SMOKE IN COCKPIT. AFT A/C BLOWER MOTOR INOP.
WE FOUND THE RETURN LINE INSIDE THE MAIN FUEL CELL HAND TIGHT WHICH CAUSE A FUEL LEAK AND THE SMELL OF FUEL IN THE CABIN.
FUEL SMELL NOTED IN FLIGHT
CREW NOTED A FUEL SMELL IN FLIGHT AND RETURNED TO BASE
DURING PERIODIC INSPECTION, THE FREEWHEELING UNIT HOUSING WAS VISUALLY DISCOVERED TO BE CRACKED ON THE MATING SURFACE TO THE ENGINE. THE CRACK EXTENDED APPROXIMATELY 0.5 INCHES FROM THE ATTACHMENT HOLE.
UPON DEPARTURE FROM HOSPITAL HELIPAD AT APPROXIMATELY 100 FT AGL PILOT REPORTED EXPERIENCING A COMPLETED FADEC FAILURE. FADEC FAIL AND FADEC MAN CAUTION LIGHTS ILLUMINATED AND FADEC WARNING HORN SOUNDED. N2 AND NR IMMEDIATELY DECREASED. UPON INSPECTION THE FADEC UNIT COULD NOT BE ACCESSED TO DOWNLOAD THE HISTORY REPORTS AND WAS COMPLETELY UNRESPONSIVE.
WHEN PERFORMING OIL CHANGE OF TAIL ROTOR GEARBOX, FOUND OIL REMAINING IN GEARBOX TO ONLY MEASURE APPROX 1 OZ. FLUID LOSS APPEARED TO ORIGINATE FROM TAIL ROTOR PITCH CHANGE CONTROL ROD SEAL.
ENG COMPRESSOR SCROLL CRACKED.
IN FLIGHT, HYD CAUTION LIGHT ILLUMINATED FOLLOWED WITH IN 10-15 SECONDS A COMPLETE LOSS OF HYD PRESSURE. PILOT PERFORMED EMERGENCY PROCEDURE UNABLE TO RESTORE HYD PRESSURE. LANDED AS SOON AS PRATICAL. MX REMOVED THE HYD PUMP BUT FOUND THE TRANSMISSION OIL PUMP SPLINES HAD FAILED. DUE TO THE FACT THE TRANSMISSION OIL PUMP HAD FAILED, IS A DIRECT RESULT OF WHY THE HYD PUMP WAS UNABLE TO PRODUCE PRESSURE SINCE THE HYD PUMP IS DRIVEN BY THE TRANSMISSION OIL PUMP.
