R/H ATTENUATOR BELCRANK (CASTING) FAILED AT CENTER PIVOT FRAME RENDERING THE ATTENUATOR DOOR INOP AND AND GIVING AN UNSAFE ANNUNCIATOR LIGHT WHICH ALLOWED HYDRAULIC SYSTEM TO CONTINUE TO RUN WHERE NORMALLY AN DUMP/UNLOADING VALVE WOULD ENERGIZE AND ALLOW FLUID TO BYPASS. THANKFULLY AIR CREW ABORTED TAKEOFF AND RETURNED FOR MAINTENANCE. UPON INSPECTION OF COMPONENTS, I OBSERVED THE HYDRAULIC TANK AND SURROUNDING AREA EXTREMLY HOT DUE TO SYSTEM NOT BEING UNLOADED.
THE AFT EVAPORATOR BLOWER PN 1133210-7 SN 1509 TO BE INOPERATIVE. AFTER 2 MONTHS & 51 HOURS OF USE, THIS IS THE FOURTH BLOWER MOTOR THAT HAS FAILED IN 4 MONTHS & 80.4 HOURS OF ACFT OPERATION. ALL SUPPLIED PARTS, SAME LOT NR ON ALL MOTORS.
THE CREW REPORTED THAT DURING PREFLIGHT CHECKS THAT THE AFT AIR CONDITIONING EVAP BLOWER FAILED. THIS WAS THE THIRD FAULT WITH THIS AFT EVAP BLOWER SYS. THIS DISCREPANCY WAS WRITTEN AFTER 6.9HRS OF OPERATIONS AFTER THE AFT BLOWER ASSEMBLY WAS REPLACED WITH NEW. TROUBLESHOOTING COULD NOT DETERMINE A CAUSE OF THE FAULT, THE SYS WAS OPERATING CORRECTLY. IN AN EFFORT TO DETERMINE THE FAULT THE ACFT SELECT SWITCH S1081 & THE AFT EVAPORATOR FAN SWITCH S1080 WERE REPLACED. AFTER 12 HOURS OF OPERATIONS NO DEFECTS HAVE BEEN REPORTED.
CREW FOUND DURING THE PREFLIGHT INSPECTION THAT THE CABIN BLOWER WAS INOPERATIVE. THE FAILED BLOWER HAD A TOTAL OF 22.7 HRS FROM INSTALLATION. THE FAILED AFT BLOWER WAS REMOVED PN 1133210-7, SN 1506. INSTALLED A NEW MOTOR SN 1508, OPERATIONS CHECKED OK.
THE CREW REPORTED THAT THE AFT EVAP CB TRIPPED & WOULD NOT RESET. TROUBLESHOOTING FOUND THE AFT EVAP MOTOR DEFECTIVE. THE UNIT WAS REMOVED PN 1133210-7, SN 0209 & REPLACED WITH NEW UNIT.
