ABOUT 20-30 MINUTES INTO FLIGHT DURING THE CLIMB OUT AT AROUND FL270-290, THE FLIGHT CREW RECEIVED AN AMBER L HYD TEMP HIGH CAS MESSAGE, FOLLOWED ALMOST IMMEDIATELY WITH A BLUE L HYD SOV CLOSED CAS MESSAGE. AFTER A FEW SECONDS, THE AMBER L HYD TEMP HIGH CAS MESSAGE WENT AWAY, ONLY TO REAPPEAR AGAIN WITH A CHIME A FEW SECONDS LATER. THE FLIGHT CREW REVIEWED THE QRH FOR THE L HYD TEMP HIGH INDICATION. THE LEFT SYSTEM HYDRAULIC TEMPERATURE WAS READING ROUGHLY AROUND 54-56 DEGREES C. THE LEFT SYSTEM QUANTITY PERCENTAGE WAS AT AROUND 67%. THE TEMP INDICATION OF 54-56 C WAS AMBER TO CORRESPOND WITH THE CAS MESSAGE. AT THAT POINT THE FLIGHT CREW ELECTED TO DIVERT TO AN ALTERNATE AIRPORT AND DECLARE AN EMERGENCY. DUE TO CONTINUING CAS MESSAGES, THE QRH INSTRUCTED THE CREW TO DEACTIVATE THE LEFT HYDRAULIC PUMP. AN UNEVENTFUL LANDING WAS ACCOMPLISHED. ONCE ON THE GROUND THE CREW NOTICED HYDRAULIC FLUID LEAKING FROM THE LEFT ENGINE COWL. MAINTENANCE WAS CONTRACTED TO INSPECT AND REPAIR THE AIRCRAFT. MAINTENANCE CREWS FOUND THE LEFT ENGINE DRIVEN HYDRAULIC PUMP EXTERNAL SURFACES TO BE EXTREMELY HOT, EVIDENT THAT THE PUMP WAS SEIZING. CURRENTLY AT THE TIME OF THIS REPORT, THE HYDRAULIC PUMP IS BEING REPLACED. ALONG WITH PUMP REPLACEMENT, THE HYDRAULIC FILTERS ARE TO BE INSPECTED AND HYDRAULIC SYSTEM FLUSHED. OPEN

REMOVED RIGHT SERVICE DOOR SLIDE AND PLACED IN CARGO AREA DEACTIVATED AND BLOWN INSPECTION A/C AND FOUND SAFE FOR ONE TIME FERRY FROM BTV TO DTW FOR REPLACEMENT OF SLIDE. L/P 1609251:A/C TO BE FERRIED FROM BTV TO DTW SLIDE 1R BLOWN AND PLACED IN CARGO AREA. L/P 1609251: REPLACED RIGHT FORWARD SERVICE DOOR ESCAPE SLIDE WITH SERVICEABLE UNIT. P/N ON/OFF D31050-105, S/N OFF 1750, S/N ON 1747, ALL CHECK GOOD, REF J/C 2560-501.

WHILE CLIMBING THROUGH 17K, GREEN HYD HI TEMP LIGHT ILLUM, COMPLIED W/ CHECKLIST, FEW MIN LATER FA REPORTED A HAMMERING SOUND FROM THE AFT CABIN, HYD QTY FULL, NO OTHER SYMPTOMS ON SHORT FINAL 'RUDDER VALVE' LIGHT ILLUM. COMPLETED PRE AND POST FIX HYD LINE TEMPS, REPLACED LANDING GEAR SELECTOR VALVE, INSPECTED TAIL SECTION FOR LOOSE OBJECTS NO DEFECTS. NOTED RUDDER VALVE LIGHT NORMAL W/ZERO GREEN HYD PRESSURE.

THE AIRCRAFT SUFFERED DAMAGE TO THE FUSELAGE SKIN AND UNDERLYING STRUCTURE BETWEEN FRAMES 38 TO 40 AND BETWEEN 19 AND 20. THE DAMAGE WAS CAUSED BY IMPACT WITH GROUND SUPPORT EQUIPMENT. DAMAGE WAS REPAIRED IN ACCORDANCE WITH BAE REF BARA/AVR/7030/05.

