Maintenance disclosures
DURING CRUISE FLIGHT A WHISTLE NOISE STARTED COMING FROM THE MAIN CABIN DOOR AND THE CABIN STARTED TO DEPRESSURIZE, PILOTS LANDED AT THE NEAREST AIRPORT WITHOUT INCIDENT.
DURING TAKE OFF, THE AIRCRAFT WOULD NOT PRESSURIZE IN AUTO OR MANUAL MODE DURING CLIMB, AIRCRAFT RETURNED TO DEPARTURE. FOUND THE OUTFLOW VALVE DEFECTIVE.
DURING FLIGHT AT ALTITUDE THE RT THRUST REVERSER UNLOCK LAMP CAME ON WITH THE MASTER WARNING, THE PILOTS FOLLOWED THEIR CHECKLIST & WENT TO EMER STOW, THE UNLOCK LIGHT STAYED ON, THE PILOTS DIVERTED & LANDED WITHOUT INCIDENT. TROUBLESHOOTING WAS PERFORMED & FOUND TO BE THE THE RT THRUST REVERSER STOW SWITCH PN 53ND78304-1, THE SWITCH WAS REPLACED.
DURING CLIMB, AT APPROX 8000 FT THE TAIL CONE DOOR CAUTION LIGHT ILLUMINATED, THE DOOR WAS CLOSED AND LOCKED BEFORE FLIGHT, UPON LANDING THE DOOR WAS STILL CLOSED AND LOCKED. THE LIGHT SWITCH WAS ADJUSTED TO REPAIR THE INPROPER INDICATION.
CREW IDENTIFIED DURING TAKEOFF THAT THE THROTTLE POSITION WHEN MATCHED SHOWED THE LT ENGINE AT 88 PERCENT AND THE RT ENGINE AT 37 PERCENT. RT ENGINE TOOK APPX 4-5 SECONDS TO CATCH UP TO THE LT. MAINT TROUBLESHOT AND REPLACED THE RT ENGINE T1 SENSOR. OPS CHECKS SATISFACTORY.
CREW IDENTIFIED WINDSCREEN OVERHEAT ANNUNCIATOR LIGHT ILLUMINATED INTERMITTENTLY DURING FLIGHT. MAINT REPLACED WINDSHIELD ANTI-ICE VALVE WITH NEW STYLE VALVE. LEAK AND OPS CHECK WERE SATISFACTORY. AFTER CONTACTING THE OEM, THEY CONFIRMED AN IMPROVED VALVE WAS AVAILABLE. THIS IS THE PRODUCT WE INSTALLED.
CREW IDENTIFIED THE WINDSCREEN OVERHEAT ANNUNCIATOR CAME ON AT TAKEOFF POWER WITH VALVES CLOSED AND SWITCH OFF. THE CREW ABORTED TAKEOFF AND MAINT WAS CALLED IN. TROUBLESHOT ACFT WINDSHIELD HEAT SYS IAW MM, NO DEFECTS NOTED. ENGINE RUN AT FULL POWER, NO BLEED AIR DETECTED ACROSS WINDSCREEN. ALL CHECKS WERE NORMAL, COULD NOT DUPLICATE.
HYDRAULIC LINE, PN 6556000-20, ON LT ENGINE IS MAKING CONTACT WITH FUEL LINE PN 3042937-01. THIS CAUSED CHAFFING IN EXCESS OF 20 PERCENT OF THE WALL THICKNESS ON THE HYDRAULIC LINE AND 10 PERCENT TO 15 PERCENT WALL THICKNESS ON THE FUEL LINE. BOTH LINES WERE REPLACED WITH FACTORY NEW PARTS. AFTER PART REPLACEMENT CONTACT WAS STILL BEING MADE AND REQUIRED THAT SLIGHT ADJUSTMENTS TO THE BENDS ON THE LINES WERE REQUIRED TO MAINTAIN ADEQUATE CLEARANCE. (K)
FLAP CONTROL VALVE PN 2710970 TO BE MAKING CONTACT WITH DUMP VALVE PN OA55319-1 CAUSING WAS SUBSTIALLY DAMAGE. BY SHORTING THE STAND OFF ON THE BOTTOM SIDE OF THE DUMP VALVE BY APROX .1250 OF AN INCH A CLEARANCE OF APROX .0625 OF AN INCH WAS OBTAINED. (K)
