE WHEEL FAILING.
WHILE PERFORMING AN A-D INSPECTION FOUND 8 LOOSE RIVETS IN THE FORWARD RIVET LINE OF THE ATTACHMENT PIVOT FITTING PN 45A21183-11 ON THE BOTTOM OF THE HORIZONTAL STABILIZER. IT IS SUSPECTED THAT THE LOOSE RIVETS WERE CAUSED BY FATIGUE AND AGE. RECOMMEND INSPECTING AREA THOROUGHLY AT EACH INSPECTION. RECOMMEND CONTACTING MFG FOR REPAIR OPTIONS. THIS IS THE 3RD AIRCRAFT IN A ROW THAT THESE SAME RIVETS HAVE BEEN LOOSE OR MISSING. (K)
WHILE PERFORMING A SCHEDULED A-D INSPECTION FOUND NR 2 (COPILOT) SIDE WINDOW CRACKED USING THE PRISM INSPECTION METHOD. THE D-INSPECTION REQUIRES THAT BOTH WINDSHIELD RETAINERS OR USING OPTICAL PRISM INSPECTION METHOD. THE IS NO STATEMENT IN THE AIRCRAFT MM OR IN THE A-D INSPECTION CHECKLIST THAT REQUIRES YOU TO INSPECT THE COCKPIT SIDE WINDOWS IN THE SAME MANNER AS THE WINDSHIELDS. ALL OF THE WINDOWS IN THE COCKPIT AREA ARE INSTALLED OR FASTENED IN THE SAME MANNER AS THE WINDSHIELDS. IT IS SUSPECTED THAT THE COPILOT WINDOW CRACKED FROM FATIGUE, OR TT INSTALLED. WOULD RECOMMEND THAT INSPECTIONS OF ALL COCKPIT WINDOWS WITH A PRISM AT SCHEDULED INTERVALS BE COMPLETED. (K)
