Maintenance disclosures
ON SHORT FINAL APPROACH, THE FLIGHT CREW NOTED A LOSS OF HYDRAULIC FLUID ON THE LEFT SYSTEM. THE EMERGENCY LANDING GEAR EXTENSION WAS UTILIZED FOR GEAR EXTENSION AND THE AIRCRAFT LANDED SAFELY. ON TAXI, THE NOSE WHEEL STEERING FAILED DUE TO THE FLUID LOSS AND THE AIRCRAFT WAS TOWED TO THE FBO. MAINTENANCE WAS SECURED TO INSPECT AND REPAIR THE AIRCRAFT. TECHNICIANS FOUND A LOOSE FITTING ON THE LEFT GROUND SPOILER SELECTOR VALVE. THE FITTING WAS RE-TORQUED, HYDRAULIC SYSTEM RE-SERVICED AND SYSTEM LEAK CHECKS PERFORMED WITH NO LEAKS NOTED. THE AIRCRAFT WAS SUBSEQUENTLY RETURNED TO SERVICE.
AFTER TAKEOFF, NR 4 ENGINE OIL PRESSURE DROPPED TO RED ZONE. CREW SHUT ENGINE DOWN. ENGINE VIB CYCLED BETWEEN 1.1 & 1.9. ROUGHLY 1 MINUTE AFTER TAKEOFF NR 4 OIL PRESSURE ILLUMINATED. NR 4 OIL PSI WAS IN THE RED. NO VIBRATION WAS FELT ON THE AIRCRAFT. NR 4 ENGINE CONTINUED TO WINDMILL AFTER SHUTING IT DOWN. INSTALLED NR 4 ENGINE P/N ON/OFF 2-003-040-15, S/N OFF P07867, S/N ON LF07637.
PAX DOOR 2 RIGHT GIRT BAR END CAPS NOT YELLOW IN COLOR (WERE WHITE). REPLACED SLIDE ASSEMBLY

DURING FLIGHT, LT GEAR NOT INDICATING UP AND LOCKED. RED LIGHT ILLUMINATED AND GEAR HANDLE LIGHT ILLUMINATED. AIR RETURN TO MSP. MAINTENANCE FOUND ICE COMPLETELY AROUND DOOR UPLOCK LATCH AND SURROUND DOOR WAS UNABLE TO LATCH CLOSED. REMOVED ICE, NO DAMAGE NOTED AT DOOR OR LATCH NOTED.
DURING NR 1 CARGO FIRE EXTINGUISHER CHANGE FOUND FLEX TUBE P/N AE711192, UNION P/N AN832 SOLID LINE P/N AE40114-4 FILLED WITH UNKNOWN SUBSTANCE. MAINTENANCE REMOVED AND REPLACED RIGID AND FLEXIBLE LINE IAW AMM CH 26, ALL CHECKS GOOD.
DURING SCHEDULED CAPACITY CHECK, BATTERY PACK PN/61-2275-1 SN/9810762 FAILED. MAINTENANCE REPLACED BATTERY ASSEMBLY PN 61-2274-1. UNIT PASSES CMM CHECKS. RETURNED TO SERVICE 8/17/04. REFER TO WO NR 373901 FOR ADDITIONAL INFORMATION.

CHECKED OUTPUT VOLT FROM NET FILTERS. CHECKED SYS WIRING. ALL CHECKS GOOD. REPLACED RELAYS 2HL14 & 2HL15, BOARD CAPACITOR 2HL23, & PCB 2HL18. PERFORMED SCREEN HEAT OPS TEST IN BOTH GROUND & AIR MODE. CHECKS GOOD. REF MM409 066888.
DURING FLIGHT 3406, WEATHER RADAR FAILED INFLIGHT. AIR RETURN TO DTW. MAINTENANCE CND PROBLEM. CHANGED R/T AS A PRECAUTION, OP CHECKS GOOD.
DURING FLIGHT 3645, ESCAPE ROPE PANEL KEEPS FALLING DOWN. MAINTENANCE APPLIED NEW HOOK AND LOOP TO THE AFFECTED AREA AS NEEDED SEC CHECKS GOOD.
DURING FLIGHT 3657, AIR BRAKE DEPLOYED UNCOMMANDED. AIR RETURN TO MSP. MAINTENANCE PERFORMED ADJUSTMENT TEST PROCEDURES IAW AMM 27-63-00 501. ALL OPS CHECK GOOD. CLEANED CL4 POTENTIOMETER CANNON PLUG.
EN ROUTE ON FLIGHT 3566, YELLOW HYDRAULIC LIGHT TEMP ILLUMINATED. MAINTENANCE REPLACED AC PUMP IAW AMM 29-20-11 AND REPLACED YELLOW ISOLATION VALVE. OPS AND LEAK CHECKS, NO DEFECTS NOTED.
ON APPROACH OF FLIGHT 3653, NR 4 ENGINE OIL PRESSURE DROP TO ZERO. LOW OIL PRESSURE LIGHT REMAIN OUT. AFTER LANDING PILOT SHUTDOWN ENGINE. LIGHT CAME ON AND INDICATED PRESSURE PEGGED OUT. MAINTENANCE REPLACED P1 CONNECTOR AT 442-00-00.
DURING C-CHECK IN CVG, THE LT O/B FLAP TAB SUFFERED CHAFE DAMAGE TO THE UPPER SKIN BETWEEN RIBS 11 & 12. DAMAGE REPAIRED IN ACCORDANCE WITH SRM 57-55-11 201 FIG 204 AND EO A57038.
DURING C-CHECK IN CVG, CRACK ON RT SIDE OF LOWER VERTICAL STABILIZER, LOCATED AT STAB TO FUSELAGE CLOSING ANGLE. REPAIRED IN ACCORDANCE WITH BAE FAX BARA/AVR/5825/02-DC, RIL HC550H9004 ISS 3 AND EO A55013.
