DURING MAINTENANCE A CRACK WAS DISCOVERED ON THE LEFT HAND CLOSEOUT RIB (P/N: 5122115-55) AT L.W.S. 106.79. THIS CRACK ORIGINATES FROM A HOLE IN THE FORWARD ROW OF FASTENERS COMMON TO THE AFT MAIN LANDING GEAR TRUNNION SUPPORT PLATE AND TRAVELS VERTICALLY TO A SECOND FASTENER IN THE SAME ROW. THE CRACK IS CONSISTENT WITH OTHERS VISUALLY IDENTIFIED IN THIS SAME AREA. THIS RIB WILL BE REPLACED.
DURING MAINTENANCE THREE CRACKS WERE VISUALLY IDENTIFIED ON THE RIGHT HAND CLOSEOUT RIB (OEM P/N: 5122115-56). ALL CRACKS ARE LOCATED IN THE AREA OF FASTENERS COMMON TO THE FORWARD TRUNNION SUPPORT. THESE CRACKS ARE CONSISTENT WITH OTHERS THAT HAVE BEEN FOUND USING THE SAME INSPECTION METHOD. THIS RIB WILL BE REMOVED AND REPLACED.
DURING MAINTENANCE A SPOT OF CORROSION WAS VISUALLY IDENTIFIED ON THE LEFT HAND UPPER AFT OUTBOARD FORWARD WING ATTACH FITTING (P/N: 0822550-45). IT IS LOCATED ON THE BOTTOM FACE OF THE UPPER HORIZONTAL LEG OF THE FITTING AND MEASURES APPROXIMATELY .674 INCHES LONG, .320 INCHES WIDE, AND .020 INCHES DEEP. THIS CORROSION IS CONSISTENT WITH OTHERS FOUND USING A VISUAL INSPECTION TECHNIQUE. THIS FITTING WILL BE REPLACED USING NEW PARTS.
THE PILOT REPORTED AN ELECTRICAL SMELL IN THE COCKPIT DURING FLIGHT. MAINTENANCE INSPECTED AND FOUND THE HEATER IGNITION SYSTEM BURNT. THIS WAS REPLACED WITH NO FURTHER DEFECTS NOTED.
DURING NDT INSPECTION AS NOTED IN SID 57-10-16, A CRACK WAS DISCOVERED ON THE RIGHT HAND OUTBOARD NACELLE FITTING. (P/N: 5292029-24) THIS CRACK MEASURES APPROXIMATELY .100 INCHES LONG ON THE OUTBOARD LOWER LEG OF THE FITTING, AND IS CONSISTENT WITH OTHER CRACKS FOUND USING THE LIGHTED BORESCOPE INSPECTION TECHNIQUE (TEXTRON MEL54-02R2) TO SATISFY REQUIREMENT OF AD2017-10-09. THIS FITTING WILL BE REPLACED USING NEW PARTS.
PILOT REPORTED CABIN HEATER CIRCUIT BREAKER TRIPPED AND A STRONG ODOR IN THE CABIN. DURING INSPECTION THE HEATER OVERHEAT SWITCH WAS FOUND TO BE DAMAGED AND BURNED.
PILOT REPORTED BURNING SMELL IN CABIN, WITH THE ODOR SEEMING TO BE ASSOCIATED WITH THE HEATER. FOUND THE HEATER FUEL LINE DAMAGED. REPLACED THE FUEL LINE.

PILOT REPORTED THE UPPER CABIN DOOR OPENED IN FLIGHT. INSPECTED THE UPPER CABIN DOOR LATCH WITH NO DEFECTS FOUND IAW MM CHAPTER 52.
SMELL OF FUEL IN THE CABIN WHEN HEATER TURNED OFF IN FLIGHT. R & R THE HEATER FUEL PUMP. OPERATIONAL & LEAK CHECK WAS SATISFACTORY.
AT SCHEDULED INSPECTION, NLG DRAG BRACE, PN 5142002-5, WAS FOUND TO BE CRACKED IN THE AREA OF NLG ACTUATOR ATTACH POINT. METHOD OF INSPECTION WAS DYE PENETRANT. THIS CRACK WAS FOUND TO BE BEYOND LIMIT ALLOWABLE OF MEB91-11R1. THIS UNIT HAS BEEN SCRAPPED & A REPLACEMENT DRAG BRACE HAS BEEN INSTALLED IN ACFT.
DURING CRUISE, THE PILOT NOTED A THE ODOR OF SMOKE IN THE COCKPIT. AN UNSCHEDULED LANDING WAS MADE. THE PROBLEM WAS TROUBLESHOT TO A PICK UP IN THE HSI. THE HSI WAS R & R.
PILOT REPORTED THAT AFTER TAKEOFF, DURING CLIMB, THE LT ENGINE BEGAN RUNNING ROUGH. PILOT RETURNED TO DEPARTURE. ON MX INSPECTION, A DAMAGED EXHAUST VALVE WAS FOUND ON LT ENGINE NR 5 CYLINDER. CYLINDER WAS R & R. DAMAGE WAS ALSO NOTED TO THE LT ENGINE TURBO CHARGER. LT ENGINE TURBO WAS R & R.
AT SCHEDULED INSPECTION, NLG DRAG BRACE OEM PN 5142002-5, WAS FOUND TO BE CRACKED IN THE AREA OF NLG ACTUATOR ATTACH POINT. METHOD OF INSPECTION WAS DYE PENETRANT. THIS CRACK WAS FOUND TO BE WITHIN ALLOWABLE LIMIT. THIS UNIT HAS BEEN REPAIRED IAW PROCEDURE OUTLINED IN MEB941-11R1 & RETURNED TO SERVICE.
CREW REPORTED RT MAIN LANDING DOWNLOCK LIGHT FAILED TO ILLUMINATE ON APPROACH. PERFORMED GEAR EXTENSION, WILL NOT EXTEND HY, LANDING WITH DEFECTIVE GEAR CHECKLIST. PERFORMED EMERGENCY GEAR EXTENSION, STILL NO DOWNLOCK INDICATION. DECLARED EMERGENCY, LANDING WAS UNEVENTFUL. MX PERFORMED MULTIPLE GEAR SWINGS, COULD NOT DUPLICATE DISCREPANCY. R & R RT MLG ACTUATOR AS A PRECAUTIONARY MEASURE. ACFT RELEASED BACK TO SERVICE.
CREW REPORTED OIL LEAK, RIGHT ENGINE. UPON FURTHER INSPECTION, FOUND 1.25 INCH TO 1.5 INCH LONG CRACK IN CRANKCASE JUST AFT OF NR 2 CYLINDER, SEVENTH STUD, LOCATED IN WEB BETWEEN OIL COOLER MOUNTING AREA AND CYLINDER ATTACH AREA.