AFTER LANDING WHILE TAXIING TO GATE ALL PAX O2 MASKS IN CABIN DEPLOYED. INSPECTED NOTED LT FLT DECK PAX O2 OVERRIDE SWITCH CONTAMINATED WITH MOISTURE AND SHORTED OUT SWITCH. REPLACED SWITCH P/N S2802-002. ALSO NOTED 11AB, 13AB CHEMICAL O2 GENS ACTIVATED. REPLACED CANISTERS P/N 117028-03 11AB, S/N OFF NSN, S/N ON 9903986 13AB, S/N OFF NSN, S/N ON 9903984, SYS OPS TESTS GOOD.
DURING FLIGHT NR 3527, NR 2 ENGINE LOW OIL PRESSURE LIGHT. NR 2 ENGINE SHUTDOWN. OIL QUANTITY READ ZERO. OIL PRESSURE 40 PSI, AT 80 PERCENT N2, OIL TEMP 160 C. MAINTENANCE REMOVED AND REPLACED NR 2 ENGINE PN 2-003-040-015, SN/OFF PO7894, SN/ON P07665 IAW MAL 7100-502. REFER JOB CARDS FOR DETAILS.
DURING FLIGHT, NR 2 PACK VALVE INDICATES OPEN WHEN SELECTED CLOSED. VALVE CURRENTLY ON MEL. ALSO RECIRC CAN BE SELECTED TO RECIRC WITH ONLY 1 PACK RUNNING. A LOUD POPPING SOUND FROM REAR OF A/C. AIR RETURN TO MSP. MAINTENANCE REMOVED AND REPLACED PERI-SEALS ON NR 2 PACK VALVE (P/N 706-3-250). OPS CHECK AND LEAK CHECK GOOD.
AFTER TAKEOFF AND CHANGE OVER FROM APU AIR TO ENGINE AIR GOT HEAVY SMOKE IN FLT DECK, WHEN NR 1 PACK WAS SELECTED ON. SELECTED NR 1 PACK OFF, SMOKE DISSIPATED. RAN QRH, RETURNED FOR LANDING. ACFT TO BE FERRIED, FOR NR 1 ENGINE REPAIR/REPLACEMENT. MX INSPECTED PACK ASSY, FOUND OK. FOUND NR 1 ENGINE OIL LEVEL 2 QTS LOW. SERVICED OIL, RAN FOR 1/2 GR AT CRUISE POWER. CONSUMPTION AT 1/16 QT PER HOUR. SUSPECT NR 2 LAB SEAL LEAK. ACFT SAFE FOR NR 4 ENGINE FERRY FLIGHT. A/C TO BE FLOWN WITH NR 1 PACK OFF (CURRENTLY REMAINS DEFERRED) NR 1 ENGINE BLEED OFF UNLESS REQUIRED FOR AIRFRAME ANTI-ICE. REPLACED NR 1 ENGINE P/N 2-003-040-15, S/N OFF P07892, S/N ON P07723. DURING FLIGHT 3407, CREW REPORTED SMOKE IN THE COCKPIT. AIR RETURN TO DTW. REFER TO SDR MALA030225 FOR ADDITIONAL INFORMATION. MAINTENANCE REPLACED NR 4 ENGINE.
DURING FLIGHT 3520 AFT F/A SMELLED SULPHUR BURNING SMELL IN AFT CABIN FOLLOWED BY MELTED PLASTIC SMELL. FLIGHT CREW SMELLED IT BRIEFLY BUT SMELL LINGERED IN AFT CABIN. MAINTENANCE INSPECTED ELECTRICAL COMPONENTS. ABOVE AFT CEILING PANELS AND ALL LIGHTING, PA SYSTEM, AIR SYSTEMS AND CIRCUIT BREAKERS. NO DEFECTS NOTED. ALL ASSOCIATED SYSTEMS OPERATED NORMALLY. COULD NOT DUPLICATE DISCREPANCY.

WHILE INSPECTING AC 535, OUT OF CVG, MAINTENANCE REPAIRED DENT IN FUSELAGE AND ASSOCIATED DAMAGE AT FRAMES 9, 10 & 11 IAW REPAIR OF FUSELAGE DENT AND ASSOCIATED DAMAGE AT FRAMES 9, 10 AND 11 IAW EO A53078.
DURING C1 CHECK, CHAFE MARKS ON RT GEAR RT DOOR (MLG) LOWER FORWARD. MAINTENANCE REPAIRED IAW LOWER FWD CORNER REPAIRED IAW BAE FAX BARA/AVR/1006/02-CV.
WHILE IN C-CHECK, IN CVG, REPAIR OF FUSELAGE DENT AT FRAME 4 IAW EO A53077.
During Flight #3610, Nose gear would not retract after Takeoff. Red Transition light on handle and Indicator. Air Turnback to MSP. Maintenance found uplock Proximity sensor bad. Replaced Proximity switch PN 8-018-01. IAW AMM 32-60-21. Ops checks of switch and gear swings all good.
LEFT MLG WOULD NOT RETRACT AFTER TAKEOFF. FLIGHT DIVERTED TO CVG. ON TAKEOFF OUT OF TYS THE LT MAIN LANDING GEAR WOULD NOT RETRACT. CYCLED GEAR DOWN AND UP 2 ADDITIONAL TIMES, NO EFFECT. LT GEAR RED AND GEAR HANDLE WERE ILLUMINATED. VERIFIED BY AC YAW TO BE EXTENDED. EACH TIME IT WAS CYCLED IT WAS INDICATING DOWN AND LOCKED. MAINTENANCE TROUBLESHOT RETRACTION PROBLEM FOUND RETRACTION BRAKE ISOLATION VALVE BAD. REPLACED ISOLATION VALVE (P/N HP94240055) (S/N ON/ 0243), IAW 32-31-30 OF AMM, SWUNG GEAR 12 TIMES. NO FAULT FOUND. OPS CHECK GOOD.
EVACUATION SLIDE ACCIDENTALLY FELL FROM MOUNTING. MAINTENANCE REPLACED EVAC SLIDE. SECURITY GOOD.
AUX PITOT HEATER POPPED TWICE, RESET ONCE.. C/B POPPED DURING FLIGHT IN RAIN AND CLOUDS. MAINTENANCE REPLACED AUX PITOT HEATER C/B. OPS CHECKS GOOD.
AUX PITOT HEATER FAIL CAUTION LIGHT ILLUMINATED NOTED C/B POPPED CHOSE NOT TO RESET SINCE OCCURRED ON PREVIOUS FLIGHT. TOOK ABOUT 20 MINUTES FOR C/B TO POP AFTER SELECTING SWITCH ON. MAINTENANCE COULD NOT DUPLICATE PROBLEM ON GROUND DUE TO LENGTH OF TIME IT TAKES TO POP C/B. REMOVED AND REPLACE AUX PITOT HEAT UNDER CURRENT MONITOR. ALL CHECKS GOOD AT THIS TIME.
CARGO SMOKE DETECTION SYSTEM FAILED AND THE CARGO FIRE EXTINGUISHER WAS FIRED. MAINTENANCE SUSPECTED BAD RELAY FROM STOCK, REPLACED RELAY 2HL14. OPS CHECKED GOOD. MAINTENANCE ALSO REPLACED NR 1 CARGO FIRE EXTINGUISHER.
