AFTER TAKEOFF FROM KELM, APPROXIMATELY 600 AGL, RIGHT ENGINE FLAMEOUT. PILOT NOTICED THE RIGHT FUEL FLOW WAS AT ZERO AND PILOT CHECKED RIGHT ENGINE CONTROL LEVERS, STOP AND FEATHER, BOOST PUMPS AND IGNITION THEN ENGINE HAD WOUND DOWN. PILOT HAD REQUESTED AN EMERGENCY TO RETURN TO KELM DUE TO RIGHT ENGINE FAILURE. PILOT REFERENCE QRH PROCEDURE TO SECURE THE ENGINE AND PERFORM A SINGLE ENGINE LANDING AND SAFELY LANDED AT KELM. BUF MAINTENANCE INVESTIGATED AND FOUND RIGHT ENGINE STOP AND FEATHER CONTROL CABLE MALFUNCTION. REMOVED AND REPLACED STOP AND FEATHER CONTROL CABLE ASSEMBLY. OPERATIONAL CHECK GOOD.
THE PILOT REPORTED THAT THE RIGHT EXPERIENCED A FLAME ON DEPARTURE FROM KELM. THE PILOT SECURED THE RIGHT ENGINE, RETURNED TO ELM AND LANDED WITHOUT INCIDENT. AMF BUF MX TRAVELED TO KELM AND FOUND THE RIGHT ENGINE STOP / FEATHER CONTROL CABLE WAS LOOSE IN THE PEDESTAL AREA. RE-SECURED CABLE, CHECKED TRAVEL OPERATION PER THE SA227 MAINTENANCE MANUAL CHAPTER 76-10-00, RAN ENGINE TO FULL POWER, OPERATIONAL CHECK WAS NORMAL. AS A PRECAUTIONARY MEASURE THE CABLE PART NUMBER 32-19012-077 WAS REPLACED WHEN THE AIRCRAFT RETURNED TO THE MAINTENANCE BASE AT KBUF.
THE PILOT REPORTED THAT AFTER TAKEOFF, THE CABIN DOOR WARNING ANNUNCIATOR ILLUMINATED. DECLARED AN EMERGENCY AND LANDED BACK AT DEPARTURE WITHOUT INCIDENT. MAINTENANCE INSPECTED THE DOOR LATCHES, FOUND TO BE WITHIN LIMITS. ADJUSTED ONE OF THE SWITCHES, CONTACTED BY THE LATCHES AND OPERATION OF THE WARNING SYSTEM, FOUND NORMAL AND NO SUBSEQUENT ISSUES REPORTED.
INBOUND, PILOT NOTICED TOTAL HYD SYS FAILURE. PILOT DECLARED AN EMER & PERFORMED A MANUAL LG EXTENSION. PILOT RECEIVED THREE GREEN DOWN & LOCKED INDICATOR LIGHTS & LANDED WITHOUT FURTHER INCIDENT. TROUBLESHOOTING REVEALED FAULTY HYD ACCUMULATOR. REPLACED ACCUMULATOR & PERFORMED HYD SYS OPERATIONAL CHECKS, ALL CHECKS WERE NORMAL.
DURING APPROACH TO KSDF RT LG UNSAFE LIGHT REMAINED ILLUMINATED AFTER GEAR WAS SELECTED DOWN. PILOT ATTEMPTED TO EXTEND GEAR, USING EMERGENCY EXTENSION PROCEDURE WITH NO SUCCESS. PILOT RECYCLED GEAR, ON THE THIRD ATTEMPT ALL 3 GEAR LIGHTS TURNED GREEN. AIRCRAFT LANDED AT KSDF WITHOUT INCIDENT. MX TROUBLESHOOTING REVEALED RT MLG INBD DOWN POSITION SWITCH INTERMITTENT, NOTED SWITCH WAS R & R WITH SERVICEABLE SWITCH. OPERATIONAL CHECK WAS PERFORMED IAW FAIRCHILD MM 32-30-00 SYS CHECKED NORMAL.
DURING A ROUTINE INSPECTION, MAINTENANCE FOUND THE LT AILERON BELLCRANK CENTER HINGE HAD A SLIGHT CRACK. THE AIRCRAFT WAS REMOVED FROM SERVICE, INSPECTED AND FOUND SAFE FOR A ONE TIME FERRY FLIGHT FROM RAP TO SAT. UPON ARRIVAL AT SAT THE DEFECTIVE BELLCRANK WAS REMOVED AND REPLACED IN ACCORDANCE WITH FAC SRM 51-40-01.
FLT 2586 - BIS-FSD - LOST OIL PRESSURE ON LEFT ENGINE, SHUTDOWN ENGINE AND ELECTED TO LAND AT ABR. MAINTENANCE FOUND STRIPPED INSERT FOR OIL BYPASS BOLT. REPLACED INSERT IN ACCORDANCE WITH THE GARRETT MAINTENANCE MANUAL. PERFORMED RUNUP AND LEAK CHECK, NO DEFECTS NOTED. AIRCRAFT RETURNED TO SERVICE.
