WHILE AT ALTITUDE, FLIGHT CREW RECEIVED A CYAN "R ENG HYD PUMP FAIL" CAS MESSAGE AND 0% QUANTITY SHOWING ON THE RH HYDRAULIC SYSTEM. CREW RAN THE APPROPRIATE CHECKLIST AND DECLARED AN EMERGENCY WITH ATC. AFTER CONSULTING WITH MX CONTROL, THE DECISION WAS MADE TO DIVERT TO SLC, WHERE THEY LANDED WITHOUT INCIDENT. MAINTENANCE FOUND THE #2 BRAKE SOV LEAKING AT A HYDRAULIC LINE B-NUT. THE B-NUT WAS PROPERLY TORQUED AND LEAK CHECK WAS NORMAL. THE RH HYDRAULIC CASE DRAIN AND PRESSURE FILTERS WERE INSPECTED FOR DEBRIS, WITH NO DEFECTS NOTED. THE RH ENGINE HYDRAULIC PUMP (PN: 51160-06) WAS REPLACED WITH A REPAIRED UNIT, LEAK AND OPERATIONAL CHECKS WERE NORMAL. THE AIRCRAFT WAS RETURNED TO SERVICE.

ATA 2597

SHORTLY AFTER THE CABIN ATTENDANT USED THE GALLEY MICROWAVE OVEN, THE OVEN TURNED ITSELF ON UN-COMMANDED. THIS ACTIVATION WENT UN-NOTICED AND BEGAN TO BURN THE REMAINING FOOD WITHIN. A FEW MINUTES AFTER, THE CABIN ATTENDED BEGAN TO SMELL SMOKE AND NOTIFIED THE FLIGHT CREW. THE CAPTAIN GAVE THE CONTROLS OVER TO THE FO AND WENT TO ASSIST THE CABIN ATTENDANT. AFTER A BRIEF INVESTIGATION, THEY DISCOVERED THE SMELL AND SMOKE COMING FROM THE MICROWAVE OVEN. AFTER THE CREW READIED SAFETY EQUIPMENT, THEY PARTIALLY OPENED THE DOOR AND THE MICROWAVE CONTINUED TO OPERATE. THE FO CONFIRMED THE AMPERAGE DRAW CONSISTENT WITH THE MICROWAVE OPERATION. THE MICROWAVE OVEN WAS "STUCK" ON. THE CAPTAIN IMMEDIATELY PULLED THE MICROWAVE CONTROL CIRCUIT BREAKER AND THE OVEN CEASED OPERATION. ONCE ON THE GROUND, MAINTENANCE WAS SECURED TO INSPECT THE MICROWAVE. TECHNICIANS CYCLED THE BREAKER AND PERFORMED OPERATIONAL CHECKS WITH NO ANOMALIES NOTED. THE AIRCRAFT WAS SUBSEQUENTLY RETURNED TO SERVICE. TWO DAYS LATER THE CREW ONCE AGAIN REPORTED THAT THE MICROWAVE OVEN ACTIVATED ITSELF UN-COMMANDED. THE AIRCRAFT WAS REMOVED FROM SERVICE AND THE MICROWAVE IS CURRENTLY BEING REPLACED.

WHILE CLIMBING OUT AND APPROXIMATELY 1000 FEET AGL, A PASSENGER REPORTED SMOKE IN THE CABIN TO THE FLIGHT CREW. FLIGHT CREW CONFIRMED WHAT TO BE SMOKE. AN-IN FLIGHT EMERGENCY WAS DECLARED AND THE CREW RETURNED TO THE AIRFIELD. FIRE CREWS WERE REQUESTED AND AN UNEVENTFUL LANDING WAS ACCOMPLISHED. MAINTENANCE WAS CONTRACTED TO INSPECT AND REPAIR THE AIRCRAFT. TECHNICIANS DETERMINED THE AIR CYCLE MACHINE (ACM) HAD SEIZED. A REPAIRED ACM WAS INSTALLED, OPERATIONALLY CHECKED SATISFACTORILY AND THE AIRCRAFT SUBSEQUENTLY RETURNED TO SERVICE.

WHILE EN ROUTE, FLIGHT CREW RECEIVED AN AMBER WOW FAIL AND CYAN PROX SYS FAIL CAS MESSAGES. AFTER DISCUSSION WITH MX CONTROL, CREW ELECTED TO CONTINUE ON TO BED AND DECLARED AN EMERGENCY WITH APPROACH CONTROL, ADVISING THEM THAT THEY MAY NOT HAVE SUFFICIENT DIRECTIONAL CONTROL TO TAXI CLEAR OF THE RUNWAY. CREW LANDED WITHOUT INCIDENT AND WAS ABLE TO TAXI CLEAR OF THE RUNWAY. PER THE RESULTS OF THE MDC DOWNLOADS, THE PROXIMITY SENSOR ELECTRONIC UNIT (PSEU) WAS REPLACED ALONG WITH SWAPPING THE NR 1 & 2 SPOILER ELECTRONIC CONTROL UNITS (SECU) FOR MONITORING. OPERATIONAL CHECKS WERE NORMAL OF THE PSEU & SECU, NO DEFECTS NOTED. THE AIRCRAFT WAS RETURNED TO SERVICE.

THE MANUFACTURER ADVISED THAT THEY HAD OBSERVED UNUSUAL AND SUDDEN SHIFTS IN OIL PRESSURE INDICATIONS, ALTERNATING BETWEEN POSITIVE AND NEGATIVE READINGS. THE MANUFACTURER ADVISED THAT THE CARBON SEAL SHOULD BE ACCESSED FOR IMMEDIATE INSPECTION. THE AIRCRAFT WAS PROMPTLY PLACED INTO MAINTENANCE. UPON ACCESS INTO THE NR 4/5 SUMP, PARTICLES WERE RIGHT AWAY OBSERVED WITHIN THE LPT STUB SHAFT AND WERE DETERMINED TO BE PIECES OF THE FORMER NR 4 CARBON SEAL ELEMENT. FURTHER INVESTIGATION REVEALED THE NR 4 CARBON SEAL CARRIER WAS MISSING ITS CARBON ELEMENT ALONG WITH HEAT DISCOLORATION NOTED ON THE LPT STUB SHAFT IN THE AREA OF THE NR 4 CARBON SEAL. THE NR 4 AND 5 CARBON SEALS WERE REPLACED WITH NEW. THE NR 4 ROLLER BEARING WAS REPLACED WITH AN IMPROVED PART NUMBER ROLLER BEARING DURING COMPLIANCE OF SERVICE BULLETIN AS907-72-9080. THE LPT STUB SHAFT WAS REPLACED WITH AN OVERHAULED UNIT. THE ENGINE/AIRCRAFT HAS SINCE BEEN RETURNED TO SERVICE.
