DURING A RECENT 404 TITAN GEAR INSPECTION ON THE MAIN LANDING GEAR, FOUND TRAILING LINK PIVOT PINS WORN EXCESSIVELY. THE PHENOLIC-LINED BUSHINGS WERE WITHIN MANUFACTUER'S SPECS. NEW PINS WERE INSTALLED.
DURING A RECENT 404 TITAN GEAR INSPECTION ON THE MAIN LANDING GEAR, FOUND TRAILING LINK PIVOT PINS WORN EXCESSIVELY. THE PHENOLIC-LINED BUSHINGS WERE SITHIN MANUFACTUER'S SPECS. NEW PINS WERE INSTALLED.
DURING A RECENT MDL 404 GEAR INSPECTION, NOTED THE NOSE GEAR ACTUATOR ATTACH STRUCTURE HAD NUMEROUS LOOSE HI-LOKS.
PILOT REPORTED ENGINE STUMBLE ON TAKEOFF AND AT CLIMB POWER SETTING. EXTENSIVE TROUBLESHOOTING FOUND LOOSE BUSHING AT RIGHT ENGINE. MIXTURE CONTROL SHAFT INSTALLED USED SERVICEABLE FUEL CONTROL ASSY. TEST FLOWN OK.
FOUND NR 2 CYLINDER HEAD CRACKED AT LOWER SPARK PLUG HOLE.
RECEIVED OVERHAULED EXHAUST RISER FROM OVERHAULER. DURING INSPECTION PRIOR TO INSTALLATION, FOUND MOUTH FOR SLIP JOINT MANUFACTURED TOO SMALL AND NOT ALLOWING SLIP JOINT TUBE TO SLIDE IN. SUSPECT CAUSE IS POOR MANUFACTURING AND QUALITY CONTROL PRACTICES. SUBMITTER RECOMMENDS COMPANY CHECK THEIR WORK BEFORE IT IS TAGGED OVERHAULED AND SHIPPED.
AFTER FLIGHT THROUGH RAIN, WATER ENTERS LIFT DETECTOR AND CAUSES STALL WARNING HORN TO SOUND CONTINUOUSLY. UNIT IS NOT EASY TO DRY OUT REQUIRING REPLACEMENT. SUBMITTER STATED THIS IS A RECURRING PROBLEM. RECOMMEND IMPROVED INTERNAL SEALING OF LIFT DETECTOR. MANUFACTURED BY 'SAFE-FLIGHT'. PROBLEM OCCURS WITH SEVERAL PART NUMBERED UNITS.
LEFT ENGINE STARTER BURNED UP DUE TO RUNNING CONTINUOUSLY DUE TO RELAY STICKING. POOR DIAGNOSIS CAUSED STARTER TO BE REPLACED WITHOUT REPLACING SOLENOID. SECOND STARTER FAILED ALSO. SAME CONDITION. RELAY REPLACED WITH SECOND NEW STARTER. SUBMITTER SUGGESTS MANUFACTURERS INSTALL WARNING LIGHT IN STARTER POWER CIRCUIT TO ALERT PILOT WHEN UNIT IS ENERGIZED. PILOT CANNOT TELL IF POWER IS REACHING STARTER MOTOR, OTHERWISE.
LEFT MAIN GEAR 'UP' HYDRAULIC LINE HOSE SEPARATED FROM END FITTING. HYDRAULIC FLUID DUMPED OVERBOARD. PILOT BLEW GEAR 'DOWN' WITH NITROGEN BOTTLE. HOSE HAD BEEN FABRICATED FROM NEW PARTS 65 HOURS PREVIOUSLY.
OIL COOLER CRACKED UNDER NORMAL CONDITIONS, LEAKING OIL. SUBMITTER SUGGESTS RE-DESIGN COOLER.
