EXCESSIVE SMOKE COMING FROM ENGINE AT IDLE. FOUND HEAVY COKING ON THE OIL STRUT AND POOR DRAIN BACK FROM 6/7 BEARINGS. ENGINE WAS INSTALLED 53.9 HRS PRIOR TO EVENT. ENGINE REQUESTED TO BE REMOVED AND RETURNED TO KEYSTONE FOR EVALUATION.
GREEN T/R BLADE HAS CRACK AT WEEP HOLE
DURING ROUTINE MAINTENANCE, MR BLADE LEADING WAS FOUND DEBONDED ON UPPER SURFACE APPROXIMATELY 3 INCHES FROM TIP.
DURING ROUTINE MAINTENANCE, CORROSION AND PITTING WAS DISCOVERED ON THE FUEL NOZZLE SHROUD.
DURING ROUTINE MAINTENANCE, CORROSION WAS DISCOVERED ON THE AFT TAIL ROTOR DRIVE SHAFT.
WHILE PERFORMING PREFLIGHT CHECK, PILOT NOTED THAT THE AFT MOST FLEXIBLE DISC PACK COUPLING ON THE TAIL ROTOR SEGMENTED DRIVESHAFTS AT THE INTERFACE WITH THE TAIL ROTOR GEARBOX WAS CRACKED. VERIFIED CRACKED COUPLING AND REPLACED WITH SERVICEABLE. PN: 407-340-340-103
PILOT STATED THAT UPON TAKEOFF THERE WAS A TAIL ROTOR VIBRATION. THEY LANDED THE ACFT & CALLED THE ACFT OOS. WE DISPATCHED A MECHANIC & UPON HIS ARRIVAL IT WAS FOUND THAT THE DISC PACK COUPLING FOR ONE OF THE SHAFTS THAT HAD JUST BEEN REPLACED HAD 1 BOLT THAT WAS REMOVED & LAYING UNDER THE COWLING WHILE THE OTHER WAS HOLDING ON BY JUST A THREAD OF THE NUT. THERE WAS NO EVIDENCE OF TORQUE STRIPE HAVING BEEN APPLIED TO THE BOLT. I AM ATTACHING PICTURES SHOWING THE EXTENT OF THE DAMAGE & THE FINDINGS OF THE POST INCIDENT INSPECTION.
UPON DETAILED INSPECTION FOUND NR 2 TAIL ROTOR DRIVESHAFT COUPLING TO HAVE ABNORMAL PLAY.
MADE A PL FOR AN ENG CHIP LIGHT. NO OTHER ISSUES NOTED. MX FOUND A SMALL SLIVER. COMPLETED ENG OIL DRAIN, REPLACED CEFA FILTER, CLEANED ENG OIL FILTER, INSPECTED RESTRICTOR. DURING PENALTY GND RUN CHIP LIGHT ILLUMINATED. ORDERING REPLACEMENT ENG.
AFTER TAKEOFF DURING A PATIENT TRANSFER FLIGHT, THE PILOT ENCOUNTERED A CWP TRANSMISSION PRESS LIGHT ILLUMINATION. IMMEDIATELY PERFORMED AN UNSCHEDULED LANDING. PILOT REPORTED 3 MINUTES OF FLIGHT TIME WITH A 2 MINUTE COOL DOWN FROM THE TIME THE LIGHT ILLUMINATED. FOUND THE LOWER CHIP DETECTOR ON THE TRANSMISSION UNLOCKED. THERE WAS NO OIL LOSS FROM THE TRANSMISSION, NO TRANSMISSION OIL OVER TEMP & NO CHIPS FOUND. AFTER A 20 MINUTE GND RUN, THE ACFT CAN THEN BE RETURNED TO SERVICE. PERFORMED 20 MINUTE RUN, CHECKED THE FILTER & DETECTORS FOR CHIPS IN WHICH NO DEFECTS WERE NOTED. PRESS & TEMPS WERE NORMAL. ACFT APPROVED FOR RETURN TO SERVICE.
TRANSMISSION CHIP LIGHT, SAFE LANDING & VERY MINOR FUZZ. CLEAN PLUGS, 20 MIN PENALTY UNDER TORQUE, NO CHIP LIGHT, BIS.
MEDICAL CREW WAS ENROUTE WITH A PATIENT ONBOARD. AT 1.1 HOURS IN FLIGHT, THE PILOT ENCOUNTERED A TRANSMISSION CHIP LIGHT. AT THIS TIME A PRECAUTIONARY LANDING WAS MADE. FOUND SOME SMALL FUZZ PIECES ON THE LOWER CHIP DETECTOR. FOLLOWING THE STEPS OUT OF THE MM CHAPTER 63 & 12, THE TRANSMISSION WAS DRAINED THROUGH A PAPER FILTER FUNNEL IN WHICH ONLY A COUPLE OF VERY SMALL PIECES OF DEBRIS WAS FOUND. FLUSHED THE SYS WITH FRESH OIL, RE-SERVICED THE TRANSMISSION WITH NEW OIL & FILTER. PERFORMED A 20 MIN PENALTY RUN & DID NOT ENCOUNTER ANY ADDITIONAL ISSUES. ACFT WAS THEN RELEASED TO SERVICE.
PICKED UP PATIENT, ENROUTE, TRANSMISSION CHIP LIGHT CAME ON. UNEVENTFUL PRECAUTIONARY LANDING MADE. CHIP PLUGS CHECKED, FOUND SILVER FLAKES, TRANSMISSION BEING REMOVED FOR REPAIR.
