THRUST REVERSER WOULD NOT INDICATE EXTENDED. FOUND BRACKET THAT HOLDS THE RETRACT AND EXTEND SWITCHES TO BE CRACKED AROUND THE EXTEND SWITCH WHICH ALLOWED THE SWITCH TO MOVE WHEN THE THRUST REVERSER CONTACTED IT DURING EXTENSION. CAUSE APPEARS TO BE CRACKING DUE TO REPEATED CONTACT BETWEEN THRUST REVERSER AND SWITCH ON AN ALUMINUM BRACKET. REPLACED BRACKET.
THE SECONDARY YAW SERVO CABLE KEEPER WAS RUBBING THE PRIMARY YAW SERVO CABLE ON THE RT SIDE CAUSING EXCESSIVE WEAR ON BOTH COMPONENTS.
ADJUSTMENT SCREW BEGINNING TO FAIL. NOTED UPON VISUAL INSPECTION AFTER FAILURE OF SAME PART ON LEFT BRAKE.
ADJUSTMENT SCREW SHEARED IN 2 PIECES, NOTED UPON VISUAL INSPECTION. AIRCRAFT ARRIVED AT TEB WEDNESDAY EVENING 7-28-99. UPON LANDING, LEFT BRAKES DID NOT WORK AND AIRCRAFT WENT OFF RUNWAY INTO GRASS.
PILOT NOTICED DRAG CHUTE LID SEEMED LOOSE DURING PRE-FLIGHT INSPECTION. PILOT (IA RATED) RE-SECURED LID AFTER CONSULTING WITH MAINTENANCE. AFTER MAKING NEXT SCHEDULED FLIGHT, PILOT NOTED DRAG CHUTE MISSING FROM AIRCRAFT. PILOT NOTED NO ABNORMALITIES DURING PREVIOUS FLIGHT. UNKNOWN WHEN DRAG CHUTE DEPARTED ACFT. LEAR TECH REP INVESTIGATED AND FOUND DISCREPANCY WITH DRAG CHUTE LOCK RING AND DEPLOY MECHANISM. IT APPEARS LOCK RING HAD INTERFERENCE WITH ROUND HEAD RIVETS WHERE FLUSH HEAD RIVETS SHOULD HAVE BEEN INSTALLED. MAINT IS FOLLOWING UP ON CORRECTIVE ACTION TO LOCK RING AND DEPLOY MECHANISM. DRAG CHUTE ASSY IS NOTED IN ACFT RECORDS AS HAVING BEEN INSTALLED BY LEARJET 11 YEARS PRIOR TO THIS INCIDENT.
