DURING TAKE OFF FROM KSLC THE PILOT SELECTED THE LANDING GEAR TO THE UP POSITION AND UPON RETRACTION OF THE GEAR RECEIVED A LEFT HAND HYDRAULIC LOW LEVEL AND HYD ILLUMINATION WARNING. THE PILOT NOTIFIED ATC OF THE CONDITION AND DECLARED AN EMERGENCY. THE PILOT THEN FOLLOWED THE QRH PROCEDURES AND LANDED BACK IN KSLC WITHOUT INCIDENT. MAINTENANCE INSPECTED THE LEFT HAND GREEN HYDRAULIC SYSTEM AND FOUND THAT THE HYDRAULIC FLUID LEVEL WAS LOW. MAINTENANCE SERVICED THE GREEN HYDRAULIC SYSTEM WITH FLUID PER AMM CHAPTER 12-13-01-3 SECTION 6. THE FLUID QUANTITY CHECK WAS WITHIN LIMITS AND THE AIRCRAFT WAS RETURNED TO SERVICE AND NO SUBSEQUENT ISSUES HAVE BEEN REPORTED.

ATTEMPTED ONE RESET AND SAME THING HAPPENED AGAIN. BOTH TIMES THE CB'S POPPED AS AIRCRAFT ENCOUNTERED TURBULENCE. DIVERTED TO SFO. INSPECTED OVERHEAD CB'S PANEL, LEFT AND RIGHT DC RELAY BOXES. NO DEFECT FOUND. REPLACED EMERGENCY DC BUS 2 OFF RELAY K0440. RESET CB'S. OPS CHECK GOOD ON ENGINE RUN. UNABLE TO DUPLICATE DISCREPANCY.

FOUND CAPTAIN''S DV WINDOW RELEASE LATCH GUMMED UP. CLEANED AND LUBRICATED. OPS CHECK OF CAPTAIN''S DV WINDOW SATISFACTORY IAW CH 56-10-00.
ENTIRE LEADING EDGE DEICE SYSTEM FAILED AT CRUISE. UPON INSPECTION FOUND A LARGE TEAR IN THE UPPER PORTION OF THE LEFT OUTBOARD BOOT. DIVERTED FROM RDM TO RDD. COMPLIED WITH REPLACEMENT OF LEFT OUTBOARD DEICE BOOT. OPS CHECK AND FUNCTIONAL CHECK GOOD OF LEFT OUTBOARD BOOT. (P/N ON: P29S-705135-29) (S/N ON: MM07380).

LEFT DUCT LEAK CAME ON SHORTLY AFTER TAKEOFF. SHUT DOWN NR 1 ENGINE AND THE LIGHT WENT OUT. RETURNED TO SAN. REMOVED INBOARD LEADING EDGE AND WING ROOT FAIRINGS (LEFT). INSPECTED ALL LINES, DUCTS AND CLAMPS, NO LEAKS NOTED. OHMED OUT ALL DUCT SENSORS, NO DEFECTS NOTED. CHECKED WATER SEPARATOR AND 18 PSI REGULATOR, NO LEAKS NOTED. COULD NOT DUPLICATE PROBLEM. OPS CHECKS GOOD.
IN FLIGHT ENGINE SHUT DOWN NR 1 DUE TO LEFT DUCT LEAK LIGHT ON. REMOVED LEFT INBOARD LEADING EDGE. INSPECTED ALL CLAMPS AND TUBING. INSPECTED ALL OVERWING CLAMPS AND TUBING AND PACK AREA. RAN LEFT ENGINE FOR 10 MINUTES AND INSPECTED FOR LEAKS. NO LEAKS FOUND, OPS CHECK GOOD.
