ON CLIMB, THE FLIGHT CREW REPORTED SMOKE IN THE CABIN AND AIR CONDITIONING TEMPERATURE HIGH. THE FLIGHT CREW DONNED THEIR OXYGEN MASKS AND DIVERTED BACK TO BNA. MAINTENANCE WAS CONTACTED TO INSPECT AND REPAIR THE AIRCRAFT. THE AIR CYCLE MACHINE WAS FOUND TO BE SEIZED. THE ACM WAS REPLACED AND OPERATIONAL CHECKS TO BE COMPLETED. IF FURTHER DISCREPANCIES ARE DISCOVERED, A SUPPLEMENTAL SDR WILL BE SUBMITTED.

DEPARTING OUT OF KJAC THE GEAR DISAGREE DISPLAYED ON THE EICAS AFTER TAKEOFF. THE LEFT GEAR WOULD NOT RETRACT. WE COULD ALSO FEEL AND HEAT THAT THE LEFT GEAR WAS STILL DOWN. RAN THE QRH CHECKLISTS, EXTENDED THE LANDING GEAR, CONFIRMED 3 GREEN “DOWN AND LOCKED” AND RETURNED TO KJAC. MX C/A: IN WORK, TROUBLESHOOTING WITH MX VENDOR AND ACCORDING TO MDC DOWNLOADS AND BAS-CRC, DOWN LOCK SENSORS NEED TO BE RIGGED. (~RTS 1/29)

CLIMBING THRU 18,000 FT, EXPERIENCED A LT ENGINE FAILURE. DECLARED AN EMERGENCY & DIVERTED. HEAVY VIBRATION IN AIRFRAME, AILERONS, ELEVATOR & RUDDER. FELT IT WAS A CATASTROPHIC ENG FAILURE, PERFORMED ENG SHUTDOWN CHECKLIST. AN UNEVENTFUL LANDING ACCOMP. INSP REVEALED EXTENSIVE DAMAGE TO LPT. INVESTIGATION FOUND ENG FAILURE DUE TO SEPARATION OF ROTATING HPT2 INTERSTAGE SEAL, WHICH DISRUPTED COOLING, LEADING TO HEATING OF HPT2 DISK RIM. DISK GROWTH ADVANCED TO A SINGLE HPT2 BLADE NO LONGER SECURED, SEPARATION OF THE SINGLE HPT2 BLADE. SEPARATION CAUSED FAILURE TO ADJACENT & DOWNSTREAM FLOW PATH. ENGINE BEYOND ECONOMIC REPAIR & REPLACED. ONGOING ROOT CAUSE INVESTIGATION CONDUCTED MANUFACTURER.
