CREW REPORTED AFTER DEPARTURE FROM MDW AT APPROXIMATELY 3000' THE RT ENG OIL LT CAME ON AND OIL PRESS READ ZERO. PERFORMED RH ENG SHUTDOWN AND RETURNED TO MDW. BEFORE LANDING, THE CREW REQUESTED EMERGENCY EQUIPMENT TO BE STANDING BY. AIRCRAFT LANDED WITHOUT FURTHER EVENT. MAINTENANCE FOUND LUBRICATION LINE B NUT NEAR PROP GOVERNOR LOOSE ON RIGHT HAND ENGINE. TIGHTENED B NUT. SERVICED ENGINE OIL. PERFORMED 4 ENGINE RUNS TO HIGH POWER TO LEAK CHECK. NO LEAKS NOTED. OPS CHECK GOOD. AIRCRAFT WAS RETURNED TO SERVICE.
ENG WOULD NOT START. ROTATION OF PROPELLER BY HAND PRODUCED A GRINDING SOUND. ENG GEARBOX WAS ACCESSED. THE MAIN SHAFT FUEL DRIVE SPUR GEAR WAS FOUND CRACKED IN HALF & SEPARATED. SEPARATED SPUR GEAR CAUSED LOSS OF FUEL DRIVE, CAUSING ENG TO NOT START. THE GEARBOX WAS RE-ASSEMBLED USING A SERVICEABLE SPUR GEAR.
THE ACFT WAS ON APPROACH, TOWER REPORTED SMOKE COMING FROM THE LT ENGINE. THE ACFT LANDED WITHOUT INCIDENT. UPON INSP OF LT ENGINE, FOUND OIL LEAKING FROM REAR TURBINE BEARING HOUSING. SUSPECT REAR TURBINE BEARING SCAVENGE PUMP FAILURE AND SUBSEQUENT SEAL FAILURE CAUSING LOSS OF OIL INTO EXHAUST STREAM. THERE WAS NO EVIDENCE OF OIL LOSS FORWARD OF THE REAR TURBINE BRG. TEARDOWN REPORT CONFIRMED THAT THE REAR TURBINE BRG FAILED. SCHEDULED ENGINE CHANGE. RETURNING UNSERVICEABLE ENGINE TO O/H FACILITY FOR TEARDOWN AND EVALUATION. TEARDOWN REPORT RECEIVED 2/28/07. PN BEARING HAS A LIFE OF 3500 HRS. DUE REPLACEMENT IAW SB TPE331-72--180R31. BRG DUE REPLACEMENT WITH BRG PN 3108098-1, SB TPE331-72-0189R31.
FLT 206 - FLIGHT CREW NOTICED A SMELL OF SMOKE IN THE COCKPIT AND RETURNED TO THE GATE. A FERRY PERMIT WAS ISSUED AND THE AIRCRAFT RETURNED TO IAD. MAINTENANCE FOUND A WIRE WITH SIGNS OF HEAT DAMAGE. THIS WIRE RUNS FROM THE LEFT ENGINE START GENERATOR TO THE LEFT ENGINE START CONTACTOR. A DETAILED INSPECTION FOUND A BUTT SPLICE INSTALLED ON WIRE KN130 WHEN THIS SPLICE WAS INSTALLED THE WIRE SHIELD WAS DAMAGED . (M)
FLT 7659 - PHL-IAD - DURING TAKEOFF, AIRCRAFT INTERCOM INTERMITTENT AFTER WATER SPILLED FROM DV WINDOW ONTO CAPTAINS LEFT KNEE AND COMM BOX DURING START OF TAKEOFF ROLL. AIRCRAFT RETURNED TO GATE, THEN WAS FERRIED TO IAD. MAINTENANCE REMOVED AND REPLACED STATION BOX, OPS CHECK GOOD. AIRCRAFT WAS RELEASED AND RETURNED TO SERVICE. (M)
FLT 6244 - IAD-PHF - DURING TAKEOFF, THE FLT CREW EXPERIENCED A HYDRAULIC PUMP FAILURE. CREW ABORTED TAKEOFF AND NOTIFIED DISPATCH AND MAINTENANCE CONTROL AND RETURNED TO IAD. MX CONTROL NOTIFIED LINE MX OPERATIONS AND MECHANICS WERE DISPATCHED TO THE AIRCRAFT. MX FOUND THE RIGHT HYDRAULIC PUMP FAILED. MX REMOVED AND REPLACED THE HYDRAULIC PUMP IAW APPLICABLE MM REFERENCE AND ACCOMPLISHED REQUIRED CHECKS. AIRCRAFT WAS THEN RETURNED TO SERVICE. (M)
FLT 6257 - ALB-IAD - DURING APPROACH, EXPERIENCED A DOOR CAPTION WARNING LIGHT ILLUMINATION. CREW DECLARED AN EMERGENCY AND LANDED AT IAD WITHOUT FURTHER INCIDENT. AIRCRAFT TAXIED TO GATE AND MAINTENANCE CONTROL WAS NOTIFIED AND MAINTENANCE WAS DISPATCHED. UPON ARRIVAL MAINTENANCE FOUND THAT THE LOWER (FLOOR) MICROSWITCH ARM HAD BROKEN. MAINTENANCE REMOVED AND REPLACED THE ARM. ALL SYSTEMS WERE CHECKED WITHOUT FURTHER PROBLEM. AIRCRAFT WAS THEN RELEASED FOR SERVICE. (M)
FLT 6248 - IAD-BGM - DURING TAXI, AIRCRAFT RETURNED TO GATE DUE TO CAPTAINS VSI SCREEN WENT BLANK. AIRCRAFT RETURNED TO GATE AND WITH NO FURTHER PROBLEMS. MAINTENANCE INSPECTED AND REMOVED AND REPLACED VIS INDICATOR, OPS CHECK OK. AIRCRAFT WAS RELEASED AND RETURNED TO SERVICE. (M)
