Maintenance disclosures
AFTER TAKEOFF, THE PILOT INCREASED THE CABIN TEMPERATURE SETTING TO HOT AND NOTED SMOKE IN THE COCKPIT A FEW MOMENTS LATER. T DUMPED CABIN PRESSURE TO EXPEL THE SMOKE, DECLARED AN EMERGENCY AND LOWERED THE CABIN TEMPERATURE SETTING. SMOKE WAS NO LONGER NOTED AND THE AIRCRAFT LANDED WITHOUT INCIDENT. MAINTENANCE TIGHTENED A RIGHT CONSOLE TUBING OUTLET CONNECTION AND PERFORMED GROUND OPERATIONAL CHECK WITH THE CABIN TEMPERATURE SETTINGS IN THE HOT POSITION. SMOKE FROM THE RIGHT SIDE CONSOLE TUBING WAS NOTED FOR A FEW MINUTES AND THEN STOPPED. NO FURTHER OBVIOUS DEFECTS WERE NOTED. THE AIRCRAFT WAS RETURNED TO SERVICE. THE SMOKE SEEMED COME FROM AN OILY PRESERVATIVE ON THE RIGHT CONSOLE TUBING.
ON APPROACH TO KBGM, SELECTED GEAR TO THE DOWN POSITION, THE NLG WAS NOT INDICATING DOWN AND LOCKED. CYCLED THE GEAR WITH THE SAME RESULTS. PERFORMED A FLYBY OF THE CONTROL TOWER AND THEY CONFIRMED THE NLG WAS NOT DOWN AND LOCKED. PERFORMED A MANUAL GEAR EXTENSION AND RECEIVED ALL THREE GREEN DOWN AND LOCKED INDICATION, PERFORMED ANOTHER FLY BY AND ATC CONFIRMED ALL THREE LANDING GEAR APPEARED TO BE IN THE DOWN POSITION. LANDED AT KBGM WITHOUT INCIDENT. AIRCRAFT WAS FERRIED TO KBUF FOR GEAR SWINGS ON JACKS. MX INSPECTED, CLEANED AND LUBRICATED THE NOSE LANDING GEAR ASSEMBLY. PERFORMED 12 GEAR SWINGS WITHOUT DEFECT, THE AIRCRAFT WAS RELEASED FOR SERVICE. NO GEAR ISSUES REPORTED ON SUBSEQUENT FLIGHTS.
DURING CRUISE FLIGHT AT FL200 PILOT NOTICED WHITE SMOKE IN COCKPIT, PILOT DONNED OXYGEN MASK & SMOKE GOGGLES, NOTIFIED ATC OF SITUATION & PERFORMED EMER PROCEDURES IN QRH. PILOT TURNED OFF BOTH BLEED AIR SOURCES & STARTED EMER DESCENT, PILOT LANDED WITHOUT FURTHER INCIDENT. TROUBLESHOOTING REVEALED THE LT ACM HAS AN INTERNAL LEAK CAUSING THICK WHITE SMOKE WITH BLEED AIR SYS ON. DEFERRED ACM, WE WILL SUBMIT A FOLLOW UP REPORT AFTER WE RECEIVE THE TEAR DOWN REPORT.
AFTER TAKEOFF PILOT REPORTED THAT THERE APPEARED TO BE SMOKE IN THE COCKPIT. THE PILOT PROCEEDED WITH THE EMERGENCY CHECKLIST AND CONCLUDED IT WAS COMING FROM THE BLEED VENTS. THE PILOT PROCEEDED TO TURN OFF THE BLEED AIR AND USED THE CABIN DUMP SYSTEM TO CLEAR THE SMOKE. THE PILOT DECLARED AN EMERGENCY, RETURNED TO SDF AND LANDED WITHOUT INCIDENT. SDF MAINTENANCE INSPECTED THE AIRCRAFT BLEED AIR SYSTEM GROUND RUN THE ENGINES WITH THE BLEED AIR AT MAX HOT, COULD NOT DUPLICATE THE SMOKE IN THE COCKPIT. THE PILOT REPORTED TO MAINTENANCE THAT THE SMOKE WAS ODORLESS AND SEEMED MORE APPARENT FROM THE RIGHT BLEED AIR SYSTEM. AS A PRECAUTION THE RIGHT BLEED AIR SYSTEM WAS DEFERRED PER THE MEL 21-20-00. CVG MAINTENANCE ON 10/26/2011 INSPECTED THE AIRCRAFT BLEED AIR DUCTS AND TUBING FOR EVIDENCE OF HEAT DAMAGE NO DAMAGE WAS FOUND. GROUND RUN COMPLETED WITH THE BLEED HEAT ON MAX HEAT FOR 20 MINUTES NO EVIDENCE OF SMOKE NOTED. NO EVIDENCE OF SMOKE HAS BEEN REPORTED ON SUBSEQUENT FLIGHTS. SUSPECT CONDENSATION IN THE BLEED AIR SYSTEM GAVE THE VISUAL EFFECT OF SMOKE.
DURING TAXI OUT FOR FLIGHT 1971, MAF TO DFW, THE LEFT BRAKE WAS DRAGGING AND BECAME MORE NOTICABLE THE FURTHER THE AIRCRAFT TAXIED. THE FLIGHT CREW RETURNED TO THE GATE. MAINTENANCE TROUBLESHOT THE BRAKES AND FOUND THE LEFT BRAKES NOT RELEASING. REMOVED AND REPLACED THE LEFT INBOARD AND OUTBOARD BRAKE ASSEMBLIES IN ACCORDANCE WITH THE FAIRCHILD AIRCRAFT INCORPORATED MAINTENANCE MANUAL. FUNCTIONAL CHECK GOOD. AIRCRAFT RETURNED TO SERVICE.
DURING TAXI WHEN THE SPEED LEVER WAS ADVANCED TO HIGH, THE LEFT WING OVERTEMP LIGHT FLICKERED. AIRCRAFT WAS FERRIED TO SAT. REPAIRED WIRE FROM FUSELAGE CONNECTOR TO BLEED AIR FAIL SENSOR IN ACCORDANCE WITH THE FAIRCHILD AIRCRAFT INCORPORATED WIRING MANUAL 26-10-02. FUNCTIONAL CHECKED AND FLIGHT CHECKED SATISFACTORY. AIRCRAFT RETURNED TO SERVICE.
