Maintenance disclosures
DURING ROUTINE MAINTENANCE, THE LOWER SKIN OF THE FUSELAGE NEAR TAILBOOM ATTACHMENT WAS FOUND CRACKED APPROXIMATELY FOUR INCHES LONG FORWARD AND AFT AND AN INCH-AND-HALF RUNNING OUTBOARD. FS 75, WL 61.77, RBL 2.0.
DURING INSPECTION AT OUR SWRMO, FOUND BAGGAGE COMPARTMENT OVERHEAD STIFFENER CRACKED/MISSING NUT PLATES. REMOVED DAMAGED STIFFENER AND REPLACED WITH NEW STIFFENER PN 206-032-309-095 AND DAMAGED SUPPORT WITH NEW SUPPORT PN 206-032-309-113 . ALL MAINTENANCE ACCOMPLISHED WITH REFERENCE TO BHT ALL SRM CH 3
SKIN CRACKED LH AIRFRAME JUST BELOW BAGGAGE DOOR. REPAIRED DAMAGE USING SPECIFICATIONS AS OUTLINED IN BHT ALL SRM CH 4 PARA 4-4-9 APPLICATION B.
DURING INSPECTION AT OUR SWRMO. CORROSION METAL DEBONDING FOUND ON CO-PILOT FLOOR, UNDER INVERTOR.
SEVERE CORROSION WAS FOUND IN SHELF LOCATED INTERNALLY TO THE 145-155 ROLLOVER BULKHEAD ON THE LT SIDE OF THE AIRCRAFT. THE SHEETMETAL SHELF THAT SITS INSIDE OF THE ACCESS PANEL AND ACTS AS THE BOTTOM OF THE CAVITY WAS FOUND TO BE SEVERELY CORRODED. THIS AREA CONTAINED INSULATION. THE DAMAGE WAS CONTAINED TO THE SHELF AND THE FORWARD FACE OF TEH FS155 BULKHEAD.
UPON TAKING OFF THE ACFT EXPERIENCED AN UNCOMMANDED YAW. THE PILOT IMMEDIATELY LANDED & SHUTDOWN THE ACFT. UPON MX INSPECTION IT WAS FOUND THAT THE TAIL ROTOR SEGMENTED DRIVESHAFT LOCATED ABOVE THE HORIZONTAL STABILIZER ON THE TAIL BOOM HAD SHEARED CAUSING A LOSS OF POWER TO THE TAIL ROTOR & SUBSEQUENT UNCOMMANDED YAW OF THE ACFT.
AFT AIR CONDITIONER BLOWER INOPERATIVE.
ON DAILY INSPECTION, MECHANIC FOUND RIVET MISSING FROM THE TAIL ROTOR BLADE TIP.
