DURING CRUISE MODE OF FLIGHT, PILOT NOTICED ENGINE OIL PRESSURE INDICATED "0". ALL OTHER INDICATIONS WERE NORMAL. PILOTED ELECTED TO RETURN TO BASE.
DURING CRUISE FLIGHT ENGINE CHIP LIGHT ILLUMINATED. LANDED ON AN OFFSHORE PLATFORM, EXAMINED CHIP PLUG FOUND METAL PARTICLES. UPON RECOVERY OF AIRCRAFT & SUBSEQUENT TEAR DOWN OF ENGINE, THE NATURE OF THE PROBLEM WAS DETERMINED TO BE THE NUMBER FIVE BEARING IN THE TURBINE SECTION.
SPALLING ON GEAR TEETH, THIS IS A RECURRING EVENT ON C-47 GEARBOXES. REPLACED GEAR W/NEW PART.

TURBINE RECEIVED IN SHOP TO PERFORM 2000 HOUR OVERHAUL. UPON REMOVAL OF OUTER COMBUSTION CASE AND COMBUSTION LINER, FOUND A HOLE IN DOME OF 1ST STAGE SHIELD. ALSO, FOUND THE BROKEN PIECE LODGED ON 1ST STAGE NOZZLE VANE. POSSIBLE IMPROPER METAL USED TO MAKE DOME. PART WAS SCRAPPED, UNABLE TO REPAIR. (X)
UPON DAILY INSPECTION, CRACKS IN CORNERS CAUSED BY INHERENT VIBRATIONS. (X)
FOUND NR 4 DISC PACK SPREAD JUST BEYOND LIMITS DURING 25-HOUR INSPECTION. FOUND NR 4 DISC PACK SPREAD BEYOND LIMITS. REMOVED NR 3 TAIL ROTOR DRIVESHAFT TO BLOWER SPLINE ADAPTER TO ENSURE IT STILL CONTAINED PROPER AMOUNT OF LUBRICANT. IT DID, BUT FOUND THE SPLINE ADAPTER P/N 406-040-316-101 HAD A STEP WORN IN IT. REPLACED THE SPLINE ADAPTER.
DURING 25-HOUR, DISC PACK AD 97-24-17 PARA 'A', FOUND DISC PACK DAMAGED.
MAIN ROTOR HUB REMOVED FOR BEARING EXTRUDING. BEARING COMPLETELY DEBONDED AT 2ND CONE FROM FLANGE.
DURING 25-HOUR INSPECTION, FOUND GAP BETWEEN THE NR 1 DISC (MOST FORWARD) AND NR 2 DISC BEYOND .015 INCH. (THE NR 1 DISC IS THE ONE WITH THE PART NUMBERS SHOWING).
DURING 25 HOUR INSPECTION, GAP BETWEEN DISCS NR 1 AND NR 2, NR 2 AND NR 3, OF THE 10-DISC PACK EXCEED LIMITS.
DURING 25 HOUR INSPECTION, FOUND GAP BETWEEN DISC NR 1 AND NR 2, NR 9 AND NR 10, EXCEED LIMITS. (TOTAL NUMBER OF DISCS IN THE DISC PACK IS 10).
DURING 25 HOURS INSPECTION, FOUND GAP BETWEEN DISCS 9 AND 10 OF THE 10-DISC PACK EXCEED LIMITS.
WHILE PERFORMING A 25 HOUR INSPECTION, DISC PACK NR 7 WAS NOTED TO BE CRACKED. THE CRACKED DISC OF THE 10 DISCS IS THE NR 10 DISC. (THE VERY AFT DISC OF THE NR 7 DISC PACK).
