Part: HOSE — LEAKING (LAV OXYGEN)
DURING SCHEDULED PHASE INSPECTION NOTED HOSE ASSY. SUPPLYING OXYGEN TO LAVATORY OXYGEN MASK COMPARTMENT FROM MAIN UPPER CABIN OXYGEN LINE ASSY LEAKING. FURTHER INVESTIGATION SHOWED MULTIPLE HOLES IN OXYGEN HOSE ASSY FLEXIBLE HOSE SECTION. SUSPECT HOSE ASSY DETERIORATED FROM AGE AND POSSIBLY HEAT FROM AIRCRAFT BEING PARKED OUTDOORS FOR EXTENDED PERIODS. RECOMMEND TECH CLOSELY INSPECT CABIN OXYGEN SYSTEM COMPONENTS DURING SCHEDULED INSPECTIONS ON OLDER AND HIGH-TIME SERIES AC FOR AGE RELATED DEFECTS. (K)
Part: LATCH — BROKEN (CARGO DOOR)
DURING SCHEDULED PHASE INSPECTION, FOUND AFT BAGGAGE COMPARTMENT DOOR AFT LATCH ASSY, NOT LOCKING. FURTHER INVESTIGATION FOUND AFT DOOR LATCH ASSY, BROKEN. REPLACED LATCH ASSY W/ SERVICEABLE LATCH ASSY, OPERATIONS NORMAL. RECOMMEND FURTHER INVESTIGATION BY MFG TO DETERMINE IF MFGED LATCHES PRONE TO EARLY FATIGUE FAILURE AND ISSUE SB TO REPLACE IF HIGH FAILURE RATES NOTED. TECHS SHOULD INSPECT THESE LATCHES CLOSELY DURING SCHEDULED PHASE CHECKS TO PREVENT POSSIBLE DOOR OPENING IN FLIGHT. (K)
Part: TRANSDUCER — INTERMITTENT (ANTI SKID SYS) · airframe 12,900 hrs · 11,489 cycles
THE CREW REPORTED THAT THE AIRCRAFT BRAKING FAILED UPON EXITING THE RUNWAY AFTER LANDING. WHEN THE ANTI-SKID SYSTEM WAS TURNED OFF, NORMAL BRAKING POWER WAS RESTORED. MAINTENANCE REPLACED BOTH ANTI-SKID WIRING HARNESSES, AND DISCOVERED BOTH MAIN LANDING GEAR WHEEL SPEED TRANSDUCERS TO BE INTERMITTENT. BOTH WHEEL SPEED TRANSDUCERS WERE REPLACED. A GROUND OPERATIONAL CHECK OF THE ANTI-SKID SYSTEM WAS SATISFACTORY. THE AIRCRAFT WAS RETURNED TO SERVICE.

AFTER TAKE OFF THE CREW REPORTED AN ODOR OF SULFUR IN THE CABIN. AT 2,000 AGL THE EMERGENCY PRESSURIZATION SYSTEM ENGAGED, AND CREW DONNED THEIR OXYGEN MASKS. THE AIRCRAFT RETURNED TO KLEX AND AN EMERGENCY EVACUATION OF THE AIRCRAFT WAS PERFORMED ON THE TAXIWAY. MAINTENANCE ISSUED MEL 21-1 FOR THE AIR CYCLE MACHINE. THE AIRCRAFT IS TO BE OPERATED WITHOUT PRESSURIZATION UNDER THE MEL. ON 06 JUNE 2005, MAINTENANCE REPLACED THE ACM, HEAT EXCHANGER, AND COALESCER FILTER. THE AIRCRAFT WAS RETURNED TO SERVICE.

CREW REPORTED, UPON GEAR RETRACTION AFTER TAKEOFF RECEIVED A RED GEAR UNSAFE WARNING LIGHT. CREW DECLARED AN EMERGENCY DUE TO THE SUSPICION THAT A NOSE WHEEL TIRE HAD BLOWN. AIRCRAFT LANDED IN KORL WITHOUT FURTHER INCIDENT. AFTER LANDING IT WAS DISCOVERED A TIRE DID NOT BLOW OUT.

CREW REPORTED THAT AFTER APPROXIMATELY 15 MINUTES INTO FLIGHT THEY NOTICED A BURNING OIL SMELL COMING FROM THE OVERHEAD VENTS. ACTIONS WERE TAKEN TO ISOLATE THE SMELL BY TURNING OFF FANS AND ISOLATING BLEED AIR SOURCES. A/C DIVERTED TO KCRG WHERE A NORMAL DESCENT AND LANDING WERE ACCOMPLISHED. MAINTENANCE RAN THE AIRCRAFT AND CONFIRMED THE PROBLEM. IT WAS DISCOVERED THAT OIL WAS DRIPPING ONTO A HOT BLEED DUCT IN THE AFT EQUIPMENT BAY. A B-NUT WAS TIGHTENED THAT RAN FROM A FILTER TO THE FLOW SWITCH. AIRCRAFT WAS RAN FOR 30 MINUTES WITH NO EVIDENCE OF THE OIL SMELL. B-NUT WAS RECHECKED AND NO LEAKS WERE NOTED. AIRCRAFT WAS RETURNED TO SERVICE.

DURING LANDING AIRCRAFT PULLED TO THE LT. AFTER THE AIRCRAFT SLOWED THERE DIRECTIONAL CONTROL WAS LOST AND THE AIRCRAFT WENT OFF THE LT SIDE OF THE RUNWAY DUE TO THE LT TIRE BEING FLAT.
