LEVELING OFF @ FL430, AMBER WOW FAIL AND CYAN PROX SYS FAULT. MX C/A: R&R PSEU, OPS TEST OF GEAR EXTENSION AND RETRACTION GOOD, NO DISCREPANCIES DENOTED AT THIS TIME. (REF. CH300 AMM 32-61-09, 32-30-00)
WHILE IN CRUISE FLIGHT, THE CREW NOTICED THE RIGHT SIDE HYDRAULIC QUANTITY INDICATION DISPLAY HAD TURNED TO WHITE. EVENTUALLY THE QUANTITY HAD DECREASED TO BELOW 4 PERCENT AND CAS MESSAGES WERE POSTING. THE CREW ELECTED TO DECLARE AN EMERGENCY AND DIVERT INTO MSY. AN UNEVENTFUL LANDING WAS ACCOMPLISHED AND MAINTENANCE SECURED TO INSPECT AND REPAIR THE AIRCRAFT. MAINTENANCE FOUND A PRESSURE LINE AT THE HYDRAULIC SWITCHING VALVE LEAKING. THE O-RINGS WERE REPLACED, RESERVOIR SERVICED AND LEAK CHECKS WERE ACCOMPLISHED WITH NO DEFECTS NOTED. THE AIRCRAFT WAS SUBSEQUENTLY RETURNED TO SERVICE WITH NO ADDITIONAL RE-OCCURRENCES.
WHILE CLIMBING THROUGH 7500 FEET, THE FLIGHT CREW HEARD A LOUD POP THAT WAS FOLLOWED BY A HISSING SOUND COMING FROM THE CABIN DOOR. THE CREW RETURNED TO THE DEPARTURE FIELD AND LANDED WITHOUT INCIDENT. ONCE ON THE GROUND, THE FLIGHT CREW NOTED A SMALL GAP BETWEEN THE CABIN DOOR AND DOOR FRAME WITH THE DOOR CLOSED. MAINTENANCE TECHNICIANS REPLACED THE DOOR SEAL AND PERFORMED PRESSURIZATION CHECKS. THE DOOR SEAL FUNCTIONED PROPERLY AND THE AIRCRAFT WAS RETURNED TO SERVICE.
DURING CRUISE AT FL430, THE FLIGHT CREW NOTED THAT THE ENGINE SYNC WAS PERFORMING POORLY, SO THE CREW TRIED TO MANUALLY MATCH THE N1 FAN SPEEDS, AT WHICH TIME THEY DISCOVERED THAT THE RIGHT ENGINE WAS NOT PRODUCING CLIMB POWER IN THE CLIMB DETENT, AND CONTINUED TO GRADUALLY GET WORSE. THE AIRCRAFT STARTED TO HAVE DIFFICULTY MAINTAINING CRUISE SPEED, SO THE DECISION WAS MADE TO DIVERT TO SLC. DURING DESCENT, THE ENGINE PERFORMANCE DEGRADED EVEN FURTHER; AN EMERGENCY WAS DECLARED AND AN UNEVENTFUL LANDING ACCOMPLISHED. MAINTENANCE WAS INSTRUCTED TO REPLACE THE RH ENGINE ECU B (P/N 2119576-1011 S/N 64-V0040) DUE TO FAULT CODE MCID 0817 (P3 FAULT) PRESENT. ECU B WAS REPLACED WITH A REPAIRED UNIT; ENGINE OPERATIONAL CHECKS WERE NORMAL. THE AIRCRAFT IS SCHEDULED FOR A MAINTENANCE CHECK FLIGHT.
WHILE ON THE GROUND TAXIING IN, THE FLIGHT CREW RECEIVED A LT ENGINE OIL CHIP CAS MESSAGE. A LOCAL MAINTENANCE VENDOR WAS CONTACTED TO PERFORM A CHIP DETECTOR INSPECTION AND ENGINE OIL SOAP SAMPLE, ALONG WITH ENGINE DATA DOWNLOADS. ALL CHIP DETECTORS WERE FOUND TO HAVE SUBSTANTIAL METAL DEBRIS, AND THE OIL SOAP LAB RESULT CAME BACK AS EXCESSIVE CARBON. AN ENGINE MAINTENANCE VENDOR PERFORMED THE ON WING ENGINE TEAR DOWN AND INSPECTION. IT WAS DETERMINED FROM THE INSPECTION FINDINGS THAT THE NR 4 BEARING CARBON SEAL HAD FAILED WHICH RESULTED IN CARBON SEAL AND METAL FRAGMENTS OBSERVED IN THE STUB SHAFT, NR 4 AND NR 5 BEARING REGION. THERE WAS NO FAILURE OF THE NR 4 AND NR 5 BEARINGS, BOTH WERE INTACT. THE ENGINE WILL BE REPAIRED ON WING.
DURING DESCENT AND PASSING THRU FL280, FLIGHT CREW DETECTED AN IMMEDIATE AND NOTICEABLE VIBRATION WHICH BECAME CONSIDERABLY WORSE AS THE DESCENT CONTINUED, CREW THEN RECEIVED A RH ENGINE VIBRATION CAS MESSAGE. AFTER COMPLETING THE ENGINE VIBRATION CHECKLIST, THE CREW SHUT DOWN THE RT ENGINE, AS THE VIBRATION WAS WORSENING AND BECOMING MORE SIGNIFICANT. FLIGHT CREW DECLARED AN EMERGENCY AND LANDED UNEVENTFULLY. MAINTENANCE WILL BE REMOVING THE ENGINE AND SENDING IT TO A REPAIR FACILITY FOR TEAR DOWN AND ANALYSIS.
WHILE ON TAKEOFF ROLL THE FLIGHT CREW NOTED A METALLIC SMELL & EVENTUALLY BLUE SMOKE AS THEY ROTATED. SUSPECTING AN AIR CONDITION PACK FAILURE THE PACK WAS TURNED OFF & RAM AIR OPENED TO CLEAR THE SMOKE. AN EMER WAS DECLARED & ON RETURN THE SMOKE & SMELL DISSIPATED. AN UNEVENTFUL LANDING WAS ACCOMPLISHED. FOUND THE TURBINE WHEEL BEARING SEIZED ON THE ACM PN 3471A020000. THE ACM WAS REPLACED WITH A REPAIRED UNIT & ALL LEAK & OPERATIONS CHECKS PERFORMED, WITH NO DEFECTS NOTED. ACFT WAS RETURNED TO SERVICE.
