AIRCRAFT WAS FLYING AN EXTERNAL LOAD OPERATION WHEN THE PILOT NOTICED THE RED LOW HYDRAULIC WARNING IND LIGHT WAS ILLUMINATED. THE PILOT WAS ABLE TO SAFELY SET LOAD ON THE GROUND AND LAND BACK AT THE HELI-PAD. UPON VISUAL INSPECTION OF THE HYDRAULIC PUMP , THE DRIVE SPLINES WERE CHEWED UP AND THE BEARINGS INSIDE WERE ROUGH/ BINDING WHEN TURNED BY HAND.
DURING A TRAINING FLIGHT, THE FUEL PRESSURE CAUTION LIGHT ILLUMINATED. THE PILOT IMMEDIATELY TURNED ON THE FUEL PUMP AND PERFORMED A PRECAUTIONARY LANDING. AFTER SHUTDOWN, FUEL WAS OBSERVED ON THE FUSELAGE. SUBSEQUENT EXAMINATION BY A MECHANIC REVEALED A LOOSE MAIN FUEL FEED HOSE FITTING AT THE ENGINE FIREWALL SHUT-OFF VALVE UNION. THE HOSE FITTING WAS THEN TIGHTENED AND SAFETIED PER AIRBUS MAINTENANCE MANUAL INSTRUCTIONS BY A COMPANY MECHANIC, AND AN AIRCRAFT/ENGINE GROUND RUN, FOLLOWED BY A FLIGHT, WAS SUCCESSFULLY COMPLETED WITHOUT FURTHER INCIDENT.
GEAR SET REPLACED AT THE FIRST OVERHAUL. SECOND GEAR SET REPLACED AT THE FIRST OVERHAUL WITH NEW PART. PART TOTAL CYCLES: 5,975.
BEARING SET DELAMINATED BEYOND LIMITS. REMOVED FROM TAIL ROTOR ASSY, S/N 6173. TOTAL TIME ON TAIL ROTOR ASSY, 499.2 HOURS. HAD BEEN ON S/N 2890 FOR 147.2 HOURS. INSTALLED NEW BEARING SET.
INTERMITTENT HIGH FREQUENCY DEVELOPED IN AIRCRAFT. EVERY DRIVE COMPONENT HAS BEEN CHANGED OUT EXCEPT THE DRIVE SHAFT AND TAIL BOOM. DRIVE SHAFT REPLACED WITH RE-CONDITIONED ONE FROM FRANCE. VIBRATION GONE FOR NOW.
