DURING SCHEDULED NDT INSPECTION A CRACK WAS FOUND ON THE RIGHT MAIN LANDING GEAR AXLE. THE LOWER LEG AND AXLE WERE REPLACED WITH NO FURTHER DEFECTS NOTED.
DURING SCHEDULED NDT INSPECTION A CRACK WAS FOUND ON THE LEFT MAIN LANDING GEAR AXLE. THE LOWER LEG AND AXLE WERE REPLACED WITH NO FURTHER DEFECTS NOTED.
PILOT REPORTED STRONG SMELL OF FUEL IN THE CABIN WITH THE HEATER ON. MAINTENANCE INSPECTED AND FOUND THE HEATER FUEL PUMP TO BE LEAKING. THE PUMP WAS REPLACED WITH NO FURTHER DEFECTS NOTED.
PILOT REPORTED STEADILY DECREASING OIL PRESSURE AT LT ENGINE. CREW INTENTIONALLY CAGED LT ENGINE WHEN âMECHANICALâ NOISES WERE HEARD AND SAFELY LANDED. INITIAL INSPECTION FOUND MULTIPLE CRANKCASE BREACHES AND INDICATIONS OF POSSIBLE OIL STARVATION. ADDITIONAL DETAILS WILL BE AVAILABLE UPON ENGINE TEARDOWN INSPECTION.
PILOT REPORTED LEFT WING LOCKER DOOR OPENED DURING TAKE OFF. MAINTENANCE INSPECTED AND FOUND THE LEFT WING LOCKER LATCH NEEDED ADJUSTMENT. ADJUSTED AS NEEDED WITH NO FURTHER DEFECTS.
PILOT REPORTED THE AVIONICS SWITCH BREAKER TRIPPED IN FLIGHT. MAINTENANCE REMOVED A REPLACED THE AVIONICS BUSS CIRCUIT BREAKER AND OPERATIONALLY CHECKED OK WITH NO FURTHER ISSUES.
THE RIGHT NACELLE CROSSOVER BEAM ASSEMBLY WAS FOUND TO BE CRACKED AT AFT FACE IN THE AREA OF TURBOCHARGER MOUNT BRACKET HOLE LOCATIONS DURING DURING DISASSEMBLY OF TURBO CHARGER ASSEMBLY TO FACILITATE OTHER MAINTENANCE. CRACK LOCATED ADJACENT TO OB MOST TURBOCHARGER BRACKET HOLE LOCATION. CORRECTIVE ACTION WILL BE TO REMOVE CROSSOVER BEAN ASSEMBLY, PERFORM DETAILED VISUAL INSPECTION OF SURROUNDING STRUCTURE AND INSTALL NEW BEAM ASSEMBLY.

RIGHT PROP CONTROL LEVER, WILL NOT CHANGE PROP PITCH. INSPECTED & FOUND RIGHT PROP CONTROL CABLE DISCONNECTED FROM PROP GOVERNOR. REPLACED RT PROP CABLE ROD END BOLT & NUT WITH NEW IAW MM CH 61-20-00 & 20-10-01.
PILOT REPORTED BURNING SMELL, NOTED WITH CABIN HEATER SWITCH ON. INSPECTED & OPERATIONS CHECKED HEATER OPERATIONS. FOUND DUCT THERMOSTAT SWITCH OUT OF ADJUSTMENT, RUNNING TOO HOT, ADJUSTED SWITCH TO WITHIN LIMITS. OPERATIONS CHECKED GOOD.
PILOT REPORTED BURNING INSULATION SMELL IN COCKPIT. INSPECTED LT SIDE CONSOLE, CB PANEL, BEHIND INSTRUMENT PANEL, FOUND NO SIGN OF OVERHEATED OR BURNT WIRES & NO POPPED CB.
PILOT REPORTED THE NOSE GEAR WOULD NOT INDICATE DOWN DURING NORMAL OR EMER EXTENSION. INSPECTED NOSE GEAR EXTENSION & RETRACTION SYS & FOUND NLG ACTUATOR GND WIRE BROKEN AT RING TERMINAL. ATTACHED NEW RING TERMINAL TO NLG ACTUATOR GND WIRE IAW 402C WDM CH 20-10-04. ROUTED & REATTACHED GND WIRE TO BULKHEAD IAW 402C WDM CH 20-10-02. PERFORMED LG FUNCTIONAL CHECK IAW 402C MM CH 32-00-00. OPERATIONALLY CHECKED GOOD AT THE TIME. SERVICED LG EMER BLOW DOWN BOTTLE IAW 402C MM CH 12-10-06.
CRACKS FOUND FOR/AFT AREA PREVIOUSLY ADDRESSED IN MEB 84-12. REPAIR CONDUCTED IAW 8110-3 FAA APPROVED DATA, REF 135415A(14).
ENROUTE LT ENGINE BEGAN TO RUN ROUGH, LANDED. RUN UP FOUND LT ENGINE MAGNETO ROUGH AND EXCESSIVE DROP. (SW01200603302) (K)
CRACKED AT AILERON YOLK ATTACH POINTS.
DURING A RETURN FLIGHT, LEFT AND RIGHT HYDRAULIC FLOW LIGHTS BEGAN TO FLICKER ON AND OFF. CYCLED GEAR DOWN AND MADE AN UNEVENTFUL LANDING. MAINTENANCE PERSONNEL FOUND HYDRAULIC RESERVOIR EMPTY. PERFORMED HYDRAULIC PRESSURE CHECK AND FOUND L-H PRESSURE LINE, FROM THE FLOW SWITCH TO ELBOW, CRACKED IN THE BEND OF THE RADIUS. REPLACED WITH SERVICEABLE LINE. PRESSURE CHECKED AND CYCLED GEAR WITH NO FURTHER LEAKS NOTED.
