LEFT CYAN THRUST REVERSER UNLOCKED CAPTION ILLUMINATED IN FLIGHT ON DEPARTURE ACCELERATING THROUGH 200 KNOTS AT 2000 FOOT ELEVATION AGL. MX C/A: APPLIED MEL 78-31-01, DEACTIVATED LH TR IAW GL5000 GVFD AMM TASK 78-30-00-040-802.
DURING CRUISE @ FL450, THE OUTER PLY OF THE LH WINDSHIELD SHATTERED. OUT OF CAUTION, CREW DONNED THEIR O2 MASKS AND IMMEDIATELY INITIATED A DESCENT BELOW 18,000FT. THE DECISION WAS MADE TO DIVERT TO GSO, WHERE THEY LANDED WITHOUT INCIDENT. MAINTENANCE REPLACED THE FAILED WINDSHIELD (P/N: GC331-0001-11 S/N: 2349) WITH NEW. OPERATIONAL AND LEAK TEST WERE NORMAL. THE AIRCRAFT WAS RETURNED TO SERVICE.
WHILE ENROUTE TO VNY, FLIGHT CREW RECEIVED A CYAN LEFT REVERSER UNLOCK FAULT CAS MESSAGE. CREW CONSULTED WITH MX CONTROL AND BOMBARDIER TECH SUPPORT VIA SAT PHONE, AND WAS ADVISED THAT ITâS LIKELY A MALFUNCTIONING MICROSWITCH, AND THAT THERE ARE NO OTHER INDICATIONS OF AN ACTUAL REVERSER UNLOCK CONDITION. OUT OF PRECAUTION, CREW DECLARED AN EMERGENCY AND LANDED SAFELY IN VNY WITHOUT ANY FURTHER ISSUES. TECHNICIANS CONFIRMED THE LT T/R UNLOCK FADEC FAULT HISTORY CODE, AND REPLACED ALL 4 LT T/R DOOR RETRACT SWITCHES (P/N: T7013-3) WITH NEW, ALONG WITH INCORPORATING SB BR700-78-101983 R1 (T/R DOOR STOW SWITCH-INTRODUCTION OF IMPROVED FUNCTIONALITY). THRUST REVERSER FUNCTIONAL CHECK WAS NORMAL. THE AIRCRAFT WAS RETURNED TO SERVICE.
