THE FLIGHT CREW REPORTED HAVING A CABIN PRESSURIZATION FAULT, DESCRIBING THE CABIN ALTITUDE CLIMBED RAPIDLY ON ASCENT TO FLIGHT LEVEL 210 AND THEN FULLY DEPRESSURIZED WHILE DESCENDING THROUGH 16000 FT. THE LANDING WAS UNEVENTFUL. THE MAINTENANCE PROVIDER REPLACED THE CABIN PRESSURE CONTROLLER ASSY WITH REPAIRED UNIT. ADDITIONAL TROUBLESHOOTING FOUND A VACUUM HOSE ATTACHING TO THE PRESSURE CONTROLLER COLLAPSED AND LEAKING. OPS CHECKS WERE SATISFACTORY, AND THE AIRCRAFT WAS RETURNED TO SERVICE.
DURING CHECKS FOR A PRESSURIZATION FAULT, THE MAINTENANCE PROVIDER NOTED THAT THE UPPER FORWARD AND AFT CAM ROLLER STOP BOLTS WERE BROKEN. THESE STOP BOLTS ASSIST IN THE POSITION OF THE DOOR WHEN CLOSED IN THE FUSELAGE OPENING. THERE WERE NO DEFECTS NOTED CAUSING THE BOLTS TO HAVE BROKEN. THE REMAINS OF THE CAM BOLTS WERE REMOVED, AND NEW CAM BOLTS INSTALLED AND RIGGED
THE FLIGHT CREW REPORTED THAT AN AC ESS BUS ANNUNCIATOR ILLUMINATED IN FLIGHT. THE MAINTENANCE BASE TROUBLESHOT AND DETERMINED THE K1XD SENSE RELAY WAS DEFECTIVE. THE DEFECTIVE RELAY WAS REMOVED AND A NEW UNIT INSTALLED. OPS CHECKS WERE SATISFACTORY AND THE AIRCRAFT WAS RETURNED TO SERVICE.
THE FLIGHT CREW REPORTED THAT DURING CLIMBOUT AFTER TAKEOFF ALL MODES OF PRESSURIZATION FAILED. THIS REQUIRED THE FLIGHT CREW TO DEVIATE THE FLIGHT AND MAKE AN UNSCHEDULED LANDING. OPERATIONS WERE CONTINUED UNDER MEL 21-31-1-1. DURING THE MAINTENANCE EVENT THE MAINTENANCE VENDOR FOUND THE PRESSURE CONTROLLER TO BE DEFECTIVE. THE CONTROLLER WAS REMOVED AND REPLACED WITH A SERVICEABLE UNIT. OPS CHECKS WERE SATISFACTORY AND THE AIRCRAFT WAS RETURNED TO SERVICE.
THE FLIGHT CREW REPORTED OF A PRESSURIZATION FAULT AFTER TAKEOFF TO 9500FT WHEN THEY ELECTED TO RETURN BACK TO MMUN FOR LANDING AND EVALUATION. OPERATIONS WERE CONTINUED PER MEL 21-31-1. 16NOV2021 - THE MAINTENANCE TEAM TROUBLESHOT AND REMOVED AND REPLACED THE AUTOMATIC CABIN PRESSURE SELECTOR WITH A REPAIRED UNIT. OPS CHECKS WERE SATISFACTORY AND THE AIRCRAFT WAS RETURNED TO SERVICE.
THE FLIGHT CREW REPORTED THAT THE AIRCRAFT FAILED TO PRESSURIZE ON DEPARTURE IN BOTH AUTO AND MANUAL PRESSURE CONTROL MODES. THE MAINTENANCE VENDOR DETERMINED A DEFECTIVE REGULATING VALVE AND REPLACED IT WITH A NEW VALVE. OPERATIONAL CHECKS WERE SATISFACTORY AND THE AIRCRAFT WAS RETURNED TO SERVICE
