Maintenance disclosures
THE PILOT REPORTED WHILE ON FINAL APPROACH INTO KSMX THAT THE AIRCRAFT MADE AN UNCOMMANDED NOSE PITCH DOWN ATTITUDE AT APPROXIMATELY 200â AGL. THE PILOT DECLARED AN EMERGENCY THEN LANDED WITHOUT INCIDENT IN KSMX. MAINTENANCE INSPECTED THE AIRCRAFT AND FOUND THAT WATER HAD ACCUMULATED IN THE AIRFRAME BELLY, WHERE THE STALL AVOIDANCE SYSTEM SERVO IS LOCATED. MAINTENANCE REMOVED THE WATER. MAINTENANCE INSPECTED AND CLEANED THE CONNECTOR TO THE SAS SERVO AND GROUND OPERATIONAL TEST WERE NORMAL. THE AIRCRAFT WAS RELEASED FOR AN OPERATIONAL CHECK FLIGHT. THE RESULTS WERE UNSUCCESSFUL AND FURTHER TROUBLE SHOOTING AND FUNCTIONAL TEST ARE REQUIRED. A SUPPLEMENTAL REPORT WITH FINAL CORRECTIVE ACTION WILL HAVE TO BE SUBMITTED.
THE PILOT WAS ENROUTE, WHEN BOTH LEFT AND RIGHT HYDRAULIC PRESSURE LOW ANNUNCIATORS ILLUMINATED. THE PILOT NOTED HYDRAULIC PRESSURE DROPPING BELOW NORMAL LIMITS. THE PILOT NOTIFIED COMPANY DISPATCH AND ARRANGED TO RETURN TO DEPARTURE. THE PILOT FOLLOWING EMERGENCY PROCEDURES PERFORMED A MANUAL LANDING GEAR EXTENSION AND LANDED WITHOUT INCIDENT. INSPECTED THE HYDRAULIC SYSTEMS AND FOUND THAT A HYDRAULIC LINE TO THE NOSE LANDING GEAR UP CIRCUIT HAD A SMALL PIN HOLE. MAINTENANCE REMOVED AND REPLACED THE LINE WITH A FABRICATED SERVICEABLE UNIT. IAW SA 227 AMM 20-40-00. MAINTENANCE REMOVED AND REPLACED BOTH LEFT AND RIGHT ENGINE DRIVEN HYDRAULIC PUMPS WITH SERVICEABLE UNITS AND FLUSHED THE HYDRAULIC SYSTEM. THE MAIN SYSTEM FILTER WAS REPLACED WITH NEW. MAINTENANCE PERFORMED GEAR SWINGS AND GROUND RUNS AND LEAK AND OPERATIONAL CHECKS WERE NORMAL. THE AIRCRAFT WAS RETURNED TO SERVICE AND NO SUBSEQUENT ISSUES HAVE BEEN REPORTED.
DURING APPROACH, THE PILOT SELECTED THE LG TO THE EXTEND POSITION, THE PILOT RECEIVED A THREE GREEN DOWN & LOCKED LG INDICATOR LIGHTS INITIALLY, SHORTLY AFTER THE RT LG LIGHT EXTINGUISHED. PERFORMED A PRECAUTIONARY MANUAL EXTENSION OF LG TO VERIFY LG WAS DOWN & LOCKED, LANDED WITHOUT FURTHER INCIDENT. TROUBLESHOOTING FOUND FAULTY LG POSITION INDICATOR. REPLACED INDICATOR & PERFORMED LG OPERATIONAL CHECK. SYS CHECKS WERE NORMAL.
DURING PRE-TAKEOFF CHECKS OF ACFT SYS WHILE TAXIING, PILOT NOTICE THE NOSE WHEEL STEERING WAS SLOW TO RESPOND. ACFT WAS APPROACHING A TURN & PILOT ATTEMPTED TO APPLY BRAKES, RT BRAKE PEDAL BROKE & PILOT FOOT SLIPPED WHICH RESULTED IN PILOT VEERING OFF TAXIWAY INTO UNPAVED FIELD AREA ADJACENT TO TAXIWAY & SHUTTING DOWN ENG. THIS RESULTED IN DAMAGE TO PROPELLER. INSPECTED ACFT & DETERMINE BRAKE MASTER CYLINDER ROD BROKE ALSO LT ENG SUSTAINED A PROPELLER STRIKE. IN THE PROCESS OF REMOVING & REPLACING LT ENG, LT ENG PROPELLER & PILOT RT BRAKE MASTER CYLINDER ROD.
ON INITIAL DESCENT, PILOT SMELLED SMOKE IN THE COCKPIT, CHECKED ANNUNCIATOR PANEL AND FOUND EVIDENCE OF SMOKE. PROCEEDED WITH QUICK REFERENCE HANDBOOK PROCEDURE AND DECLARED AN EMERGENCY. TURNED RADAR OFF, SMOKE APPEARED TO STOP. LANDED WITHOUT FURTHER INCIDENT. MAINTENANCE FOUND POWER SUPPLY PC BOARD WITHIN RADAR INDICATING UNIT BURNED. R & R RADAR INDICATING UNIT, OPS CHECKED GOOD.
MFE-SAT - FLT 1851 - AFTER TAKEOFF, THE RIGHT MAIN GEAR SHOWED INTRANSIT LIGHT REMAINED ON. FLIGHT CREW CYCLED GEAR WITH NO CHANGE IN INDICATION. AIRCRAFT RETURNED TO MFE AND LANDED WITHOUT FURTHER INCIDENT. MAINTENANCE FOUND UPLOCK ROLLER BEARING FROZEN. REMOVED AND REPLACED UPLOCK ROLLER IN ACCORDANCE WITH THE FAIRCHILD AIRCRAFT MAINTENANCE MANUAL. FUNCTIONAL CHECK GOOD AND AIRCRAFT RETURNED TO SERVICE.
MAF - LEFT MAIN GEAR WOULD NOT FULLY RETRACT ON TAKEOFF, ATTEMPTED 4 GEAR CYCLES WITH NO LUCK. A NORMAL LANDING WAS MADE AT THE DEPARTURE POINT. MAINTENANCE INSPECTED AIRCRAFT, REMOVED AND REPLACED THE LEFT MAIN GEAR LANDING GEAR UP LIMIT SWITCH. INSTALLED NEW SWITCH PART NUMBER ML1092 AND PERFORMED GEAR SWING IN ACCORDANCE WITH THE FAIRCHILD AIRCRAFT INCORPORATED MAINTENANCE MANUAL 32-30-00. OPERATIONAL CHECK GOOD, AIRCRAFT RETURNED TO SERVICE.
ON FLIGHT 1851 IN CRUISE THE RIGHT WING OVERHEAT LIGHT CAME ON. AIRCRAFT TURNED BACK AND RETURNED TO MFE WITHOUT FURTHER INCIDENT. MAINTENANCE TROUBLESHOT SYSTEM AND FOUND MOISTURE IN THE WING ROOT CONNECTOR FOR THE WING OVERHEAT LIGHT SYSTEM. CLEANED AND DRIED THE CONNECTOR. PERFORMED SYSTEM FUNCTIONAL CHECK, NO DEFECTS NOTED. AIRCRAFT RETURNED TO SERVICE.
